Traditional Home Care Not Meeting Your Needs?

CDPAP is a self-directed task-based home care model for Medicaid eligible consumers who:

Need home care services

…and…

Can manage their own care* OR…

- have a responsible adult willing to assume the program’s responsibilities on their behalf.

The Basis of the Program:

Many disabled individuals have the ability to choose their own caregivers and direct their own home care services.

CDPAP’s Solution:Through Medicaid funding, CDPAP works with the

Local Department of Social Services (LDSS) to make it possible for approved participants to designate their own personal assistant.

Assistants can be anyone except a parent, or a participant’s designated representative

Lets Recap: How does CDPAP Work?

• Consumers can hire, train, supervise, or dismiss their personal assistant.Control

• Consumers schedule when to get personal assistance, directing how and when their care plan is managed.

Independence

• Consumers are responsible for training personal assistants and overseeing the care given.

Responsibility

A study by the U.S. Department of Health and Human Services says that people who reach age 65 will likely have a 40 percent chance of entering a nursing home at some point. Many of these cases might be avoided with CDPAP.
