Management of hepatitis B virus in the liver transplantation and immunocompromised setting

Hepatitis B virus (HBV) infection is at risk for reactivation when the immune response is suppressed. Immune suppression including transplantation or anticancer chemotherapy enhances the hepatitis B viral load, whereas reduction in immune suppression may also be harmful because of immune restoration. In the absence of effective prophylaxis, the recurrence of hepatitis B after transplantation was as high as 75% leading to early graft loss and a reduction in long-term survival. Moreover, the clinical outcome of HBV reactivation during and after anticancer chemotherapy may lead to hepatic failure and even death. The negative impact of immune suppression is related to two non-exclusive factors: (i) high HBV viral load may result in a rare severe liver disease called fibrosing cholestatic hepatitis by the direct toxicity of high levels of HBV proteins; (ii) The pathophysiology of HBV reactivation is mainly immune-mediated via HBV-specific T lymphocytotoxicity. This double-edged sword explains why prophylactic therapy should be considered for all HBV-infected patients. This review addresses current knowledge on the clinical aspects and management of HBV in the liver transplantation and immunocompromised setting.

서 론
만성 B형 간염의 치료는 항바이러스 제제의 사용과 더불어 지속적인 발전을 거듭하고 있으나 여
전히 전세계적으로 유병률과 사망률이 높은 감염성 질환이고 매년 50만명 이상의 환자가 B형 간염과
관련된 만성 간질환의 합병증으로 사망하고 있어서 간이식의 적응증이 된다.1 그러나 1990년도 초반
까지 B형 간염과 관련된 만성 간질환은 간이식의 상대적 금기증에 해당되었는데, 그 이유는 B형 간
염 환자에서 간이식후 예방적 치료를 하지 않은 경우 75%에 이르는 높은 비율로 이식 후 간염이 재발
하고 이러한 이식 간의 재발 간염은, 진행 속도가 빠른 담즙 경화성 간염(fibrosing cholestatic hepatitis)
으로서 1년 생존율이 33-65%로 낮고, 대개 이식 후 2-3년 안에 이식 간을 잃게 되기 때문이다.2 그러나
수동 면역제제인 hepatitis B immune globulin (HBIG)과 라미부딘과 같은 항바이러스 제제를 이식 전
후 예방적으로 사용하기 시작하면서 이식 후 B형 간염의 재발을 5년에 5% 미만으로 줄일 수 있게 되
었고 이로써 B형 간염 환자에서의 간이식 성적은 B형 간염 이외의 간질환 환자군과 같거나 오히려 우
수하게 되었다.2 경구 항바이러스 제제의 이식 전후 사용은 간이식의 성적을 향상시킬 뿐 아니라 고

비용의 HBIG 사용을 줄일 수 있고 HBIG 치료에 실패한 환자에서도 적용이 가능하다.3
이식 후 B형 간염의 재발에 관여하는 요소로는 이식 전 바이러스 부하가 적거나 빨리 제거될 수 있
는 경우에는 이식 후 B형 간염의 재발률이 낮다고 알려져 있다. 즉, 이식 전 HBeAg과 HBV DNA가 음
성인 경우와 D형 간염의 중복 감염, 급성 간부전, 이식 전 내성 돌연변이종이 없는 경우에는 재발률이
낮은 반면, 반대의 경우에는 이식 후 재발률이 높다. 만성 B형 간염의 합병증으로 간이식 시행 후
HBV에 재감염 되는 기전은 (i) 수혜자의 혈중에 존재하는 HBV에 의한 급속한 재감염 및 (ii) 간 이외
의 부위에서 replication하는 HBV에 의한 재감염이 제시되고 있다.4,5 이 중 후자에 대해서는 다소 이견
이 있으나 이식 후 주된 HBV strain은 이식 전 환자의 혈청에서 검출된 바이러스 strain이 아니고 말초
혈액 단핵구에 존재하는 strain과 일치함을 보여주는 연구 결과가 있다.6 따라서 이식 전 B형 간염 바
이러스의 증식을 최대한 억제하고 이식 후 B형 간염 바이러스의 재감염을 예방하는 치료가 간이식
의 성적을 향상시키기 위해 매우 중요하다.
