It has information to help you find your own way to meet any needs for support and care to help you, or a loved one, lead the lives that you want.

Good Neighbour Schemes

Community Information Network

Day centres

If you want help with transport, odd jobs, gardening, general errands, or someone to chat to, a Good Neighbour Scheme is for you.

Good Neighbour Schemes are local voluntary groups which offer a service in their community for those in need of help and support. The service aims to help you to stay independent, safe and well.

Day centres offer a range of activities, from computer classes to lunches, exercise groups to film clubs. They serve as community hubs where people can get social care and health advice and information and can provide a valuable break for carers.

Oxfordshire County Council runs seven Health and Wellbeing Centres that support adults of all ages with physical disabilities, older frail adults, and older adults with dementia, mental health issues and learning disabilities:

The Council is encouraging Good Neighbour Schemes in Oxfordshire to form and develop. There are now over 50 schemes up and running in Oxfordshire and new schemes are emerging all the time.

Stay involved, informed, in touchThe Community Information Network exists to provide you with information on local support services and activities, money matters and social care.

Whether you want to meet people, get help at home, find out about benefits and support, discover a computer or exercise class, or volunteer in your local community, the Community Information Network is here to help you. The service is free and operates across the county through information drop-ins, over the phone or visiting you at home.

Your local Community Information Network team can help you find activities and support near to where you live. You can meet them at community information events and drop-ins or you can give

them a call. They can also visit you at home if you have difficulty getting out and about, want a family member to join you, or would rather talk privately.

Oxfordshire Travel Advice LineThe Oxfordshire Travel Advice Line (Oxtail) offers free impartial advice on a range of transport options for older people or those with a high level of support needs.

Bus passesYou must be eligible to apply for a bus pass. To find out more, call Oxfordshire County Council’s Social and Health Care team on 0845 050 7666.

Blue BadgesThe national Blue Badge Scheme helps people with severe walking difficulties that travel as drivers or passengers to park close to their destinations.

To apply or request an application form for a Blue Badge, please visit www.oxfordshire.gov.uk/cms/content/apply-blue-badge or call Oxfordshire County Council’s Social and Health Care team on 0845 050 7666.

ParkingOxfordshire County Council may provide a disabled person’s parking place on a public road where there is a need.

Residents can apply for one of these parking places to be provided close to their homes or work.

Community First Oxfordshire’s website, www.communityfirstoxon.org where you will find more information about the work of this charity that brings together people who care about local village life.

Oxfordshire Accessible Transport and Access GroupThis group looks into and advises on difficulties faced specifically by people using public transport and highways and works with bus and rail companies to make sure transport in Oxfordshire is accessible and affordable. It raises issues affecting disabled people with County and District councillors, as well as working on national disability issues.

If you are a disabled passenger and have a complaint, the group will look into it for you and try to get the issue resolved.

RailcardsA Railcard will give you discounts on rail fares, online or from a staffed rail station. There are several schemes for passengers with different needs, including carers.

ShopmobilityShopmobility is a free service offering loan of wheelchairs and electric scooters to people who need them for shopping and sightseeing.

Oxfordshire Family Information Service

Oxfordshire County Council’s Family Information Service provides children, young people and families in Oxfordshire with free information and advice.

Your District Councils (contact details on page 31) have a list of all public toilets in your area and can tell you how accessible they are. They’ll also let you know about local shops and businesses which have accessible toilets.

The Changing Places website has a map of toilets that are fully accessible and have the equipment that disabled people may need. This can be found at www.changing-places.org

Dementia Oxfordshire is a new service to help you with advice on all aspects of dementia and the support that is available locally to help people to live well with dementia. There are a variety of activities, support groups and services to support people with

The Oxfordshire Reablement Service

If you have been ill or had an accident or injury which has left you feeling less able and less confident than before, the Oxfordshire Reablement Service can help you get back to living your life as independently as possible. It is designed to be a short-term service of only one or two weeks and can be extended up to a maximum of six weeks.

Delivered by Oxford Health NHS Foundation Trust, this service:• helps people to maintain or regain their independence

by learning or re-learning the skills necessary for daily living;

• assists in the accurate assessment of people’s needs for ongoing social care support;

• is free at the point of delivery up to a maximum of six weeks for Oxfordshire residents over the age of 18, who need and can benefit from the service and meet the eligibility criteria;

Support and care – where do I start?

Assessments

The Care Act

If you have difficulty with day-to-day activities, such as getting washed and dressed or getting yourself something to eat, you may be eligible for support from Oxfordshire County Council.

Being ‘eligible’ means that, because of illness or disability, you need support and care to manage some of the basics of everyday life, such as eating, using the toilet, maintaining relationships or keeping yourself safe at home. There are national criteria for the Council to use to work out if you are eligible.

