www.metroplusmedicare.org. For any questions about the information contained in this
directory, please call our Member Service Department at 1-866-986-035, 24 hours a day, 7 days
a week. TTY users should call 711.
MetroPlus Health Plan is an HMO, HMO SNP plan with a Medicare contract. Enrollment in
MetroPlus Health Plan depends on contract renewal.
Out-of-network/non- contracted providers are under no obligation to treat MetroPlus Health Plan
members, except in emergency situations. Please call our customer service number or see your
Evidence of Coverage for more information, including the cost- sharing that applies to out-of-
network services.

Section 1 – Introduction
This directory provides a list of MetroPlus Health Plan’s network providers. To get detailed
information about your health care coverage, please see your Evidence of Coverage (EOC).
You will have to choose one of our network providers listed in this directory to be your Primary
Care Provider (PCP). Generally, you must get your health care services from your PCP. Your
PCP can help you get care from other providers, such as specialists. Some services may require a
referral. Please check your Evidence of Coverage for more information about how to access
specific benefits.
The network providers listed in this directory have agreed to provide you with your health care,
vision, and dental services. You may go to any of our network providers listed in this directory;
however, some services may require a referral. Talk to your PCP about getting a referral to
another provider. If you have been going to one network provider, you are not required to
continue to go to that same provider. In some cases, you may get covered services from non-
network providers. Please see your Evidence of Coverage for more information.
If a non-network provider sends you a bill directly, you should not pay that bill. Call MetroPlus
Member Services, and we will explain how to submit the bill to us to determine how it should be
paid.
MetroPlus Health Plan does not cover care received outside of the service area except in case of
an emergency. You may get covered emergency medical care wherever you need it, anywhere in
the United States or its territories. After the emergency is over, you are entitled to follow-up care
to be sure your condition continues to be stable. Your follow-up care will be covered by our
plan. If your emergency care is provided by out-of-network providers, we will try to arrange for
network providers to take over your care as soon as your medical condition and the
circumstances allow.
You must use network providers except in emergency or urgent care situations. If you obtain
routine care from out-of-network providers, neither Medicare nor MetroPlus Health Plan will be
responsible for the costs.

This directory lists the PCPs, specialists, pharmacies, hospitals and other providers that
participate in the MetroPlus Health Plan network. Providers are listed in alphabetical order by
specialty, by zip code.
You can also use the MetroPlus Health Plan website, www.metroplusmedicare.org, to find a
provider. On the website, go to the “Members” page and click on “Find A Doctor” to search for a
physician, specialist, pharmacy, or hospital. You can search for a provider or pharmacy that
meets your needs, such as one that has an office in or near a particular zip code that is convenient
to you.

Changes to our pharmacy network may occur during the benefit year. An updated
Provider/Pharmacy Directory is located on our website at www.metroplusmedicare.org. You
may also call Member Services for updated provider information.
Introduction
This booklet provides a list of MetroPlus Health Plan’s network pharmacies. To get a complete
description of your prescription coverage, including how to fill your prescriptions, please review
the Evidence of Coverage and MetroPlus Health Plan’s formulary.
When this provider/pharmacy directory refers to “we,” “us”, or “our,” it means MetroPlus
Health Plan. When it refers to “plan” or “our plan,” it means MetroPlus Advantage Plan
(HMO D-SNP) or MetroPlus Platinum Plan (HMO).
We call the pharmacies on this list our “network pharmacies” because we have made
arrangements with them to provide prescription drugs to Plan members. In most cases, your
prescriptions are covered under MetroPlus Health Plan only if they are filled at a network
pharmacy or through our mail order pharmacy service. Once you go to one pharmacy, you are
not required to continue going to the same pharmacy to fill your prescription but can switch to
any other of our network pharmacies. We will fill prescriptions at non-network pharmacies
under certain circumstances as described in your Evidence of Coverage.
All network pharmacies may not be listed in this directory. Pharmacies may have been added or
removed from the list after this directory was printed. This means the pharmacies listed here may
no longer be in our network, or there may be newer pharmacies in our network that are not listed.