이식 환자의 B형 간염 치료
1. B형 간염 재발 예방을 위한 이식 전 치료
이식 전 항바이러스 제제의 투여는 바이러스의 증식을 최대한 억제하여 이식 후 재발을 예방하는
데에 목적이 있다. 이식 전에 항바이러스 제제를 투여하여 HBeAg 혹은 HBV DNA 혈청 음성 전환을
유도하여 이식 성적을 향상시킬 수 있고, 간이식 전 항바이러스제 치료는 만성 B형간염 치료 가이드
라인을 따른다.7,8 간이식의 대상이 되는 비대상성 간경변증 환자에서 라미부딘 치료는 간기능을 호
전시키고 간이식의 필요성을 줄일 수 있는 것으로 보고되었으나, 4년 치료 시 60-65% 이상에서 라미
부딘 내성 바이러스가 발생할 수 있고 다른 항바이러스 제제의 효능을 떨어뜨리는 부정적인 영향으
로 인하여 더 이상 일차 치료약제로 권유되지 않는다.9 아데포비어는 비대상성 간경변증 환자의 초치
료제로서 단독 연구된 바 없는데 비교적 높은 내성 발생률(5년에 30%)과 상대적으로 낮은 바이러스
억제력으로 인하여 일차 치료 약제로 권유되지 않는다. 텔비부딘은 라미부딘에 비하여 HBV DNA 음
전율(<300 copies/mL)이 높으며(47 vs. 36%), 바이러스 돌파도 적고(29 vs. 39%), 104주 치료 동안 생존
률도 높다(83 vs. 75%).7 그러나 라미부딘에서와 마찬가지로 비대상성 간경변증 환자에서 텔비부딘
투여 시에 내성바이러스의 발생은 간부전으로 진행할 위험성이 있다.7 비대상성 간경변증 환자 191
명을 대상으로 무작위로 엔테카비어 (1 mg/일) 또는 아데포비어(10 mg/일)를 96주간 투여한 결과, 24
주 및 48주 째 혈청 HBV DNA 불검출율(<300 copies/mL)은 엔테카비어가 아데포비어보다 우수하였
으며(24주; 49% vs. 16%, 48주; 57% vs. 20%), 48주 째 HBeAg 혈청 전환율은 양군 간에 차이가 없었다
(6% vs. 10%).10 55명의 비대상성 간경변증 환자의 초치료에서 엔테카비어를 사용하여 12개월 치료한
결과, 약 절반(49%)에서 Child-Pugh 점수가 2점 이상 호전되었으며 무이식(transplantation-free) 1년 생
존율은 87.1%였다.11 진행된 간경변증 환자에서 테노포비어를 사용한 연구 결과는 매우 제한되어 있
으나 48주간 2상으로 진행된 이중맹검 무작위 임상 연구에서 테노포비어의 항바이러스 효과는 비대
상성 간경변증이 아닌 환자군과 동일하였다.12 비대상성 간경변증 환자는 신속하고 적극적인 치료가

요구되므로 HBV DNA가 PCR 검사상 양성인 경우 항바이러스 효과가 우수하며 내성 발현율이 적은
엔테카비어 또는 테노포비어 같은 경구용 항바이러스제 치료를 권장한다. 항바이러스제를 13투여 받
은 일부 환자에서는 3-6개월 정도 시간이 경과하면서 간이식이 필요하지 않은 정도로 임상적인 호전
을 보이는 경우가 있는데, 항바이러스제 투여에도 불구하고 간부전으로 진행되는 경우에는 간이식
이 필요하며, 이식 전 후 항바이러스제 치료가 이식 후 간염의 재발 위험을 줄여준다.14
2. 이식 후 B형 간염 재발의 예방적 치료
1) Hepatitis B immune globulin (HBIG) 단독 치료
HBIG의 작용 기전은 명확하지 않으나, 간 이외의 부위에서 증식하는 HBV로부터 naïve hepatocyte
를 보호하는 것으로 생각되며 면역 복합체를 형성하여 순환하는 virion을 중화시킨다는 가설도 제시
되고 있다.15 이식 후 초기에 HBIG 용량이 불충분 경우나 후기에 HBIG에 대한 내성 돌연변이종이 발
생하는 경우 B형 간염이 재발한다. 재발 고위험군 환자에서 초기 HBIG 고용량 요법으로 8효과적인
치료가 가능한데 무간기 및 이식 후 7일 간 매일 HBIG 10,000-20,000 IU을 정주하고, 이후 매달 HBIG
10,000 IU을 정주하는 고용량 요법을 사용하여, anti-HBs 역가를 500 IU/L 이상 유지한다.2 HBIG 고용
량 요법으로 간이식 후 약 16-35%의 B형 간염의 재발률을 보고하였다. 저위험군에서는 anti-HBs 역가
를 100-150 IU/L 이상 유지하는 방법을 사용할 수 있다.2 그러나 HBIG의 장기간 사용으로 HBV 표면 단
백에 대한 내성에 의한 B형 간염이 재발할 수 있으며, 이 외에도 HBIG 정주에 따르는 불편과 고비용
및 혈청을 이용한 HBIG 제작 시의 감염 가능성 등 때문에 최근에는 HBIG과 항바이러스 제제를 병합
하여 HBIG의 사용량을 줄이거나 HBIG을 전혀 포함하지 않는 요법이 시도되고 있다.
