Getting Started
Before You Begin…
Before you can complete this procedure, the following conditions must be met:
• You must be a Facility Editor or Network Patient Editor to add, update, and delete clinical information for patients at a facility within your scope.
• All other roles can view patient clinical records for patients at facilities within their scope.
• The patient must be admitted to the facility and have a treatment record in the selected modality (HD or PD) during the selected clinical month (including Transient patients).

Facilities must meet CMS’ clinical submission guidelines by submitting all available clinical data that is requested by CROWNWeb, a minimum of once per month for each patient in the facility.
If no lab data is available, you can satisfy the submission guidelines by using the Not Available (NA) checkbox.
CROWNWeb does not have a report that summarizes the data that has been submitted for all patients that month. You will need to view the clinical screens for each patient individually to validate this information.

When you access the Manage Patient Clinical Values screen, a gray menu bar displays below the main menu. Selections in this menu are dependent on the Collection Type selected:

Selecting a link in the gray menu bar takes you directly to that clinical area on the screen, so you can quickly begin to enter data without scrolling.

When you are finished with entering clinical data, click any Submit button.

If the data passes validation, the Manage Patient Clinical Values screen displays the message, “Successfully submitted clinical data.”

If no clinical data was collected during the reporting month forhemodialysis, vascular access or peritoneal dialysis, click the No Clinical Data Available For All Collection Types checkbox, and then click Submit.
Acceptable reasons for using this checkbox include:
• Patient was hospitalized for the entire reporting month and had not been discharged
• Patient was discharged from your facility after the first of the reporting month without having been treated that month

If you did not enter any required data, CROWNWeb displays warning messages (as shown below). If you do not have any clinical information for this patient, click the checkbox and then click Submit again.

All fields on the Manage Patient Values Clinical screen are required. If data for a specific field(s) is not available, click the N/A checkbox for that specific field and the field and associated Date field will be grayed out.

Transient patients will appear on your clinical list(s). If you drew labs while the patient was in a transient status at your facility, enter that data. If no labs were drawn, use the individual NA checkboxes to indicate this.
Vascular Access data for transient patients would be entered reflecting the access used during the last treatment of the reporting month while they were at your facility.

If you do not have all of the clinical information for a patient, youhave the ability to save your information and then complete it later.NOTE: When you save clinical information, fields that have been completed are not validated. Validation will only occur when you Submit clinical information. Remember that clinical data in a saved status will not be counted as having completed and submitted data for that reporting month.

Patient vascular access data carries over for a Home facility from month-to-month, but not when a patient is a transient at another facility. The transient facility can find vascular access data for a transient patient using the following steps:
1. From the patient’s View Patient Attributes screen, click on the Clinical Data Summary link in the gray sub-menu.
2. Click on a previous Clinical Month’s link to view that month’s clinical lab data for the patient.
3. Under Vascular Access, you can view and note the Date Access Type Changed, AV Fistula Usable Date and/or Fistula Creation Date fields, and enter that data in the transient patient’s Vascular Access fields.
4. Complete any of the other required fields in Vascular Access.

Delete Clinical Information
If you accidentally enter the wrong clinical information for a patient, you can delete that record.
