Novemberhasbeenanotherbusymonth.
HKCFP was one the three Colleges under HKAM being invited toparticipateintheacademicexchangesymposiumentitled“InnovationsinResidencyTraining in theUniversityofChicagoandHongKong”whichwasco-hostedbytheHongKongAcademyofMedicineandtheUniversityofChicago.ThesymposiumwasheldattheHKAMbuildingonMonday2November,2015andtheparticipantswerespiltintothreegroupswiththreespecialtyCollegesas facilitators:FamilyPhysicians,Surgeons,andPhysicians.

OurCollegewasrepresentedbyDr.RubyLeeandDr.WilliamWongasspeakersandDr.DavidChaoasthefacilitatorforthesession“InnovationsinFamilyMedicine/GeneralPracticeResidencyProgrammes”togetherwithspeakersfromUniversityofChicago,namelyDr.MariEganandDr.MarkPotter.BoththeHKCFPandtheUniversityofChicagopresentedthecharacteristics,commonalitiesanddifferencesoftheirFamilyMedicinetrainingprogrammesandexchangedviewsonthefuturedirectionsoftrainingstructureandcontents.OtherColleges likePaediatriciansandEmergencyMedicine joinedthissubgroupfordiscussiontoenhancetheir trainingprogrammes. Itwasaveryenlighteningexperience tosharewithalltheparticipantsofthesymposium.Icancategoricallysayourtrainingisamongstthebestintheworld!
HKAMhostedalecturedeliveredbyDr.AtulGawande,MD,MPH,whoisarenownedauthor,surgeonandpublichealthresearcherfromtheU.S.,titled"BeingMortal:MedicineandWhatMatters intheEnd"on12thNovember2015.Dr.Gawandebeganhislecturebysharinghispersonalexperienceforendoflifecare.Herecognizedthatwell-beingformostpeopleislargerthanmeresurvival.
His humanistic approach in end of life care is in fact an excellentreflection of what we have always aspired to in Family Medicine:quintessential patient-centred care. I am surprised to learn 50%ofAmericanspassedaway indignity inhospicesasa resultof theObamacare.HongKonghasa long longway togo. In fact, familydoctorsinothercountriescontributesubstantiallyinendoflifecareandsomeareevenheadofhospices.Dr.Gawandehasawebsitededicatedto themedicalcommunity, theAriadneLabs (www.ariadnelabs.org)

Old and New Looks of GOPC – Kowloon East Cluster Kwun Tong Jockey Club General Outpatient Clinic and
Kwun Tong Community Health Centre
The Kwun Tong Jockey Club General OutpatientClinic(KTJCGOPC)wasestablishedin1964andhadservedthecommunityformorethanhalfacentury.InMarch2015, itwas relocated toYuetWahStreetandrenamedas theKwunTongCommunityHealthCentre(KTCHC).ThisprojectwasmadepossibleasitwaspartoftheKwunTongDistrictredevelopmentplan.
KTCHC ismanagedby theDepartmentofFamilyMedic ine and Pr imar y Health Care under theKowloon Eas t C lus te r (K EC) of the Hos pi ta lAuthority. It is the firstCHC located in theurbanarea and is developed according to the PrimaryCare Development Plan s tated in Hong KongStrategyDocumentpublishedin2010.Improvementin facilities,environmentandworkflowareaimedatenhancing theprovisionofqualityprimarycareservicestopatientsinthecommunity.

In order to enhance the gatekeeping role
of Family Physicians, a triage clinic was set
up at KTCHC to manage routine referrals
of predefined surgical and or thopaedics
condit ions f rom KEC general outpatient
clinicsandotherreferrerstotheSurgicaland
Orthopaedics Specialist Outpatient Clinics
(SOPCs) of the United Christian Hospital.
Comprehensiveassessmentandmanagement
includingadditionalhospital investigations,
expanded drug formular y and ef fec t ive
referral systemtoSOPCareprovidedby the
clinic.

With the ageing population, it is expected

iv) Nurse and Allied Health Clinics (NAHCs)
Multidisciplinary team in NAHCs provides
assessment and management of common
health problems, namely wound care, fall
prevention, continencecareand respiratory
rehabilitation.
v) Smoking Counseling and Cessation Service In thepast,patients fromKwunTonghad to
gotoclinics inotherareassuchasNgauTau
Kokforsmokingcessationservice.Thisservice
isnowestablishedinKTCHC.Multidisciplinary
team of trained anti-tobacco nurses and
doctorswouldofferprofessionalcounseling,
follow-up services and pharmacotherapy if
required.