2) 뉴클레오사(타)이드 유사체 단독 치료(Complete HBIG free strategy)
일부 연구에서 선별적인 라미부딘 단독 요법을 시행하고 17개월 간 추적하여 5.2%의 내성 발생률
과 87%의 환자 생존율을 보고한 바 있으나 일반적으로 치료 2년 째 B형 간염 재발율이 35-50%에 이르
고 16HBV DNA 양성인 경우 재발율이 60%까지 되기 때문에 현재 권장하고 있지 않다.17 최근 개발 된
뉴클레오사(타)이드 유사체인 엔테카비어나 테노포비어의 경우 바이러스 억제력이 강력하고 약제
내성이 적은 항바이러스제로서 간이식 전 만성 B형 간염에서의 치료 성적으로 미루어 볼 때, 간이식
후 이 약제들의 단독 사용 시 B형 간염 재발이 효과적으로 억제되는지 규명하는 잘 계획된 대규모의
연구가 필요하다. 실제로 Fung 등이 80명의 HBsAg 양성 환자를 대상으로 HBIG 없이 엔테카비어 단독
으로 예방적 치료를 시행하고 26개월 간 추적하였을 때 간이식 당시 B형 간염 바이러스의 완전 반응
률은 26%였으나 1, 2년 째 HBsAg 소실률이 각각 86%, 91%였고, HBV DNA 소실률이 98.8%에 달한다
고 보고하였다.18
3) HBIG과 뉴클레오사(타)이드 유사체 병합 치료
(1) 고용량 HBIG라미부딘과 고용량 HBIG 병합요법으로 B형 간염의 1-2년 재발률을 10% 미만으로 줄일 수 있게 되

었으며, 비용-효과적인 측면에서도 고용량 HBIG 단독요법에 비해 우수하였다.19-22 6개의 독립적인
연구결과를 종합하여 메타분석을 시행한 결과 라미부딘과 HBIG 병합요법은 HBIG 단독요법에 비해
B형 간염의 재발률 및 이와 연관된 사망률을 각각 12 배 낮추는 것으로 확인되었다.
(2) 저용량 HBIGHBIG은 고비용, 감염 위험성, 납 중독 등의 문제가 있어 anti-HBs 역가에 따라 저용량의 HBIG을 적
용였을 때 고용량 HBIG과 효과가 유사함이 보고되었다. Angus 등은 라미부딘과 함께 첫 주에 저용량
의 HBIG을 400 또는 800 IU로 매일 근주한 뒤 이후 매달 근주하였는데 18.4개월 동안 추적 관찰하였을
때 32명 중 31명이 HBsAg 음성으로 유지되었고 모든 환자가 HBV DNA 음성으로 유지되었다.23 2개의
meta-analysis에서 라미부딘과 저용량 HBIG 병합요법은 라미부딘 단독 또는 HBIG 단독요법에 비하
여 모두 B형 간염 재발의 위험도가 낮았다.24,25 이상의 결과에 근거하여 라미부딘과 HBIG 병합요법
이 간이식 후 B형 간염 재발의 예방을 위한 표준치료로 권고되고 있으나 HBIG의 적정 용량, 투여 경
로, 목표 anti-HBs 역가 등에 대한 추가 연구가 필요하며, 저용량의 HBIG이라고 하더라도 주기적인
추적 검사, 비경구 투여의 불편 및 비용 등은 여전히 해결이 필요한 문제이다. 최근에 바이러스 억제
력이 강력하고 약제 내성이 적은 엔테카비어 또는 테노포비어를 HBIG 와 병합 치료하여 좋은 성적을
보여준 초기 연구 결과가 있어 향후 이러한 약제들의 역할이 더욱 기대된다.26
(3) HBIG withdrawal
① 라미부딘 단독 유지
상술한 HBIG의 문제점으로 인하여 몇 몇 연구에서 HBIG을 중단하고 라미부딘만 단독으로 유지
하는 요법을 평가하였으나 라미부딘 내성 바이러스 발생이 여전히 우려된다. 한 전향적 연구에서 29
명의 환자를 간이식 후 1개월 째 까지는 HBIG과 라미부딘 병합치료 한 뒤 병합 치료군과 라미부딘 단
독 유지군으로 무작위 배정을 하고 18개월 간 추적하였을 때 양 군에서 B형 간염의 재발은 관찰되지
