UnitI-TheoreticalConceptsandPrinciplesofMentalHealthNursing,TherapeuticRelationships/Communication,NursingProcess,LegalandEthicalIssues,&CultureThisunitpresentsanintroductiontopsychiatricnursingandananalysisofthedominantconceptualmodelsforpsychiatricnursingpractice.Studentnursesbegintoexploreandcultivatetheirownpersonalintegrationwithinthatpractice.Criticalthinkingskillsareemphasizedandtherapeuticcommunicationstrategies,developmentaltheories/therapiesandpatientassessmentmethodologiesarepresented.Abio-psycho-socialapproachisusedinexaminingprinciplesofpatientcenteredcare,thementalhealth-psychiatricillnesscontinuumandthenursingprocessasrelatedtopsychiatricdisorders.Thepsychobiologicalbasisofbehaviorisreviewed.Theconceptsofculture,ethnicity,worldviewandculturalcompetencyareappliedtomentalillness.Thelegalandethicalguidelinesforsafepracticeareidentified.Additionally,theconceptofneurotransmittersinrelationtopsychiatricillnessandpsychotropicmedicationareintroduced.

UnitII–Stress,SchizophreniaSpectrumDisorders,Substance-RelatedandAddictiveDisorders,Anger,Aggression,&ViolenceTheconceptsofstress,stressmanagement,psychoneuroimmunology,andthegeneraladaptationtheoryarepresented.Contemporaryetiologictheories,pathophysiologytreatments,andnursinginterventionsforthepatientwithschizophreniaandschizophreniaspectrumdisordersareinvestigated.Theroleofthebiologicaltherapies,includingpsychotropicmedicationandrelatedsideeffectsareexplored.Conceptscentraltosubstanceusedisordersarepresented.Theneurotransmittersinvolvedinthebrainrewardsystemactivatedinsubstanceusedisordersaredelineated.Thetheoryandneurobiologyofangerandaggressivebehaviorisexploredalongwiththeprinciplesofde-escalation.UnitIII-BipolarDisorders,DepressiveDisorders,Trauma/Stressor-related&DissociativeDisorders,SuicideThisunitexploresthemajormooddisorders,includingdepression,bipolardisorder,dysthymiaandcyclothymia.Suicideassessmentandinterventionarediscussed.Majortherapiesandnursinginterventionsforthemooddisordersareexamined.Assessmentandnursingcareofthepatientwithtrauma,stress-relatedanddissociativedisordersarepresented.Theuseofthegroupprocessinthementalhealthsettingisexploredwithanemphasisongrouptherapy.Methodologiesforgroupinteractionareexamined,asaregroupleadershipstyles.UnitIV-Anxiety&AnxietyRelatedDisorders,PersonalityDisorders,TherapeuticGroups,FamilyInterventionsAdaptiveandmaladaptivelevelsofanxietyarediscussed,asaredisturbedpattersofcopingwithanxiety,includingthenon-psychoticclinicalsyndromes.Assessmentandnursinginterventionsforeachlevelofanxietyaredelineated.Adaptiveandpathologicmanifestationsofdefensemechanismsareexplored.Thisunitpreparesthelearnertorecognizerigidpatternsoflivingmanifestedinpersonalitydisorders.Theuseofthegroupprocessinthementalhealthsettingisexploredwithanemphasisongrouptherapy.Methodologiesforgroupinteractionareexamined,asaregroupleadershipstyles.Assessmentoffamilystructure,phasesofthefamilylifecycle,andhealthyversusunhealthyfamilyfunctionarepresented.Differenttypesoffamilytherapyareexplored.UnitV–SomaticDisorders,EatingDisorders,Crisis&Disaster,Child,OlderAdult&IntimatePartnerViolence,SexualAssaultPsychologicalstatesthatmanifestphysicallythroughsomaticsymptomdisordersaredelineated.Assessment,evidence-basedtreatmentoptionsandnursinginterventionsforthepatientwitheatingdisordersareinvestigated.Theoreticalconceptsassociatedwithsexualassault,abuseandintimatepartnerviolencearediscussed.Legalresponsibilitiesforreportingarespecified.Adaptiveandineffectiveprocessesrelatedtocrisisanddisastersareexaminedalongwithspecializedinterventionsthatreturnthepatienttoprecrisisfunction.

PREREQUISITES:Forgenericstudents:agradeofCorbetterinfirst,secondandthirdsemesterNursingCoursesandrequiredgeneraleducationcourses.ForLVNtoRNstudents:admissiontotheregisterednursingprogramandsuccessfulcompletionofthethirdsemesterorbyspecialadmissionbytheAdmissionCommittee/NursingSuspensionandReadmissionCommittee.TransferstudentsmustmeeteligibilityrequirementsasdeterminedbytheAdmissionCommittee/NursingSuspensionandReadmissionCommittee.Allstudentsmustdemonstratesafe,correctandcompetentperformanceofclinicalskillstaughtinallpreviouscourses.Studentsarerequiredtoreviewandtobeknowledgeableofcommunicationtheoryandtechniques.

