Maximizing Health IT, Modeling, Tracking, Tracing, and Other Public Health Tools during the COVID-19 Outbreak

April 20, 2020

Presenter
Presentation Notes
As part of AMIA's ongoing COVID-19 Webinar Series exploring the role informatics experts are playing in the fight against the global pandemic, this session will discuss the use of informatics by public health agencies to assist in determining where best to allocate resources to maximize health efficiency and efficacy. Public health informatics has been essential in the systematic, coordinated mobilization to support COVID-19 outbreak management. The discussion will include the challenges of meeting rapidly-evolving requirements while maintaining high-quality, pre-pandemic and pandemic health care. Computer modeling, contact tracing, tracking, and other informatics tools are taking on COVID-19 to simulate the disease course, predict how the disease will unfold, and to identify how it can best be tackled. Some variables include key numbers such as the disease incubation period, how quickly the virus spreads through the population, and the case-fatality ratio. These numbers and others are critical in managing populations and determining policies to control disease spread. This webinar will include a discussion of case management, disease modeling, and guidance for the deployment of resources.

Health Informatics is the science of how to use data, information, and knowledge to improve human health, including the execution of scientific research, the delivery of health care services, and the promotion of public

health. AMIA is the multi-disciplinary, inter-professional home for 5,400+ health informatics experts.

Presentation Notes
One way to think about informatics is through scales. We tend to think of patients as an n of 1 or 10 to the 0 power. Practices are usually measured in panels of a thousand, populations are in the millions, and globally we’re talking billions of patients. On the other end of the scale, we have thousands of diseases, millions of small molecules and billions of lines of DNA. When we apply the scope of health informatics to this scale, it helps articulate the various, overlapping boundaries of the five main areas of informatics: TBI professionals use informatics tools and methods that link biological entities (genes, proteins, and small molecules) to clinical entities (diseases, symptoms, and drugs)—or vice versa. CRI professionals leverage IT and data systems to manage information related to clinical trials and also involves informatics related to secondary research use of clinical data. Clinical informatics experts focus primarily on information systems that impact patient care and manage information related to their diseases and symptoms – and they do so at the practice level. Public Health Informatics is the application of informatics in areas of public health, including surveillance, reporting, and health promotion. Public health informatics, and its corollary, population informatics, are concerned with groups rather than individuals. Consumer informatics professionals analyze consumers' needs for information; studies and implements methods for making information accessible to consumers; and models and integrates consumers' preferences into health information systems. Finally, we can also think about policies on this scale: And perhaps the policy response to the coronavirus best exemplifies why the globe of health informatics is so important. Across all scales from genomic to disease to patients/consumers, practices, populations and the entire globe, informatics and AMIA members have a role to play. With all this as our background, I’d like to introduce today’s panel and discuss our game plan for the next 50 minutes.

To highlight how our members and the broader informatics community is addressing this global pandemic we are launching the AMIA COVID-19 Webinar Series. We will look at the pandemic through a health informatics lens and is designed to share informatics responses to the COVID-19 pandemic. Panelists will share their specific domain expertise, including clinical informatics, public health informatics, translational bioinformatics, clinical research informatics, and consumer health informatics. We will also have special emphasis webinars covering topics related to global health, telemedicine, and public policy during the COVID-19 pandemic. These webinars are open to all at no cost.

Presenter
Presentation Notes
From front-line informaticians and public health informaticists, to clinical and bioinformatics researchers, AMIA's members are uniquely positioned to leverage data and evidence in the fight against COVID-19. The first webinar of the AMIA COVID-19 Webinar Series will be an overview of Public Health Informatics issues, including shifts in the regulatory landscape and new legislation developed by the US Congress. We will focus on the key health informatics policy issues to watch and discuss additional steps that may be needed to address this ongoing epidemic.

Questions that need answers to enable Data Driven Policy• How many cases will we see?• When will it peak?• Will we run out of beds? ICU beds? Ventilators?• Should we shut down bars and restaurants? Schools? Businesses?• How much (extra) PPE is needed, and where can we get it from?• When can we start things back up?• What should we start back up first?

CAVEAT: STILL NO predictive COVID model There NO generally accepted modeling approach or

output that to produce a definitive (or even consensus) future epidemic curve of the spread and clinical impact of SARS-CoV-2 causing COVID-19 in the United States or other countries

Exponential early rise DOES NOT NECESSARILY MEAN continued exponential growth at later dates

Shape of the epidemic curve and length of stay in hospital units matter for initial hospital load

The BIG QUESTIONS Prevalence?

- Slow- vs. Fast-growth models- HUGE impact on what comes next

Societal Impact? - Why do some epi curves not look at all like others- What are local effects?- Range: prior air pollution

Treatment Variability/Outcomes? - Large variation between countries, counties, hospitals

Electronic Public Health Surveillance is Not “Done” in U.S.

• Broader public recognition in COVID-19 than ever before• Outbreak management, contact tracing, case management, connecting lab

results, coordinating response measures, situational awareness, reporting to others

• Not fully addressed by syndromic surveillance or electronic laboratory reporting

• Syndromic surveillance started with patient chief complaints because those were the data that could be accessed electronically

• Electronic Laboratory Reporting (ELR) was similar and a helpful start, but lab results do not contain critical clinical data or the patient demographic information for case management and contact tracing

eCR Now

Electronic case reporting (eCR)

The automated identification of reportable health events in electronic

health records and their transmission to state and local public health authorities

for review and action.

1. Cohort-based COVID-19 rapid eCR implementations for provider sites that have eCR enabled EHRs

2. A new eCR Now FHIR app that other, non-eCR enabled, EHRs can immediately implement to automate COVID-19 eCR

3. Extension of the existing eHealth Exchange policy framework through a developing Carequality eCR implementation guide

(4/20) Last week we on-boarded new COVID-19 eCR for 4.5 million patients and 13,000 physicians
