Although the surgical procedures are performed at Memorial Medical Center in Springfield, all pre- and post-operative care is offered on the BroMenn campus. BroMenn offers bariatric support services including: nutrition counseling, behavioral health care, physical therapy, laboratory and imaging services—all offered within an interdisciplinary team format led by experienced bariatric surgeons. Memorial Bariatric Services’ surgeons offer a pre- and post- bariatric surgery care clinic twice monthly on the BroMenn campus for the convenience of area patients. THE PROBLEM OF OBESITY Obesity is a chronic disease affecting approximately one in four Americans. It is considered the second leading cause of preventable death and disease (second only to smoking) in our country. People with obesity often experience related medical conditions such as type II diabetes, high blood pressure, cardiovascular disease, painful joint problems, breathing/lung problems and depression, among others. Approximately 9 percent of Americans suffer from clinically severe obesity—obesity severe enough to dramatically increase risk for other diseases, disability and premature death.

HEALTHQUALITY OF LIFEDO I HAVE CLINICALLY SEVERE OBESITY?

a method of estimating an individual’s degree of obesity is the Body Mass Index or BMI. Clinically severe obesity is defined as a BMI of at least 40 or a BMI of at least 35 if there are certain other medical conditions present (e.g., high blood pressure). Non-surgical treatments for weight loss (diet, exercise and behavioral modification) have very limited effectiveness for those with clinically severe obesity. (Ninety-five percent will re-gain the lost weight within five years.) For those with clinically severe obesity, bariatric surgery offers a more effective treatment for long-term and substantial weight loss.

HOW EFFECTIVE IS BARIATRIC SURGERY?

Weight loss results depend on a variety of factors including pre-operative weight, patient age, the type of procedure performed, and patient ex-ercise and eating behaviors after surgery. Pub-lished data on weight loss have shown that most patients who undergo weight loss surgery lose a substantial amount of weight and maintain most of the weight loss 10 years after surgery. health status and quality of life are usually improved with dramatic, sustained weight loss. Conditions such as type II diabetes, high blood pressure, acid reflux, sleep apnea, urinary stress inconti-nence and depression typically improve.

WHAT ARE THE RISKS?

all surgical procedures pose risks for complica-tions, and one must carefully consider the po-tential for good results against those risks. also, with bariatric surgery, close medical follow-up, including monitoring of nutritional status, will be a life-long requirement.

WHO IS A CANDIDATE FOR BARIATRIC SURGERY?

Patients must meet the following criteria to be considered for bariatric surgery with Memorial Bariatric Services at Bromenn:•Age21–60(gastricbypass,sleevegastrectomy)•Atleast18yearsofage(LAP-BAND®)•BodyMassIndex(BMI)greaterthanorequal

PRE-SURGERY EVALUATION AND PREPARATION

all prospective bariatric surgery patients must undergo careful evaluation and preparation with the interdisciplinary team. In addition to an evaluation from the surgeon, patients must undergo mental health, nutrition and physical activity evaluations. other medical specialists may be involved as needed. Patients are provided detailed information about surgical options, realistic expectations for results, possible side effects and risks of complications.

SURGERY AND HOSPITALIZATION

Generally, the patient is admitted on the day of surgery. all bariatric procedures are performed under general anesthesia. the LaP-BaNd® procedure is usually done on an outpatient basis. Patients undergoing the sleeve procedure or the gastric bypass are hospitalized for two to three days. Patients typically return to work between one and four weeks after surgery.

BARIATRIC SURGERY PROCEDURES

Memorial Bariatric Services at BroMenn offers three forms of bariatric surgery—the adjustable gastric band (LaP-BaNd®), the vertical sleeve gastrectomy, and the roux-en-Y gastric bypass (gastric bypass). the LaP-BaNd® and sleeve procedures work by limiting the amount of food that can be eaten at a sitting. the gastric bypass limits the amount of food consumed at a sitting aNd limits the amount of calories that can be absorbed into the body. all three procedures are performed laparoscopically. Laparoscopic surgery is performed through several very small incisions, rather than one large incision and means shorter recovery time and less pain for the patient. all surgical procedures are performed at Memorial Medical Center in Springfield.

POST-SURGERY CARE

Patients require very close and long-term monitoring of medical and nutritional status after bariatric surgery. this needed follow-up care is available at BroMenn and includes a monthly support group led by a health care professional.

FINANCIAL CONSIDERATIONS

Most patients are understandably concerned about the financial costs associated with bariatric surgery. In addition to the surgery itself, there are costs associated with the needed pre-operative evaluations and after-care requirements. Patients seeking bariatric surgery should be aware that medical insurance coverage of such services varies from policy to policy. Patients are advised to contact their insurance carriers to inquire about coverage, not only for the surgery and hospitalization, but also for the pre-operative evaluations and post-operative care.
