Accessibility Codes
P = Parking Parking spaces, including van accessible space(s), are accessible. Pathways have curb ramps between the parking lot and at drop off locations.
EB = Exterior Building Curb ramps and other ramps are wide enough for a wheelchair or scooter user. Handrails are provided on both sides of the ramp. There is an “accessible” entrance to the building. Doors open wide enough to let a wheelchair or scooter user enter, and have handles that are easy to use.
IB = Interior Building Doors open wide enough to let a wheelchair or scooter user enter, and have handles that are easy to use. Interior ramps are wide enough and have handrails. If there is an elevator, it is available for public/patient use at all times the building is open. The elevator has chimes/verbal announcements for floor passing and next floor arrival, and Braille buttons within reach. The elevator has enough room for a wheelchair or scooter user to turn around. If there is a platform lift, it can be used without help.
R = Restroom The restroom is accessible and the doors are wide enough to accommodate a wheelchair or scooter user and are easy to open. The restroom has enough room for a wheelchair or scooter to turn around and close the door. There are grab bars which allow easy transfer from wheelchair to toilet. The sink is easy to get to and faucets, soap and toilet paper are easy to reach and use.

PA = Participant Areas The entrance of the participant area is accessible, with doors that open wide enough to accommodate a wheelchair or scooter. The participant area has enough room for a wheelchair or scooter user to approach and park for participation in activities or use of exercise equipment.
T = Exam Table/Scale or Medical Equipment In a medical office, the exam table moves up and down and the scale is accessible with handrails to assist people with wheelchairs and scooters. The weight scale is able to accommodate a wheelchair. In other service locations, there is a height adjustable chair and tables. The mammography machine can accommodate a wheelchair. Lift equipment is available to assist with transfers.
Choosing a Primary Care Provider
Now that you have joined Santa Clara Family Health Plan (SCFHP), one of the first things to do is to choose a doctor or clinic for each health plan member in your family. The doctor or clinic you choose will be the Primary Care Provider (PCP) for yourself or your child. Each person in your family who is a member of SCFHP may have his or her own PCP. You do not need to choose the same doctor or clinic for your whole family.
Note that if you are enrolled in both Medi-Cal and Medicare, you do not have to choose a PCP because Medicare is the primary payor.
Your PCP is the one who takes care of you and is the first person you see when you need care, if it is not an emergency. Your PCP will also refer you to a specialist when needed. If you are seeing a specialist, ask your specialist for their advice in choosing a Primary Care Provider.
Each Primary Care Provider works with certain specialists and hospitals. When you choose a PCP you are also choosing to use the specialists and the hospital(s) with whom the doctor works. We call this a “network.” You will find each doctor’s network listed under his or her name in the full listing by city.
SCFHP has several networks: VHP Network and their affiliated clinics and Community Clinics; Physicians Medical Group of San Jose; Premier Care of Northern California; Kaiser Permanente; Palo Alto Medical Foundation; and a number of independent physicians who work directly with SCFHP.

If your SCFHP network is Kaiser Permanente, you can locate a Kaiser provider online by going to https://healthy.kaiserpermanente.org/doctors-locations. If you have additional questions on selecting a provider you can also call the Kaiser Member Services Contact Center toll free at 1-800-464-4000 (TTY users call 711).
How to Select the PCP Who is Right for You
One of the best ways to make sure you get excellent health care is to have a PCP with whom you can build a long-term relationship. Your PCP is someone who knows your medical background and understands what’s important to you. You can choose a PCP who is a:
• Family practice physician (usually treats all ages)• Pediatrician (treats infants and children)• Obstetrician/gynecologist (OB/GYN) (treats women) who also does primary care• General internal medicine physician (treats adults)• Clinic, including a Federally Qualified Health Center (FQHC), Community Clinic
or an Indian Health Service facility in Santa Clara County
You may receive care from a nurse practitioner (NP), physician assistant (PA) or certified nurse midwife (CNM) to provide your PCP services. The NP, PA or CNM must work in your PCP’s office and be supervised by your PCP. If you are pregnant or you are planning to become pregnant, you also have the right to select an out-of-plan certified nurse midwife (CNM).
When you have a PCP you know and trust, you feel comfortable talking about anything. And, all your basic care, including routine exams, preventive care and treatment for illnesses or injuries, goes more smoothly. Your PCP also knows when it’s best to refer you to a specialist.
When choosing a PCP, there's a lot to think about. Here are some questions to ask yourself when choosing a PCP.
1. Do you want a PCP who is close to home, school or work?Keep in mind that most appointments are scheduled during the day, Mondaythrough Friday. Consider where you are during these times. At work? At home?Helping out at your children’s school? This will help you find a PCP who is themost convenient for you.
2. Are you more comfortable with a male or a female PCP?Discussing health issues and treatments can be a very personal experience. It’simportant that you are comfortable talking to your PCP about any medicalsituation that may come up. Deciding whether you prefer a male or female PCPcan ensure that you are as comfortable as possible.

3. Do you have any language requirements? Medical treatments and diagnoses can be complex regardless of what language you speak. That’s why it’s important for you and your PCP to communicate without any language barriers. Choosing a PCP who speaks your language will help ensure good communication.
4. Is there a hospital close to your home, school or work that you want to use? You might want to find a PCP who is affiliated with, or has privileges at that hospital.
5. Are you eligible for Community-Based Adult Services (CBAS)? Some PCPs and provider networks do not work with CBAS centers. In order to receive CBAS services, you need to choose and remain with a PCP contracted with SCFHP through VHP Network or Kaiser Permanente, or as an independent physician. If you choose a PCP who does not work with a CBAS center, you cannot participate in the CBAS program.
6. Do you want to be able to access your PCP’s office online? California law requires health plans to provide timely access to care. This means that there are limits on how long you have to wait to get health care appointments and telephone advice.
Santa Clara Family Health Plan (SCFHP) is committed to providing timely access to care for all members. SCFHP strives to ensure that all health services are provided in a timely manner. SCFHP will continue to notify our members of any changes or updates made regarding the current policies.

Cómo elegir a un proveedor de atención primaria (PCP)
Ahora que se ha unido a Santa Clara Family Health Plan (SCFHP), una de las primeras cosas que debe hacer es elegir un médico o una clínica para cada miembro de su familia que participe en el plan de salud. El médico o clínica que elija será el médico de atención primaria (Primary Care Provider, PCP) de usted o de su hijo. Chọn Nhà Cung Cấp Dịch Vụ Chăm Sóc Chính

Khả năng Tiếp cận Chăm sóc Kịp thời
Luật pháp California quy định các chương trình bảo hiểm y tế cung cấp khả năng tiếp cận chăm sóc kịp thời. Điều này có nghĩa là có giới hạn về thời gian quý vị phải chờ đợi để đặt được lịch hẹn chăm sóc sức khỏe và tư vấn qua điện thoại.
Santa Clara Family Health Plan (SCFHP) cam kết cung cấp khả năng tiếp cận kịp thời để chăm sóc cho tất cả các thành viên. SCFHP cố gắng đảm bảo rằng tất cả các dịch vụ y tế được cung cấp một cách kịp thời.