않았다.27 그러나 이후 추적 관찰에서는 15%의 환자에서 B형 간염 재발이 관찰되었는데 위험도는 양
군 사이에 유의한 차이가 없었다.28
② 라미부딘과 아데포비어 병합
전향적 연구에서 간이식 후 최소 12개월이 경과한 시점에 저용량 HBIG을 아데포비어로 전환하였
을 때 16명의 환자는 라미부딘/아데포비어 병합 치료를 받았고 18명의 환자는 라미부딘과 저용량
HBIG 병합요법을 유지하여 21개월을 추적 관찰하였는데 양군 모두에서 B형 간염이 재발한 환자는
없었다.29 유사한 다른 연구에서는 47명의 환자에서 간 이식 후 라미부딘과 HBIG을 병합투여 한 뒤 12
개월 경과 시점에 28명의 환자에서는 HBI을 중단하면서 아데포비어(n=23) 또는 테노포비어(n=5)로
치환하여 라미부딘과 병합치료 하였고, 나머지 19명의 환자에서는 테노포비어 단독(n=10) 또는 엔
테카비어 단독(n=9) 요법으로 전환하였는데 모든 군에서 24개월 추적 기간 동안 HBV DNA가 음성으
로 유지되었다.30 또 다른 연구에서는 라미부딘 내성 바이러스에 감염된 16명의 B형 간염 환자에서 간
이식 후 라미부딘과 아데포비어의 병합 치료를 시행하였는데 8명은 HBIG을 중단하면서 라미부딘에
아데포비어를 병합하여 치료하였고 나머지는 HBIG을 사용하지 않고 처음부터 라미부딘/아데포비

어 병합치료를 시행하였을 때 21개월의 추적 관찰 시 양군에서 HBV DNA는 검출되지 않았다.31
③ 테노포비어/엠트리시타빈
21명의 환자에서 간이식 후 HBIG을 6개월 이상 투여하고 중단한 뒤 테노포비어/엠트리시타빈으
로 교체하였을 때 31개월 추적 관찰 시 21명 전원에서 HBV DNA는 검출되지 않았고 20명에서는
HBsAg이 음성으로 유지되었다. 21명 중 3명에서 신기능 장애가 발생하였는데 2명은 tacrolimus가 원
인이었고 1명은 테노포비어가 원인이었다.32 비록 소규모 연구이나 바이러스 억제력이 강력하고 약
제 내성이 적은 약제가 HBIG을 대체할 수 있다는 가능성을 시사한다.
3. 간이식 후 재발한 B형 간염의 치료
간이식 전/후 예방적 치료에도 불구하고 B형 간염이 재발한 경우 라미부딘 치료는 비교적 효과적
이지만, 장기간 사용하였을 때 3 년에 50% 이상의 내성률이 보고되었으며, 이러한 라미부딘 내성은
간 이식의 성적을 저하시키는 것으로 알려져 있다.33-35 최근 간이식 후 재발한 B형 간염 치료에 있어
테노포비어와 엔테카비어의 효과에 대한 소규모 연구가 보고되었으나,36 잘 계획된 대규모 연구 결
과가 필요하다. 간이식 후 라미부딘 내성을 보이는 241명의 재발성 B형 간염 환자를 라미부딘과 아데
포비어의 병합요법으로 치료하였을 때 96주 후 65%의 HBV DNA 음성화율 및 2%의 라미부딘 내성률
을 보였다.37 비록 소규모의 단기간 연구 결과들이지만 최근 간이식 후 라미부딘 내성 변이종의 치료
에 있어 테노포비어가 좋은 성적을 보고한 바 있어 향후 추가적인 연구 결과가 주목된다.35 하지만 엔
테카비어의 경우 이미 라미부딘 내성 환자에서 구제요법으로 단독 치료하였을 때 높은 엔테카비어
내성발생률을 보고된 바 있어,38 간이식 환자에서도 라미부딘 내성을 보이는 경우에는 추천되지 않
는다. 따라서 간이식 후 B형 간염이 재발하는 경우 바이러스 억제력이 강력하고 약제 내성이 적은 항
바이러스제를 사용하며, 만약 약제 내성이 있는 경우에는 만성 B형간염 치료 가이드라인을 따른다.7
간이식 환자에서 B형 간염의 예방 및 치료에 대하여 Table 1에 정리하였다.

