Introduction and Forward

A core function of the San Antonio Metropolitan Health District is to monitor the health status of the community in order to identify the conditions which adversely affect the health and wellbeing of residents and target areas or groups at highest risk. This enables the District and our public health partners to be more efficient and effective with available resources.

With this in mind, this publication of the “Bexar County 2009 Mortality Report” gives considerable insight about how San Antonio is impacted by health conditions which result in premature death and give context to how these conditions compare to the US population as a whole. Data gathered for this publication are sourced primarily from our own City of San Antonio death certificates with supporting documentation from the US Census Bureau, Texas Department of State Health Services, and the Centers for Disease Control and Prevention (CDC). Because each death recorded in Bexar County must be certified by the State Vital Statistics Registrar after extensive review, data used in this analysis come from 2009, the last full year for which this information is available. To facilitate review of this material, whenever possible we have used geographic maps, time series graphs and charts in favor of data tables, which have been included in the appendix of this document. The collection and analysis of these data are used in the creation and implementation Public Health intervention strategies designed to ensure that residents have a reasonable expectation of leading lives of maximum potential and disease free years.

I am hopeful that you will find some interesting information as you peruse this document. Bexar County has experienced an increase in the total number of deaths since 2000, much of which can be attributed to the increase in population over the last decade. While these statistics indicate significant declines in heart disease, stroke, and diabetes mortality overall, the total number of premature deaths (residents under age 65) has increased faster than the County’s population growth as a whole. Moreover, deaths related to liver disease, kidney disease, septicemia, influenza and pneumonia were well above historic averages.

We remain committed to working with many partners to reduce preventable and premature death in our community. We have recently expanded our programs targeting the epidemic of obesity and overweight in our community which impacts the health of many residents, including our children. Improving the health status of our community requires a coordinated effort between schools, health care organizations, the business community, foundations, and volunteer organizations. I hope this report can help guide us in setting priorities and shaping collective efforts to improve the health of our residents.

DEMOGRAPHICS

• The 2009 US Census Bureau population estimate for Bexar County of 1,651,448 represents an 18.6% increase over the year 2000 census1.

• Bexar County was listed as the fourth fastest growing County in Texas, adding more than a quarter million residents since 2000. Births account for 53 % of this growth. 2

• The median household income increased 16.1% since the 2000 census to $45,688.1

• Bexar County residents living in poverty increased 7.5% since the 2000 census to 17.1% living below in 2009.2

• Bexar County median age in 2009 was 32.4, up from 32.1 in 2000, yet still below the state average of 33.1.2

LEADING CAUSES OF DEATH

• On an average day in Bexar County 30 deaths will occur, of which a third will be residents under age 65.

• Heart Disease and Cancer were the two leading causes of death in 2009, accounting for 42% of all deaths

• The age adjusted death rate of 702.8 / 100,000 residents is lower than the 2008 national average of 758.6 (all causes).

• The Life Expectancy at birth for Bexar County residents is 79.5 and remains above the U.S. average of 78.3 years.

• The 2009 age adjusted death rates for Bexar County men were 33% higher than women.

• Overall deaths attributed to Heart Disease, Stroke, and Diabetes have decreased since 2001 despite an overall increase in the Bexar County Population.

LEADING CAUSES OF PREMATURE DEATH (<AGE 65)

• In 2009 nearly a third of all Bexar County deaths were residents under 65, totaling 3,381 deaths.

• The leading causes of premature deaths in Bexar County were Cancer, Heart Disease, and Liver Disease.

• Age adjusted death rates for Liver Disease, Kidney Disease, and Septicemia were more than 50% higher than US rates in 2009.

• The leading causes of premature death for residents 15-44 by ethnicity were Motor Vehicle Accidents for Hispanic residents (57 deaths), Suicide for White residents (37 deaths), and Homicide for Black residents (23 deaths).

LEADING CAUSES OF DEATH BY COUNCIL DISTRICT

• Heart Disease was the leading cause of death in each City Council District except City Council Districts 6, 9, and 10 where Cancer was the leading cause of death.

• Heart Disease death rates were below the national average of 195.0 deaths/100,000 residents in all City Council Districts except Council District 5 (216.0/100,000).