To find out if you are eligible, you can have an assessment. This is an opportunity to understand your situation, the needs you may have and your strengths and capabilities. It will involve talking to you, your partner, carer, relatives or friends, as appropriate, to consider the factors that affect your health, wellbeing and independence.

The assessment will include the outcomes that you want to achieve in your day-to-day life as well as consideration of any needs you may have for care and support. The assessment will also consider how the provision of care and support could help you achieve those outcomes and promote your wellbeing.

The Care Act outlines how care and support should be provided in England. It came into effect on 1st April 2015 and replaced all pre-existing adult social care legislation.

The law focuses on promoting wellbeing and health with emphasis on personal dignity, physical and mental health, protection from abuse and neglect, and control over day-to-day life. This has been Oxfordshire’s approach for a number of years.

This Guide will give you a good starting point to help you think about options and provide you with information so that you can find solutions yourself to help you to remain independent and well.

You have the right to have an assessment regardless of your ability to pay for the support you may need. The assessment may be over the telephone or face-to-face, and in the future you will also be able to choose to complete the assessment online.

If the assessment finds that you do have needs for care and support which are eligible to be met by the Council, you will be helped to make a support plan. This will include the Council telling you how much meeting your needs is likely to cost – known as a Personal Budget.

Having a Personal Budget helps you to have control over your life, identify your needs and the things you want to do and, if you wish, to manage your own support. You can choose to have your Personal Budget paid to you as a Direct Payment, which means you can plan exactly how to use it to meet your needs.

The Care Act introduced a nationally agreed level of care and support needs that all councils consider when assessing the support they can give to individuals. It also introduced the right to an assessment for carers (see page 17) and independent support from an advocate if you have difficulty in participating in assessments or planning for your needs.

Caring can be very stressful but you are not alone. At any one time in Oxfordshire over 60,000 people are providing unpaid care for a family member, friend or neighbour. It is very important that you take time to look after yourself while you are caring for someone else.

You may not see yourself as a ‘carer’ – you see yourself as a son, daughter, partner, friend or neighbour. But if you regularly support and care for someone who could not manage without you then you are a carer and there is help available to support you to carry on with this vital work.

If you provide unpaid support to someone who could not manage without your help, the law says you have the right to have your own needs assessed, even if the

This assessment, called a ‘Carer’s Assessment’, gives you the chance to say what you need as a carer, and to find out what support might be available to help you.

The assessment can also give you information about services available, and other ideas for supporting you in your caring role. Your Carer’s Assessment will show if you qualify for support services from the Council. Many carers will be entitled to receive a support plan and Personal Budget in their own right, for services that enable them to continue in their caring role while still being able to pursue their own interests or lifestyle. Carers will not be charged a contribution to services arranged through the Council.

Respite at home

Home-based respite services give carers a break from their caring responsibilities. A care worker comes in to look after the person you care for, so you can have some time to yourself.

Respite at home is considered to be a service for the

person you care for. If they have a Personal Budget for their own needs (see page 16), they could use that money to pay for it. A financial assessment of the person being cared for will confirm whether they need to pay towards the cost of this service.

Benefits you may be entitled to as a carer

Carer’s Allowance If you care for someone for at least 35 hours a week and you are over 16, you may be entitled to Carer’s Allowance. However, you cannot claim it if you are in full-time education or employment and it can affect other benefits or pension you receive.

Carer’s PremiumIf you are over 65 you may qualify for Carer’s Premium. This can entitle you to an increased amount of income-related benefits (such as Pension Credit, Council Tax Benefit and Housing Benefit).

Carer’s CreditCarer’s Credit is a National Insurance credit which will let carers build up qualifying years for the basic State

Help in an emergencyEmergency Carers’ Support Service is a support service for adult carers of adults in Oxfordshire. Experienced staff look after the cared-for person when the carer has an emergency and is unable to do so. Care can be provided from one to 48 hours and, in exceptional circumstances, up to 72 hours.

Once you are registered, you can access emergency help quickly when you need it. You will receive a registration card which you can keep in your purse or wallet.

Equipment, aids and gadgets continued

Assistive Technology ranges from very simple gadgets, such as medication aids (boxes with compartments for particular days of the week and times of day), to Telecare (devices that can automatically and remotely monitor emergencies and call for help).

TelecareTelecare is a term used to describe devices (for example, movement, flood, gas, and smoke or fall detectors) that automatically send a signal when a problem is detected and calls for assistance. Personal pendants are also available as part of Telecare; when pressed, a fully trained operator assesses the nature of the emergency and reacts accordingly.