4. HBV에 대한 숙주 면역 반응 복원
1) 예방접종
간이식 후 B형 간염 예방 접종을 시행하였을 때 82.4%에서 혈청전환이 일어나고 14개월 추적 관찰
시 HBsAg이나 HBV DNA의 재발이 관찰되지 않음을 보고한 연구가 있으나39 다른 연구에서는 반응률
이 17.6%에 불과하였고,40 또 다른 연구에서는 이보다 더 낮은 7.7%였다.41 그러나 특정 환자군에서는
높은 반응률을 보이는 경우가 있어 이에 대한 연구가 필요하다.
2) Adoptive immunity transfer
홍콩에서 수행된 연구에서 간이식을 받은 50명 중 21명(42%)에서 간이식 후 HBsAg이 소실되고 an-
ti-HBs가 자연적으로 나타나는 것이 처음으로 보고되었다.42 21명 중 19명은 anti-HBs가 양성인 공여
자로부터 간을 공여 받았으며 공여 받은 간을 통한 adoptive immunity의 전달에 의한 현상으로 해석된
다. 동일한 연구자들이 후속 연구를 통하여 공여된 간을 조사하였을 때 공여자에서 기인한 HBV 특이
T 림프구 와 B 림프구가 각각 59%, 28%에서 관찰됨을 보고하였다.43 일부 환자에서 anti-HBs 역가가
1,000 IU/mL를 넘었지만 이후 7개월에 걸쳐 감소하는 것이 관찰되어 B형 간염에 대한 평생 면역이 형
성되었다고 말하기 어렵다. 따라서 향후 면역력의 강도와 지속 시간을 향상시키는 방안에 대한 연구
가 필요하다.
면역 억제 환자의 B형 간염 바이러스 치료
1. 서론
B형 간염의 효과적인 예방 백신 및 수직 감염 예방법의 개발 이후 전세계적으로 B형 간염 바이러스
보유율은 점차 감소하고 있는 상황이다. 그러나 아직까지 HBsAg 양성률은 서구에 비하여 유의하게
높으며 HBsAg은 음성이더라도 anti-HBc IgG 가 양성인 경우나, 혈중 HBV DNA 농도가 1,000 IU/mL 미
만이고 혈청 간 효소 수치가 정상 범위에 있는 B형 간염 건강 보유자의 경우에도 면역억제제나 항암
화학요법제를 사용하는 경우 HBV 증식 및 면역 체계의 활성이 모두 영향을 받을 수 있고 이에 따라
간염이 악화될 수 있다.44-46 HBV 재활성화는 보통 혈중 HBV DNA 농도가 기저치의 10배 이상 또는 108
IU/mL 이상으로 상승하면서 혈청 간 효소 수치가 정상 상한치의 3배 이상 또는 100 IU/L 이상으로 상
승하는 것으로 정의된다.47,48 HBV 재활성화로 진단하기 이전에, 항암제의 사용과 관련된 간 손상, 간
전이, 또는 다른 바이러스에 의한 간염 등을 감별하여야 한다. HBV 재활성화의 빈도는 약 20-50%로
알려져 있는데 대부분 초기에 무증상인 경우가 많으나, 일부는 황달과 비대상성 간부전의 증상을 동
반하는 임상적 악화를 보이며 간부전에 의하여 사망에 이르기도 한다.47,49-51 특징적으로, HBV DNA
는 면역이 억제되는 시기에 나타나기 시작하고 면역억제제의 사용을 중단한 뒤 ALT의 상승을 동반
한다. 만약 HBV 재활성화가 악성 종양으로 항암화학요법을 시행 받는 환자에서 발생하게 되면 항암
치료의 중단 또는 조기 종료에 의하여 항암치료의 성적에 부정적인 영향을 미칠 수 있다.52-54

2. HBV 재활성화의 위험 인자
HBV 재활성화의 위험인자로는 치료 전의 혈중 HBV DNA 농도, 악성 종양의 종류, 면역억제제 또
는 항암화학요법의 종류와 강도 등이 보고되고 있다. 예를 들면 HBV 재활성화는 다른 고형암
