To access the page to print credit forms click “Credit Form”.


To download and print patient credit forms click “Credit Form”.


Have patients fill out and sign the printed

credit form.


Once the patient has filled out and signed the credit form click on “Compassionate Finance”.


Enter the patient’s total cost.


Enter the patient’s down payment.


Select a term that the patient requested. Call our advisor line if you do not see a term you need.


Patients must choose a day of the month between the 1st and 28th to have this payment automatically withdrawn from their checking account by Compassionate Finance.

Ask the patient what day they prefer and select that day of the following month.


Enter the rest of the patient’s information from their credit form into the appropriate loan application fields.


Proceed by clicking “Run Credit”.


Confirm that the patient has filled out

and signed the printed credit form

by selecting the check box.


To download and print patient credit forms click “Credit Form”

Click “Run Credit”


Within 3-5 seconds after running credit you will receive a Status of “Credit Verified” which means they are approved. -or-


1) If you receive a “Declined” Status, you are required to print a denial of credit letter and give to the patient.

2) Click “Deny Credit” and a denial letter will print for your to give to the patient.


Once the application status is “Credit Verified” Click “Print Agreement”

Click “Download Agreement” to view and print the loan contracts


This tells you what to have signed by the patient and where to fax the signed credit form and loan agreements. Fax: 1-866-907-3508


To download and print patient credit forms click “Credit Form”

This is a cover page you will give the patient that summarizes the patient’s loan and instructions on how to contact Compassionate Finance.


This is the top half of the “Retail Installment Contract”.


This is the bottom half of the “Retail Installment Contract” where the patient will need to sign.


This is the where the patient signs the ACH Agreement Form. You will also need to obtain a voided check or a document from their bank with their checking account information.

This paragraph informs the patient that a one-time account setup fee of $30 will also be deducted from their first monthly payment.


Fax the signed credit form, retail installment contract and ACH agreement form along with a voided check or other bank document to the fax number listed on the cover page. Fax: 1-866-907-3508


Once the patient has signed the loan agreements click “complete”.