Simple gadgetsA range of gadgets exists to remind or enable you to do something, or alert a family member or carer living with you that you need help. These gadgets are sometimes referred to as standalone Assistive Technology. Here are some examples of gadgets that Oxfordshire County Council can help to provide:

Memory promptsThese are used to assist people with memory difficulties by providing daily memory prompts using personally recorded voice messages at specific times of the day to remind you to do specific activities, for example, ‘take medicine’ or ‘eat lunch’.

Movement activated memory prompts A movement activated message player allows a message to be recorded and played back when a person walks past the sensor, for example, ‘take your

keys with you and lock the door’ or ‘make sure you have turned the cooker off’.

Location devicesLocation devices are designed for people who may be at risk of getting lost when they go out on their own and can help them to return home safely. They can be used to locate a person when needed, allow the wearer to call for help and alert friends and family if a person goes outside of areas they know and feel safe in, has a fall or uses transport. People can also have two way conversations using these devices.

Will I have to pay for my Assistive Technology?If you arrange this through Oxfordshire County Council they will undertake a financial assessment to work out what, if any, contributions to the cost of Assistive Technology you need to make. Gadgets (standalone Assistive Technology) are provided to you based on your need for the equipment and do not require a financial assessment.

Occupational therapy

Oxfordshire County Council’s Social and Health Care team takes referrals for occupational therapy, which can help identify whether you would benefit from any pieces of equipment, from simple solutions such as a walking aid or a second stair rail, to major adaptations such as walk-in showers.

If you have difficulty with day-to-day activities such as getting washed and dressed or getting yourself

Disabled Facilities Grants A Disabled Facilities Grant can help eligible disabled people to adapt their homes to make their day-to-day life easier. This is a means-tested grant – whether you can receive it depends on your other sources of income. To apply for a Disabled Facilities Grant you must first be assessed by an occupational therapist (see page 19).

Essential Repair GrantsThis is a means-tested grant for home owners. An Essential Repairs Grant of up to £5,000 can be made available to eligible people to help with essential repairs to the home.

Flexible Home LoansThis loan scheme for owners and occupiers aged 60 and over is designed to help improve the comfort, safety and security of, and help with repairs to, your home.

Small Repairs Service Available to owners, occupiers and tenants who

are aged 60 and over, disabled and suffering from a long-term health condition. This service covers small plumbing, electrical and basic household repairs. There will be a labour charge as well as the cost of any materials used.

Care homes A registered care home provides accommodation, meals and help with personal care tasks such as washing, dressing, bathing, getting in and out of bed, getting around and eating.

Care homes with nursingCare homes with nursing provide care from a qualified nurse employed by the home. If a care home providing nursing is the best solution, a specially trained nurse will assess your nursing needs, to determine the level of nursing care the NHS will fund. Paying for your own residential care

If you are funding your own care because you are not eligible for local authority funding, there are other forms of financial assistance you may be entitled to. The important thing is not to try to do it alone, always seek professional advice.

My Care My HomeMy Care My Home offers a free telephone and face-to-face advice service through home visits aimed at helping people find a suitable home care provider, or care in sheltered or Extra Care schemes and care homes. It is also able to visit people who are currently in hospital and require a package of care before they can be discharged.

My Care My Home offers referrals to Independent Financial Advisers; however, if you wish to arrange your own financial adviser, My Care My Home can give you the necessary details to enable you to do this.

The home you choose must be suitable for your assessed needs, comply with any terms and conditions set by your local authority and not cost any more than they would usually pay for someone with your needs. My partner needs care, how does this affect me financially? The Council will undertake a joint financial assessment for you and your partner, and will also carry out individual assessments for whoever needs care. Oxfordshire Association of Care Providers (OACP) is a community interest company formed in 2014 to represent and support the diverse range of care providers operating across the county. Its members include registered care providers, community-based services and Personal Budget holders and cover a complete range of private, independent and voluntary sector providers. Most providers work with adults, but some have services for young people and children. This diversity is reflected in our Board membership.

It represents all types of social care, in all settings, for all conditions. This includes care homes (and care homes with nursing), home care agencies, supported living providers, community mental health services, housing-related support, day-time support and respite services. Its 90+ members also include services which provide advocacy, information, advice and support.

It is this wide-ranging and diverse membership, which provides the strength of OACP. It is able to look across the whole sector and see trends and opportunities for providers to work together for the benefit of the sector. It aims to support care providers to deliver high-quality, person centred care and recognises that quality with annual awards for best practice.

With its overview, OACP recognises common themes: the difficulty of recruitment, the pressure on delivering quality services in a challenging financial context, and with tight regulatory standards. Its forums offer opportunities to bring together providers to focus on specific themes, or to identify joint working that would improve aspects of delivery.
