• Less (Battachyaria and Lakdawalla, 2003))

• Outcomes: CMHS

• Utilization: Cooperative Cardiovascular Project (CCP) data on heart attack patients (1994/95)

Two measures of income: per capita family (CPS) and zip code (CMHS, CCP)