(14-21%)에 비하여 비호지킨 림프종 환자에서 24-67%의 빈도로 빈번하게 관찰되는데 림프종의 치
료에 사용되는 항암요법의 강도가 강하며 림프종 환자에서 HBsAg 양성 빈도가 더욱 높게 나타나기
때문이다.50,55-57 유방암 환자에서도 41-56%의 빈도로 높게 관찰되며 고농도 항암제의 사용 및 an-
thracycline 제제의 사용과 관련이 있다.58,59 부신피질 호르몬제는 면역을 억제하여 HBV 재활성화의
위험도를 증가시키는 동시에 HBV 복제를 직접 자극하는 것으로 알려져 있다. Rituximab은 부신피질
호르몬제와 함께 림프종의 치료에 흔히 사용되는 약제로서 HBV 재활성화의 위험을 더욱 증가시킨
다.60,61 재활성화의 위험도는 혈액종양 환자가 조혈모세포 이식 전 고강도 항암요법을 받는 경우 상
승하게 된다.62,63 이 외에도 HBV genotype 또는 HBV genome의 특정 돌연변이 등이 재활성화와 관련되
어 있다고 알려져 있다.64-66
3. HBV 재활성화의 치료
HBV 재활성화는 예방이 중요한데 항암화학요법 시행 전 HBsAg과 anti-HBc IgG 에 대한 선별 검사
를 시행하여 양쪽 모두 음성인 경우 예방 접종을 시행하고, HBsAg 양성 환자에서는 혈중 HBV DNA
농도에 관계 없이 항바이러스 제제의 예방적 투여를 시행한다.66 라미부딘의 예방적 투여는 HBV 재
활성화 및 이로 인한 간 부전과 사망을 유의하게 줄이는 것으로 보고되었다.56,63,67,68 따라서 HBV DNA
가 상승할 때까지 기다리기 보다는 항바이러스 제제를 예방적으로 투여하고 항암제 치료 종료 후 최
소 6개월에서 12개월 까지 상당 기간 동안 유지하는 것이 추천된다.68-70 예방적 항바이러스 제제의 정
확한 투여 기간에 대해서는 다소 이견이 있으나 라미부딘을 3개월 미만으로 투여한 경우 항암 치료
전 혈중 HBV DNA 농도가 2,000 IU/mL를 초과했던 경우 특히 HBV 재활성화의 위험이 높았다. 반면
HBV DNA 농도와 관계 없이 6개월 이후에도 재활성화에 대한 주의가 필요하다. 라미부딘의 예방적
투여에 의해서도 내성 바이러스가 발생할 수 있는 것으로 보고되어, 바이러스 억제력이 강력하고 약
제 내성이 적은 항바이러스제의 장기간 예방적 투여에 대한 연구가 필요한 실정이다.56 아시아에서
수행된 후향적 다기관 연구에서는 HBsAg 양성인 림프종 환자를 대상으로 rituximab이 포함된 항암요
법을 시행하기 전 엔테카비어와 라미부딘을 예방적으로 투여하였을 때 HBV 재활성화의 비율이 엔
테카비어 군에서 유의하게 낮음을 보고하였다(6.3% vs. 39.3%; P<0.05).71 인터페론은 골수의 억제 및
간염의 악화 가능성이 있어 금기에 해당한다. HBsAg이 음성이지만 anti-HBc IgG 양성인 환자(HBV 잠
복/과거 감염)에서도 HBV 재활성화가 가능하지만, 기저 암종 및 항암요법 제제의 종류에 따라 재활
성화의 위험도가 달라지기 때문에 이러한 환자들에게 통일된 권고안을 제시하기는 어려운 상황이
다. 그러나 rituximab이 포함된 항암요법을 받는 림프종 환자와 같은 고위험 군에서 혈중 HBV DNA가
검출되는 경우에는 예방적 항바이러스 제제의 투여가 필요하며 조혈모세포 이식을 시행한 백혈병
환자에서는 기저 HBV DNA가 검출되지 않더라도 매 1-2개월 마다 HBV DNA를 검사하여 예방적 항
바이러스 제제의 투여를 결정하는 것이 좋다.

