INTRODUCTIONIn biological terms, a community is a group of interacting organisms sharing a

populated environment.In human communities, intent, belief, resources,preferences, needs, risks, and a number of other conditions may be present and common, affecting the identity of the participants and their degree of cohesiveness. Traditionally, a community has been defined as a group of interacting people living in a common location. The word is thus often used to mean a group of people sharing common geographic boundaries and/or common values and interests. (Maglaya, Araceli, 2004). It is characterized by its geographic boundaries composing of different institutions and organizations linked in a complex network which has a formal and informal power structure and a communication system. In addition, it is being categorized into three: Community as a Place which is primarily concerned with physical and topographical characteristics of the community; Community as People which includes socio – demographic and economic characteristics of the people constituting the community, and Community as a Social System which focuses on the social roles, behaviors and practices being developed within the community.

On the other hand, Community Organizing is a systematic approach or study of health condition involving collection, organization, synthesis, analysis and interpretation of health statistical data. It defines the opportunities and resources involved in initiating community health action programs. Its primary purpose is to determine the health status of a certain community in order to identify factors which may directly and indirectly influence the health of the population. Moreover, it describes the different factors such as demographic variables, socio – economic and cultural factors, environmental factors, health resources, political and leadership patterns that relate to existing health and illness patterns of the community and its people.

The researchers are assigned in Barangay 178A, Macopa Street San Vicente Ferrer, Camarin Caloocan City. Through the application of COPAR, the researchers seek to identify the different factors that will affect the health condition of the people in Macopa Street. The researchers aim to arouse awareness among the community people about their current condition as well as the impact of their actions on their health. With this, the awareness of the people can be used to help the people achieve optimum level of health.

HISTORY OF BARANGAY CAMARIN

One among 188 Barangays in entire Caloocan is Barangay 178-Kiko Camarin founded on year 1968. With total area of 549.12 hectares bounded by NORTH-TALA Leprosarium; SOUTH – MALIGAYA Park subdivision; EAST – Catmon Road (Barangay 179); and WEST – KING SOLOMON (Barangay 174), from 5,000 first inhabitants, populace reach now to 91,485 among which 46,657 comprises females (51%) and 44,828 (49%) males; population density 166.603 persons per hectares; annual growth rate 5.57% (natural = 2.38% and migration 3.27%); 0-17 ages 44.81%’ 18-ages-above = 55.19%; employed 48.26%; unemployed = 51.74% and mortality rate 1% annually; number of household = 35,807.

On January 15, 2011 the health care provider aims to provide a health teaching for the family that we chose in the community. The health teaching will last for about 30 minutes. Our teaching plan aims to provide knowledge to the family, this teaching plan will give them awareness and it would help them to improve their health status.

The health care committee provides a teaching plan that would help in the enhancement of the client’s health, especially in her case wherein at the age of 7yrs. old she is having an asthma attack.

The Ibanes family lives at Macopa St. Caloocan City for almost 19 years. The head of the family was Mrs. Ibanes her grandmother. Angenet was daughter of Mrs. Vicky on her first husband and now they live with her second husband’s family. Her father has no permanent job; they are supported by her husband’s brother. They live in a concrete one story house and currently under renovation, in their house there is only one room and their family is the one who sleeps in the room.

Past Health History

Her mother states that Angenet was diagnosed with asthma when she was 2 years old. She’s been prescribed with Cotrimoxazole and she takes it 3x a day, they consulted a physician in the clinic on Kiko Center, when we asked her mother why is she bringing Angenet on Kiko instead on their on barangay center, the mother answered, “mas okey dalhin sya sa kiko center, kasi jan sa center naming wala ngang bayad ang konsulta pero gumagastos din kmi sa gamut, dahil sa generic ang nirereseta napapatagal ang paggaling ng anak ko, eh sa kiko ok ng sigurado yung gamut, kahit napapagastos kami ng mahal sandal lang gumagaling agad ang anak ko.” Her mother said that her daughter is incomplete in immunizations, Angenet only have BCG of 3 doses. Ate Vicky said that her daughter doesn’t have any allergies on foods, but they don’t let Angenet take care of cats, because they believe that it could contribute on her asthma attacks. Last 2010 Angenet was bitten by a dog in front of their house, they brought her to a doctor to seek medical attention, but ate Vicky said that Angenet didn’t complete the doses of anti-tetanus, she only gets the first dose, because according to her mother it’s too expensive. Her mother was happy because even if Angenet has a low immune system she was never been confined in a hospital “hanggat maaari inaagapan namin na lumala ang sakit nya.” As stated by her mother. Recently Angenet took Calpol for her cough; they give it 2-3x a day, until the cough disappears. Ate Vicky said in addition that “pinapausukan ko din pala yan, 2-3 beses kada araw, rumirenta ako ng pampausok dyan sa may court.”

