A) Final Amendments Final amendments started to be released on July 18, 2014. 7 - 16% of the Local Health Departments (LHDs) have signed their Final

amendment. 13 - 29% of the LHDs final amendments are pending LHDs action. 6 - 13% of the LHDs still are working on Amendment 4

B) Financial Status Report’s (FSR) for Fiscal Year 2014

74% were submitted by the due date. 17% were submitted within 30 days of the due date.

C) Obligation Reports: Path: Grantee>Reporting>Progress Reports

Available on September 13, 2014. Due date for submission to MDCH through the MI E-Grants system is September

23, 2014. Agencies should enter their best estimate of source of state funds for the remaining

reporting period.

November 30, 2014: Due Date for Final FSRs for the WIC programs. December 15, 2014: Due Date for Final FSRs for remaining projects.

E) Local Maintenance Of Effort (LMOE)

Actual LMOE reports for FY 2014 are due March 31, 2015. Actual LMOE reports will be available to complete by going to

“Grantee>Reporting>Progress Reports” and selecting Grant Program CO-2015 Comprehensive Agreement- 2015.

Started releasing project applications on August 28, 2014 Application Submission Due Date: September 17, 2014. The Authorized Official should submit projects on the “Grantee>Grant

Application>Grant Application Preview” screen.

E) Local Maintenance of Effort Projected LMOE report will be available to complete as soon as you sign your

agreement by going to “Grantee>Reporting>Progress Reports”. This is an attachment report in the Hearing Project similar to FY 2014. Report due date is November 1, 2014.

F) First Amendment

Planned Release date for Local Health Departments is October 28, 2014.