STUDENTLEARNINGOUTCOMES/COMPETENCIES:Atthislevel,whichcomprisescoursesinthethirdandfourthsemesterofthenursingprogram,studentsareexpectedtocontinuetoapplyandadaptmedicalsurgicalnursingconceptstopatientsacrossthelifespaninavarietyofcommunitybasedhealthcaresettings,modifyplanofcareandmakedecisionsforpatientsatavarietyofdevelopmentalstagesonthebasisofgeneralguidelinesorprinciplesderivedfrompreviousexperiences,organizeandprioritizenursinginterventionswithsupervision,anduseappropriateresourcestoassistinsolvingpatientproblems.Thestudentcanadapttodifferentagegroupsbasicskillsanddevelopsnewskillsapplyingguidelinesthatarebasedoncuesfromexperts.Theyattempttocorrelateandbuildonmedicalsurgicaltheoryandpractice.ProgramLearningOutcomesforlevelthreeare:

1. ApplythenursingprocessusingtheRoyAdaptationModelincaringforindividualsandgroupsacrossthelifespananddevelopmentalstages.

2. Practiceprofessionalbehaviorstandardsofnursingpractice.3. Demonstrateclinicaldecisionmakingthatisaccurateandsafe.4. Providesafe,patient-centeredcare.5. Functioneffectivelywithinnursingandinterprofessionalteamsutilizingeffective

communicationstrategies.6. Incorporateevidence-basedpractices,whichsupportclinicalreasoning.7. Identifyareasforimprovementinqualityandsafetyofhealthcaresystems.8. Utilizetechnologytoresearchpatientinformation,communicatewith

COURSEOUTCOMES/COMPETENCIES:Attheendofthiscourse,withappropriatestudyandpracticeintheclassroomandclinicalexperience,thefourthsemesterstudentnursewillbeabletoassess,analyzeandcontributetothesafemedicalandpsychosocialneedsofthementalhealthpatientinthepsychiatricsetting.Thestudentwillfocusonthepatientasawholewhilemeetingnursingneedsinthephysiologicalmode,aswellastherapeuticandreferralneedsintheself-concept,rolefunctionandinterdependencemodes.RefertothelistofStudentLearningOutcomes(1-8)listedabovethatwillbeidentifiedandmatchedtoitrelatedcourseoutcomeattheendofeachcompetencyinparentheses.Thenursingstudentwillbeableto:

1. AssessmentalhealthpatientsfromaholisticperspectiveutilizingtheRoyModel(1,3,4,7,8).

2. Compareandcontrastcommontheoreticalframeworksusedinmentalhealth(psychiatric)nursingpractice(1,4,6,8).

3. Relateetiology,pathophysiologyortheory,treatments,nursinginterventions,andself-assessmentforeachtopicdiscussed(1,2,3,4,6,8).

4. Appraisecommondefensemechanismsandtheconceptsofthestressadaptationtheory(1,3,4,6,8).

5. Utilizetheprocessofpsychiatrichistorytakingandmentalstatusexaminationstocompareandcontrastpast,current,andexpectedpatientadaptationlevelsaswellasmeasureprogresstowardmentalwellness(1,3,4,7,8).

6. Establishandmaintainnurse-patientboundariesincludingappropriateuseofself-disclosure(2,3,4).

7. Demonstrateculturallysensitivecareandgrowingculturalcompetencebyexploringpersonalbiases/stereotypesandapplyingknowledgeofappropriateskillsnecessaryforworkingwithculturallydiversepatients/groups(1,2,3,4,5,6,7,8).

8. Demonstratetherapeuticcommunicationtechniques(verballyandinwriting)toassess,implement,andevaluatecareforpatientsacrossthelifespanwithmentaldistress,disabilityordisorder(2,3,4,5,6,7).

CHALLENGE/CREDITBYEXAMINATIONPROCEDURE:StudentsrequestingcreditbyexaminationforcoursesintheRegisteredNursingProgrammustmeettheLosAngelesHarborCollege“CreditbyExamination”policypriortorequestingcreditbyexaminationinplaceofenrollinginandcompletinganynursingcourse.Asstatedinthecatalogue,thepolicyrequirementsinclude,butarenotlimitedto,thefollowing:

CLINICALCOMPONENTUponsuccessfulcompletionofthetheoryportionofthecourse,thestudentwillcompleteeighty-oneclinicalhoursintheacutecarepsychiatricsetting.Todemonstrateclinicalcompetence,thestudentwill:

• CompletetwopracticecareplansheetsandtwocomprehensivewrittenNursingCarePlansfortwodifferentpatientsutilizingappropriatediagnosesasspecifiedintheNursing343course
