What is Stored Energy

Affected Employee

• An employee whose job requires them to operate or use a machine or equipment on which servicing or maintenance is being performed under lockout or tagout.

• An employee whose job requires them to work in an area in which such servicing or maintenance is being performed.

Authorized Employee

• Each employee who locks out or tags out machines or equipment in order to perform servicing or maintenance on that machine or equipment.

• An affected employee becomes an authorized employee when that employee's duties include performing servicing or maintenance covered under this section. All authorized employees need to have a lock or tag to work on all production equipment. Remember Piper Alpha?

The 1988 Piper Alpha platform disaster in the North Sea is the worst oil rig accident to date. • The accident was caused by a

breakdown of several procedures. • The most visible cause of the incident was improper

lockout / tagout procedures of a major pump on the

platform.

• Another issue was end blinds made of wood instead of metal.

• Just 61 survived while 167 men died. The dead included two crewmen from a rescue vessel.

• A mechanical device that physically prevents the transmission or release of energy such as:

Circuit breaker Disconnect switch Double block and bleed valve Blanking and blinding

Group Lockout / Tagout

• During all group lockout /

tagout operations where the release of hazardous energy is possible, each authorized employee performing service or maintenance shall be protected by their personal lockout or tagout device.

CAP Safety Meetings – Lockout / Tagout

Lockout / Tagout is a safety procedure which is used in industry and research settings to ensure dangerous machines are properly shut off and not started up again prior to the completion of maintenance or servicing work. 1. An energy isolating device (the

disconnect switch or valve) is placed in the off position.

2. A keyed lock is then placed over the energy isolating device.

3. This lock remains over the energy source until servicing or maintenance is completed.

Lockout / Tagout is required in general industry employment where servicing and maintenance of machines and equipment could cause injury to employees due to unexpected startup or release of stored energy.

• Each lockout or tagout device shall be removed from each energy isolation device by the authorized employee who applied the lockout or tagout device.

When the authorized employee who applied the lockout or tagout device is not available to remove it, that device may be removed under the direction of the employer, provided that: • Verification by the employer that the authorized employee

who applied the device is not at the facility.

• All reasonable efforts were made to contact the authorized employee to inform them their lock or tag has been removed.

• The authorized employee has this knowledge before they resume work at that facility.

To keep equipment from being energized during repairs or maintenance, it can often be locked out.

The best way to prevent electrical shock is to lock-out and tag-out the machine or equipment.

Employer must demonstrate a tag-out is as effective as lock-out.

Methods of demonstration include:

• The removal of an isolating circuit element such as a fuse.

• Blocking or locking of a control switch or opening of an extra disconnecting device.

When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.

Primary responsibility is vested in an authorized employee for a set number of employees working under the protection of a group lockout or tagout device (such as an operations lock).

Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when they begin work, and shall remove those devices when they stop working on the machine or equipment being serviced or maintained.

When using an extension cord and the plug is not under the exclusive control of the person doing the service, an electrical plug lockout device must be used.

Electrical extension cords must be explosion proof if used in electrical classified areas.