Family History of Illness

According to her mother she thinks that Angenet got her asthma from her father, “mahina din kasi yung unang asawa ko, kaya siguro namana nya yun, pero sa pmilya ng asawa ko nngayon wala namang may hika, high blood lang ang sakit nila.”

We told ate Vicky to rate her daughter’s health from 1-10 as 10 being the highest score, she said that she will give a score of 7, because “alam ko naman na may kulang talaga sa kalusugan ng anak ko, pero hindi ko naman sya bibigyan ng mababang score kasi kahit ganyan yan hindi pa naman yan naoospital.” Foe the past year they only consulted the doctor for the reason that she was bitten by a dog. Angenet took baths every day, and her mother always reminds her to wear slippers all the time. She also brushes her teeth before she goes to sleep. Her mother and father are both smokers. For them the ventilation in their environment is adequate for them, they do not segregate waste, they just put it on a “sako”, their toilet facility was a water sealed type. And there are some presences of vectors in the house.

Usually Angenet only eats “sabaw” and rice, she don’t have any vitamins. Sometimes if she doesn’t like the food she ate bread instead. She also drinks milk but only the brand of Bear brand.

She drinks 3 glasses of water a day estimated (750cc/day), and she also drinks milk. Angenet prefers meat that vegetables and fish, usually she likes fried foods, and foods with sauce. On skin problems she has wounds, according to her mother she got that when she plays outside with her friends.

ANALYSIS:

An adequate food intake consists of a balance of essential nutrients. People require essential nutrients in food for the growth and maintenance of all body tissues and the normal functioning of all body processes. People often describe their health and physical fitness by their activity, because mental well-being and the effectiveness of body functioning depend largely on their mobility status. Individuals with positive self-concept are better able to develop and maintain warm interpersonal relationships and resist psychological and physical illness. (Kozier, Erb, Blais and Wilkinson Fundamentals of Nursing 5th ed. Pp. 1014, 802, 878)

INTERPRETATION:

Her mother gave her a rate of 7 because her mother based it on her current health status. And at her age, she preferred fried foods. Her mother believes that she easy get sick because of what she is eating.

Angenet usually sleeps at around 8-9pm if she took a nap in the afternoon she sleeps at 9pm, but if she plays all day she easily gets sleepy. She wakes-up at around 5-6am; her mother said that she has a continuous sleep at night. Every night she watches TV until she feels sleepy.

She can already write her name; according to her mother that if angenet wants something she will not stop annoying her mother until they give what she wants.

ANALYSIS:

School age child sleeps for 10 hrs. at night, sleep time remains relatively constant.

INTERPRETATION:

They attend mass during Sundays but sometimes they left Angenet at home, her mother said that before going to sleep she usually prays. And she knows who God/Jesus is.

She has dental cavities and missing teeth. With mouth odor. Maybe she likes sweets

and seems she don’t regularly brushing her teeth.

According to dental studies, about 85% of people with persistent bad breath have a dental condition that is to blame. Gum

disease, cavities, oral cancer, dry mouth and bacteria on the tongue are some of the dental problems that can cause bad breath.

IV. PHYSIOLOGY & PATHOPHYSIOLOGY

PHYSIOLOGY

The respiratory system comprises of the nose, mouth, throat, larynx, trachea, bronchi and lungs. The function of the respiratory system is to facilitate gaseous exchange to take place in the lungs and tissue cells of the body. Oxygen is required by cells in the body to allow various metabolic reactions to take place and to produce energy and is therefore essential to life.

The respiratory system may be defined as the organs and tissues through which air is passed into and out of the body to allow the necessary gaseous exchanges to take place.

The nervous system is an organ system containing a network of specialized cells called neurons that coordinate the actions of an animal and transmit signals between different parts of its body. In most animals the nervous system consists of two parts, central and peripheral. The central nervous system of vertebrates (such as humans) contains the brain, spinal cord, and retina. The peripheral nervous system consists of sensory neurons, clusters of neurons called ganglia, and nerves connecting them to each other and to the central nervous system. These regions are all interconnected by means of complex neural pathways.

Neurons send signals to other cells as electrochemical waves travelling along thin fibres called axons, which cause chemicals called neurotransmitters to be released at junctions called synapses. A cell that receives a synaptic signal may be excited, inhibited, or otherwise modulated. Sensory neurons are activated by physical stimuli impinging on them, and send signals that inform the central nervous system of the state of the body and the external environment. Motor neurons situated either in the central nervous system or in peripheral ganglia, connect the nervous system to muscles or other effector organs. Central neurons, which in vertebrates greatly outnumber the other types, make all of their input and output connections with other neurons. The interactions of all these types of neurons form neural circuits that generate an organism's perception of the world and determine its behavior. Along with neurons, the nervous system contains other specialized cells called glial cells (or simply glia), which provide structural and metabolic support.