51yearsofactiveserviceandbecameapiece
of precious memor y of our patients and
colleagues (Fig.10).KTCHCwillcontinue the

The Hidden Diabetic Patients
Hiddendiabeticpatients,atermI recentlyacquaintedwith, referstothosepatientswithdiabeteswhoremainedundiagnosed.Doyouhaveanyideahowmanyofthemarethereinthecommunity?
On4thOctober,2015, I representedourCollegetospeakondiabetes inthecommunity inapressconferenceorganizedbyDiabetesHongKong. Inthepressconference,Dr.LiKaFai,ChairmanoftheDiabetesHongKong,highlightedthat therearecurrently300,000hiddendiabeticpatients inHongKong. IechoedDr.Li’s figurewiththedataextractedfromtheThematicHouseholdSurveyReportNo.50, released in2012. Itstatedthatonly5%people inHongKongwasknowntohavediabetes.However,thewellquotedprevalencefordiabetesinHongKongis10%.Thisrepresentedthat5% of the population might have “hidden diabetes”.
TheGovernmenthas recently released the latestTHSReportNo.58 inmid-October,2015.Themostupdateddataisthat331,300residentsinHongKonghaddiabetes,equivalentto4.8%ofthepopulation.Thatmeanstherearestillmorethan350,000people inHongKongwithundiagnoseddiabetes!Threeyearsdowntheroad,nothinghaschanged!
This isanastonishingfigure.Thesepatients,upontheir firstpresentation,mayhavesufferedfrommyocardial infarction,strokeor legamputation.We,family doctors, advocates of continuity of care, preventive and comprehensive care, are at a pivotal position to help identifying these hidden patients through our daily clinical encounters.
Accordingto theHongKongReferenceFramework forDiabetesCare forAdults inPrimaryCareSettingswhichadvocatesforrisk-basedscreeningapproach,thegeneralpopulationwithageatorabove45yearsofage;thosewithhypertension;subjectswhoareoverweight;andindividualswithfamilyhistoryofdiabetes…aresuitabletargetsfordiabetesscreening.
Todate, therearesome1,500doctors registered in thePrimaryCareDirectory (PCD).SupposeeachdoctorinthePCDisabletomotivateonesuchat-riskpatienteveryweektoundergodiabetesscreening,heorshewillbeableto identify5to10diabetespatientseveryyear.Eachyear,upto15,000morediabetespatientswillbeidentifiedandofferedprompttreatment.With350,000hiddendiabeticpatients,such“rateofdiagnosis”iscertainlystillfarfromideal,giventhefactthattherewillbepopulationgrowthaswell.IbelievetheGovernmenthasacompellingobligationtodealwiththisissue,beitinmotivatingmorecolleaguestojoininthePCDorincentivizingthepublictoinvolveinevidence-basedpreventivecareactivities.
November14eachyear is theWorldDiabetesDay.Take thisdayas themilestone, and startidentifyingourhiddendiabetespatients.Threeyears later,when theGovernment released itsnewTHSreport,perhapswemaybeable tochangetheunhealthysituationonhiddendiabeticpatients.Ofcourse, identification is justtheveryfirststep.Howtobettermanagesuchpatients iscomplicatedwhichrequiresawarenessandactionsfromthepublicandprivatesectors.

2015 Resuscitation Council of HK (RCHK) Cardiopulmonary resuscitation (CPR) skills are essential for all healthcare professionals. CPR saves lives and it would be beneficial if more general public also acquires this skills. It is with this vision that the Resuscitation Council of Hong Kong (RCHK) is promoting bystanders CPR. As a founding member of RCHK, we are delighted to share with you the opening speech by Dr. Mary Kwong, Chairlady of RCHK in the 4th Annual Scientific Meeting of RCHK, jointly organized with HK Red Cross, the International Federation of Red Cross and the Red Crescent Societies.

Howtoimprovethesurvivalrate&thesurvivaloutcomeofapersonsufferingsuddencardiacarrestout-of-hospital,isagreatchallengetoResuscitation.I,personally,hadapainfulrecall.Nooxygen,nobloodflowtothebrainforeveryminuteofdelayinresuscitation,meansmoredamagetothevictim,totheanoxicbrain.Toimprovethesurvivalrateandneurologicaloutcome,immediateresuscitationbyatrainedlaypersonnear-by,whatwecallbystanderCPRorbystandercardiopulmonaryresuscitationbeforearrivalofambulanceisaMUST.CPRseemsveryintimidatingtothepublic.Then,howcanweincreasethebystanderresuscitationrate?Theansweristoprovidethemtrainingandknowledge,empowertheirconfidence,applaudtheirattitudeandwillingnesstodoCPR,andprotectthembyGoodSamaritanLaw.AllourInstitutionmembersarecontinuouslyencouragingCPRtrainingtoallmembersandlaypeopleinthepublic.Morethanthat-Iamdelightedtoreportthat:

2. RCHKsupports“MassCommunityCPRTraining”.ThesuccessfuleventwasorganisedbyTseungKwanOHospital,&SaiKungDistrictCouncil,heldinJan2015with260participantsfromthepublic.

IappreciatethevisionsoftheseHKforerunners,theircontributionsshouldberecognisedinternationally,ifwehavearepresentativeinRCAorILCOR.IamferventlyhopingthatRCHKwillachievethisrole.