You can get prescription drugs shipped to your home through our network mail order delivery
program. For more information, please contact us or see the mail order section of this

This directory is for Manhattan which includes the area in which you live. However, we cover a
larger service area, and there are more pharmacies where your prescriptions may be covered by
our Plan. For information on more pharmacies in our plan network not listed in this directory

Mail Order Pharmacies
You can get prescription drugs shipped to your home through our network mail order delivery
program.
For refills of your mail order prescriptions, you have the option to sign up for an automatic refill
program. Under this program, we will start to process your next refill automatically when our
records show that you should be close to running out of your drug. We will contact you prior to
shipping each refill to make sure you are in need of more medication. You can cancel scheduled
refills if you have enough of your medication or if your medication has changed. If you choose
not to use the auto refill program, please contact us 10 days before you think the drugs you have
on hand will run out to make sure your next order is shipped to you in time. To opt out of the
automatic refill program, please contact us by calling Member Services (phone numbers are
printed on the back of this directory.
Typically, you should expect to receive your prescription drugs from 7-10 days for a new
prescription, and 3-4 days for a refill from the time that the mail order pharmacy receives the
order. If you do not receive your prescription drug(s) within this time, please contact us at 1-
866-693-4615 (TTY: 1-800-881-2812).

Sección 1 – Introducción

本手冊提供MetroPlus Health Plan的網絡內藥房名錄。欲獲得處方藥保險的完整描述，包
括處方藥配藥方式，請閱讀《承保福利說明》及MetroPlus Health Plan處方一覽表。
本服務提供者/藥房名錄中的「我們」或「我們的」指代MetroPlus Health Plan。本文
件中的「計劃」或「我們的計劃」指代MetroPlus Advantage Plan (HMO D-SNP)或
MetroPlus Platinum Plan (HMO)。
本名錄中出現的藥房稱為我們的「網絡內藥房」，因為我們已達成協議，透過這些藥房為
已購買我們計劃的會員提供處方藥。在大多數情況下，MetroPlus Health Plan僅承保您在
網絡內藥房或透過我們的郵購藥房服務購買的處方藥。您無須每次都去同一家藥房領取自
己的處方藥，完全可以轉至其他的網絡內藥房進行。某些情況下，我們也會在網絡之外的
藥房為您提供處方藥，詳情可見《承保福利說明》。
名錄可能並未詳盡包含所有網絡內藥房。因為自本名錄付印之後，仍可能有新的藥房加入
或退出名錄。所以，可能出現雖然列入本名錄、但已不屬於我們網絡的藥房，或雖已加入
我們的網絡、但尚未列入本名錄的情況。本名錄最後更新日期為04/01/2020。要獲得最新

郵購藥房
您可以透過我們的網絡郵購送貨計劃將處方藥郵寄到您家中。
如需續配郵購訂單藥物，您可以選擇註冊自動續配藥計劃。在該計劃下，當我們的記錄顯
示您的藥品快用完時，我們將自動開始處理您的下一次續配藥。在發運每個續配藥前我們
都會與您聯絡，以確信您需要更多藥品。如果您有足夠的藥物或者您的藥物已變更，您可
以取消計劃的續配藥。如果您選擇不使用自動續配服務，請在您手頭上的藥品用完前提前
10天與我們聯絡，以便確保您的下一訂單的及時送達。要結束自動續配計劃，請致電會員
服務部聯絡我們（電話號碼已印於本名錄的封底。
一般來說，您應在藥房收到訂單後的7-10日內收到新處方藥物，在3-4日內收到續配藥物
。如果您沒有在此時間內收到您的處方藥，請聯絡我們1-866-693-4615 (TTY:
1-800-881-2812)。
MetroPlus Health Plan是一家擁有Medicare合約的HMO、HMO SNP計劃。參加MetroPlus