B. PREDISPOSING FACTORS1. Host

Our patient’s name was AI, 7 years old, female, a Filipino citizen. The patient is very prone to these diseases because she is a school age children which primarily still developing her immune system and it is likely to happen in this age group. The patient’s father smokes 8 cigarettes a day which greatly contributes to the occurrence of these diseases. As part of her developmental activities, the patient is also fond of playing with her classmates. She has poor eating habits which makes her vulnerable to have a disease.

2. Agent The agent that triggers to the occurrence of her condition is a biologic agent which is a bacteria transferred through airborne droplets. She acquired this through her siblings because her sister had also a past health history of these diseases 3. Environment The environment that triggers the development of these diseases was the socio-economic type because their home is very congested and has poor ventilation which makes it easy for the bacteria or virus to be transferred from one person to another.

ECOLOGIC MODEL: WEB MODEL

The Web model shows the relationship among the three predisposing factors mainly agent, host and environment which also determine their imbalance that may lead to an occurrence of a disease. This primarily talks of the multiple causation of the disease. The diagram below shows the possible imbalance of the predisposing factors:

Web model is used when an organ is said to be affected by a disease and is caused by multiple factors. Common colds and cough suppresses the immune system of an individual especially when it comes to a child with a low immune system. And when it happens, the body most specifically the thermoregulator which is the hypothalamus compensate to this changes in order to maintain balance to the body of an individual. As a result, the body expel heat and as one of the major manifestation of this is the fever or when a person has an elevated temperature beyond the normal limits. The environment as well as the agent is interrelated to each other since the client has poor ventilation and congested area, bacteria can be easily transferred from one another through airborne droplets.

CONCLUSION AND RECOMMENDATION:

There are several factors why these diseases occurred. In order to control and eliminate the existing problem that the client currently experiences, the health team most especially the nurse must offer some appropriate interventions such as providing knowledge, information and skills through the use of health teaching. The health care provider must explain the importance of drug compliance most specifically when they are taking antipyretic, mucolytic and decongestant to relieve her from these diseases. Client must also eat nutritious food for the faster recovery, increasing vitamin supplements, such as vitamins A and C. Excellent care for the patient such as providing at least good ventilation will really help a lot in the prevention and elimination of these diseases.

VI. Project Background

Based on Maslow’s Hierarchy of needs, food is one of the physiologic needs of every individual. Proper maintenance of good nutrition promotes continuity of wellness and life. Otherwise, malnutrition or even death may occur.

This project can help in maintaining good nutrition among the students as to prevent the occurrence of many undesirable instances that may happen to inadequate nourishment.

Malnutrition may have occurred due to low economic status and lack of knowledge about proper nutrition. This affects many elementary pupils because they prefer non- nutritious foods rather than nutritious ones. This proposal is developed through the keen observation of the School Nurse Committee on the nutritional development of the Grade I pupils of Camarin Elementary School. The organizer was able to realize the plan to decrease the prevalence of malnutrition in the community.

VII. Project Justification

Abrupt action on the problem will help prevent its further aggregation and development of complication which might arise to another problem. The effects may cause the pupils to have low self-esteem, poor physical health, and mental illness. Poverty and knowledge deficiency are two of the most common reasons that cause malnutrition. This project may help solve the problem and people may realize the consequence if it will continue and not be properly intervened.

The general objective of this project is to help the community in the promotion of health, prevention in case of sick and well individuals and improve the community’s health condition.

Last January 17 2011, We the Community Organizer Committee endorsed our proposed and approved project entitled “Butas Ay Takpan, Upang Di pamugaran” to BSN130 Group 119. Project Proposal, Materials and Spot Map we’re explained to the next group in order for them to determine the next steps that should be done and also with the materials which were bought for the said project which was also explained and endorsed to them. The spot map for the site of canal construction was showed to them. Pictures were also included for easy access and estimation of how much time will be consumed for the cleaning and construction of the

site. We had also informed them that the materials that will be need for the construction were already bought. The receipt for the materials was also endorsed to them.

The Health Care Provider Committee also endorsed to Section 130 group 119 the following: Searching client for Case Presentation, Case Presentation, Assignments at the Barangay Health Center for immunization, pre-natal check-ups and outpatient department and Helping the School Health Nurse Committee in assessing children for Feeding Program.

The Planning committee endorsed the group log book as well as its contents. The committee also endorsed the progress of the programs in the committee and what the planning committee’s roles are in the process. Accomplishment plans are also to be endorsed as well as the daily action plans that are part of the community organizer’s daily activities.

The School Nursing Committee endorsed the feeding program to BSN 130 group 119. The committee also endorsed the materials needed during the program. They were also made familiarized with the number of pupils to be fed and the step by step procedures before going on with the feeding program. The list of pupils, assessment tools and the spot map of the school were also explained and handed to them.

Program plans for the feeding program are also to be endorsed by the Planning Committee including the feeding program schedules in order to plan ahead what needs to be accomplished. Lastly the process of making the Community Action Plan was endorsed which will be a part of the manuscript at the end of the community organization experience.