• Cancer mortality rates were below the national rate of 185.2 in all Council Districts. The overall Cancer mortality rate in San Antonio in 2009 was 129.7 per 100,000.

• Motor Vehicle Fatality rates were below the national death rate of 11.8 deaths / 100,000 residents in all Council Districts except Council District 4 (13.1 /100,000).

Definitions and Methods Age-adjusted rate: A rate that has taken into account influences on a crude

rate, such as differences in age composition of the population. Age adjustment,

using the direct method, is the application of age-specific rates in a population

of interest to a standardized age distribution in order to eliminate differences

in observed rates that result from age differences in the population

composition. Age-adjusted rates are useful for comparison purposes only, not

to measure absolute magnitude. (To compare absolute magnitude, numbers or

crude rates are used.)

Age-specific rate: Rate obtained for specific age groups (for example, age-

specific fertility rate, death rate, marriage rate, illiteracy rate, school

enrollment rate, etc).

Cause of death: Any condition which leads to or contributes to death and is

classifiable according to the tenth revision of The International Classification of

Diseases (ICD-10).

Contributing cause of death: Any conditions significantly contributing to

death, and listed on the death record as a secondary cause of death or

condition of significance contributing to a mortality event.

Crude death rate: The rate of any demographic or vital event that is based on

an entire population.

Ethnicity: The classification of a population that shares common

characteristics, such as, religion, traditions, culture, language, and tribal or

national origin.

Federal poverty guidelines: The poverty guidelines updated periodically

in the Federal Register by the U.S. Department of Health and Human

Services under the authority of 42 U.S.C. 9902(2).

ICD-10: The International Classification of Diseases, 10th edition. A

system for classifying diseases and injuries developed by the World Health

Organization and used worldwide to improve comparability of cause of

death statistics reported from different countries. The tenth revision has

been in use since January 1, 1999.

Life expectancy: The average number of years that a person can

anticipate living after a given age, usually birth. Most often based upon

the current mortality experience of a population.

Mean: The arithmetic average of a set of values. It is calculated as the

sum of the values divided by the number of values.

Median: The value in an ordered set of values above and below which

there are an equal number of values; the 50th percentile.

Premature death: Deaths which occur prior to age 65, the age in which

most are actively employed and not eligible for Medicare benefits.

Race: A geographical population of humankind that possesses inherited

distinctive physical characteristics that distinguish it from other

populations.

Underlying cause of death: The disease or injury which initiated the

train of morbid events leading directly to death, or the circumstances of

the accident or violence which produced the fatal injury.

Bexar County Population Demographics The City of San Antonio is geographically contained within the political boundaries of Bexar County. In 2009 there were approximately 1,651,448 Bexar County residents, of which 1,373,668 resided in the City of San Antonio. Figure 1. Bexar County Age Distribution by Ethnicity illustrates that Bexar County is predominately Hispanic (58%) with a significantly lower median age (28 years) than other residents.

San Antonio, now the 7th largest City in the United States, is located in one of the fastest growing counties in the country. The US Census Bureau estimates that Bexar County population grew by more 250,000 residents between 2000 and 2009, of which 53% were attributed to resident births, making births a critical component of Bexar County’s population growth. Bexar County had a significantly higher annual birth rate (74 / 1,000 women 15-44) than the US general population (66.7 / 1,000 women 15-44). The Bexar County Birth-Death ratio or replacement rate was 2.6 births for each death occurring in 2009. This compares to a national ratio of 1.6 births for each US resident death in 2009.

These population trends are expected to continue into the foreseeable future. As San Antonio continues to grow, it is anticipated that the proportion of Hispanic residents will account for more than 63% of the County’s population by 2040.

• The two leading causes of death, Heart Disease and Cancer

accounted for more than 42% of all Bexar County deaths in 2009. However, age adjusted death rates for both causes of death remain below the national average.

• Chronic Lower Respiratory Disease (Emphysema / COPD) moved ahead of Stroke as the third leading cause of death in Bexar County with 690 total deaths in 2009.

65) in 2009, resulting in an age adjusted death rate of 364.4 deaths per 100,000 residents.

• Premature deaths due to Liver Disease, Birth Defects, and Septicemia were significantly higher than national rates, while Cancer, Heart Disease, and Accident deaths were significantly lower than national rates.