항바이러스 제제를 얼마 동안 유지할 것인지에 대해서 유럽 간학회 권고안에서는 12개월을 유지
하도록 권고하고 있으나 근거 논문이 라미부딘을 대상으로 한 것이어서 엔테카비어나 테노포비어
를 대상으로 하는 연구 결과가 필요하다.51,56,70 또한 미국 간학회의 권고안에서는 사용 기간이 12개월
미만으로 예상될 경우 라미부딘이나 텔비부딘도 사용할 수 있으며 그 이상의 기간 동안 사용해야 될
경우 엔테카비어나 테노포비어를 권고하고 있다.72 그러나 이러한 권고안들을 지지하는 근거 자료가
부족한 실정이다. 미국간학회, 유럽간학회, 그리고 아시아태평양간학회 권고안의 차이점을 Table 2
에 정리하였다.70,73
4. B형 간염 재활성화의 최근 연구 성과 및 전망
Pei 등은 29명의 림프종 환자를 대상으로 시행한 후향적 연구에서 HBsAg가 음성이고 기저 anti-HBs
역가가 100 mIU/mL 미만인 경우 rituximab 치료 후에 anti-HBs 항체 역가가 더욱 감소하고 HBV 재활

성화의 위험도가 증가함을 보고하였다.74 캐나다에서 시행된 항암 치료 전 HBV 선별 검사에 대한 비
용-효과 분석에서는 이전의 분석 결과와 달리, rituximab이 포함된 항암치료를 받는 모든 림프종 환자
에서 치료 전 HBV 감염 여부에 대한 선별 검사를 시행하였을 때 HBV 재활성화가 10분의 1로 감소됨
을 보여 비용-효과적이라고 분석하였다.75
B형 간염 재활성화의 예방에 대한 연구는 대부분 라미부딘을 대상으로 하고 있어 라미부딘의 높
은 내성률을 고려할 때 최근에 개발된 강한 바이러스 억제 효과와 낮은 내성률을 보이는 약제들에 대
한 연구가 필요하다. 몇 몇 후향적 연구에서 엔테카비어나 텔비부딘이 라미부딘에 비하여 우월한 효
과를 보인다고 보고하였다.76,77 이와 관련하여 림프종 또는 고형암 (유방, 대장, 위, 폐암) 환자에서 엔
테카비어와 라미부딘의 효능을 비교하는 전향적 다기관 무작위 배정 임상이 진행 중이어서 결과가
주목된다 (NCT01580202).
결 론
만성 B형 간염 환자는 간이식 후 B형 간염이 재발할 수 있고 이식 간에 빠르게 섬유화가 진행하는
등의 부작용으로 인하여 간이식의 성적이 저하된다. 따라서 B형 간염의 재발을 예방하기 위하여 라
미부딘과 HBIG을 단독 혹은 병합하여 치료하게 된다. 그러나 라미부딘은 내성 바이러스의 발생률이
높아 강한 바이러스 억제 능력과 낮은 바이러스 내성률을 보이는 항바이러스 제제들로 대체하는 연
구가 진행중이다. 한편 HBIG의 경우 고비용, 감염의 위험 및 투여 방법의 불편 등으로 인하여 HBIG
의 용량을 줄이거나 사용기간을 단축하고 다른 항바이러스 제제로 전환하는 등의 다양한 요법이 연
구되고 있다.
만성 B형 간염 환자가 항암화학요법을 받는 경우 항암치료 중이나 종료 후에 HBV 재활성화의 위
험성이 높아진다. 이러한 환자들에게 예방적 항바이러스 제제의 투여가 권고되나 대부분 내성 발생
률이 높은 라미부딘을 대상으로 이루어진 연구에 근거하고 있으며 최근에 개발되어 임상에 적용 중
인 강한 바이러스 억제 능력과 낮은 바이러스 내성률을 보이는 약제들에 대한 연구는 부족한 실정이
다. 향우 어떤 약제를 얼마의 기간 동안 사용하는 것이 더 나은 결과를 보일 것인지에 대한 연구가 필
요하다.
