INTRODUCTION

ESF 8 provides a multitude of health services to supplement and support disrupted or

overburdened local medical personnel and facilities and to relieve personal suffering and trauma

during a disaster event.

Public Health and medical services refers to emergency and resident medical care; doctors,

technicians, supplies, equipment, ambulance and emergency medical services, hospitals, clinics,

and units, planning and operation of facilities and services. Pre-hospital emergency services and

medical transport refers to the coordination of Emergency Medical Services (EMS)

professionals, equipment and supplies to address disaster response surge, triage, treatment and

transport.

PURPOSE

The purpose of this ConOps is to provide overarching guidelines and protocols for EMS support

of Louisiana Medical Special Needs Shelters (MSNS) and Critical Transportation Needs Shelters

(CTNS). This document outlines shelters operations, EMS and shelter chain of command, and

roles of EMS within the shelters.

Shelters

There are three types of shelters where EMS resources will be placed in order to assist with

interior operations and to aid in the event of an evacuee needing medical transportation. They are

Medical Special Needs Shelters, Critical Transportation Needs Shelters, and Federal Medical

Stations.

Medical Special Needs Shelters Medical Special Needs Shelters (MSNS), managed by the Office of Public Health (OPH), are

congregate facilities that provide safe refuge to individuals who have medical needs that they

cannot manage themselves (require assistance) and who do not have anywhere to evacuate.

These individuals should have a caregiver with them at the MSNS. Medical Special Needs

Shelters are temporary living arrangements that provide medical monitoring and limited medical

care only until the emergency is over.

EMS will provide staff for both transportation as well as assistance inside MSN shelters.

Critical Transportation Needs Shelters

Critical Transportation Needs Shelters (CTNS), managed by the Department of Children and

Family Services (DCFS), are dedicated to people who do not have transportation for self-

evacuation. Parish pick up points are established in key locations in South Louisiana and

transportation is provided by para-transit vehicles. Evacuees are sorted to either CTNS or MSNS

shelters at the pick-up points. All para-transit vehicles are routed through Baton Rouge to

Louisiana State University where evacuees are evaluated to determine whether they are

medically able to continue to the CTNS destination or if they need to be removed to go to a

MSNS or hospital.

EMS will provide staff for both transportation as well as assistance inside CTN shelters.

Federal Medical Stations The Federal Medical Station (FMS) is a cache of medical supplies and equipment that are

packaged into 50-bed modules, and can be used to set up a temporary non-acute medical care

facility. The FMS equipment is provided by the Center for Disease Control and Prevention

(CDC) Division of Strategic National Stockpile (DSNS) and pre-staged in Louisiana for a total

of 1250 available cots with supplies.

The staffing for the FMS with the necessary skill set is provided by Health and Human Services

(HHS). EMS will provide transportation from the FMS when requested.

Operations

Resource Placement

Louisiana Bureau of EMS (BEMS) provides ambulance support for the following medical

operations in a state declared emergency:

Evacuations

Search and rescue

Surge operations

Shelter operations

While shelter standby is an important aspect of EMS operations, patient evacuations and search

and rescue operations take priority during a disaster. Shelter Operators should be aware that

because of this priority, the number of ambulances assigned to a shelter may be reduced if EMS

resources are limited.

In general, BEMS will be providing EMS resources for the following State shelters:

Resource Details Each Advanced Life Support (ALS) crew has two personnel. At a minimum, there is one

Paramedic assigned to the crew and one Emergency Medical Technician (EMT) assigned to the

crew. Paramedics have an expanded scope of medical practice in comparison to that of an EMT.

Neither Paramedics nor EMTs are permitted to operate outside of the scope of practice.

ALS crews operate out of their ambulance. Each ALS ambulance meets the minimum

specifications regarding functionality, equipment, and supplies in accordance with LAC

48:I.Chapter 60. ALS crews are able to function based upon both standing protocols as well as

online medical direction. All shelter crews will function in accordance with the BEMS Disaster

Protocols. These protocols include policies and procedures for both emergent and non-emergent

operations.

Resource Chain of Command EMS crews will report through their chain of command as follows:

The EMS Shelter Strike Team Leaders will oversee between 4-6 units each. Each ALS crew will

report to their EMS Shelter Strike Team Leader. Each ALS crew will have a Shelter point of

contact (POC). At the MSNS, this will be the Shelter Medical Director. At the CTNS, this will

be the Planning Section Chief. The points of contact will provide EMS crews with instructions

regarding set up and operations specific to their shelter.

Resource Operations

1. ALS crews will arrive at the beginning of the operational period for which they are

assigned.

a. Off going crews will relay a shift report to the oncoming crews which will

include:

i. Name of the Shelter Coordinator POC for the oncoming operational period

ii. Any incidents from the previous operational period

iii. Any changes in policy/procedure/protocol

b. Oncoming crews will check their station at the shelter to ensure that all tools

necessary for operations are in working order

c. Oncoming crews will report to the Shelter POC to introduce themselves and

receive any instructions for the current operational period

d. At the commencement of each operational period, Shelter Strike Team Leaders

will be issued the following (which shall be shared with crews):

i. Incident Action Plan which will include:

1. Incident Objectives

2. Assignment List

3. BEMS Communication Plan

a. Shelter Units will be provided an operations channel

4. Medical Plan

5. Organizational Chart

2. All crews are expected to be inside of the shelters assisting with daily operations. These

operations may not be medical in nature, but crews are expected to assist as needed.

a. If Shelter POCs have any issues with EMS crews, they are to report them to the

Shelter Strike Team Leader assigned to that crew.

b. If EMS crews have any issues at the shelter where they are assigned, they are to

report them to their Shelter Strike Team Leader.

3. If a crew must transport a patient to a hospital, they must notify their Shelter Strike Team

Leader that they will be transporting, where they will be transporting, and whom they

will be transporting.

4. EMS crews are to have their Stretchers, Monitors, Airway kits, and Medication kits

inside the shelters in the event of a medical emergency.

a. Crews must be responsible for their equipment at all times.

b. DHH/OPH/BEMS is not responsible for any equipment that is lost or stolen.

5. If a shelter is in need of more EMS resources or current EMS resources are no longer

needed, the request should be sent through either the DCFS chain of command (CTNS)

or the OPH chain of command (MSNS). The BEMS MACC will coordinate all EMS

resource requests once received.

6. If EMS crews from opposite operational periods are sharing an ambulance and

equipment, then the Shelter Strike Team Leader will transport oncoming/off-going crews

to and from their billet(s).

a. EMS crews will provide their own food, water, and supplies for the duration of

the event. Any resources needed by EMS crews must be conveyed through the

crew’s Shelter Strike Team Leader.

i. Crews should not be fed with shelter meals unless approval is given

explicitly by the BEMS MACC.

b. EMS crews will be provided with a location to quarter.

Conclusion

The Bureau of EMS is dedicated to ensuring that EMS Shelter resources are effectively placed

and that crews operate as valuable assets when it comes to aiding in shelter operations.
