Receive Email “Invitation to edit”Click “Open in Forms”

Go to “File” and select “Make a copy”

Name Your Survey & Click “OK”

Customize instructions Add Your Deadline

Use the template paper survey at PTA.org/ExcellenceTools
to find areas to edit such as “(Your PTA Name).”
Click “SEND” to copy and share your survey link

Click Link and check “Shorten URL”Click COPY to save your link

description and deadline.
