Asthma
� Heterogene Erkrankung, charakterisiert durch eine chronische Inflammation der Atemwege mit dauerhaft bestehender Hyperreagibilität.
� Die typischen Beschwerden sind wechselnde, häufig anfallsweise Dyspnoe, Giemen, thorakale Enge und Husten verbunden mit einer Limitierung des Expirationssflusses.

DefinitionsDefinitions
Asthma
Asthma is a heterogeneous disease, usually characterized by chronic airway inflammation. It is defined by the history of respiratory symptoms such as wheeze, shortness of breath, chest tightness and cough that vary over time and in intensity, together with variable expiratory airflow limitation. [GINA 2014]
COPD
COPD is a common preventable and treatable disease, characterized by persistent airflow limitation that is usually progressive and associated with enhanced chronic inflammatory responses in the airways and the lungs to noxious particles or gases. Exacerbations and comorbidities contribute to the overall severity in individual patients. [GOLD 2015]
Asthma-COPD overlap syndrome (ACOS) [a description]
Asthma-COPD overlap syndrome (ACOS) is characterized by persistent airflow limitation with several features usually associated with asthma and several features usually associated with COPD. ACOS is therefore identified by the features that it shares with both asthma and COPD.

Asthma COPD OverlapSyndrom (ACOS) (Beschreibung)
� Asthma mit persistierender aber teilweise reversibler Obstruktion des Atemflusses mit oder ohne Emphysem beziehungsweise bei einer COPD, die von einer zumindest teilweise reversiblen Atemflussobstruktion (mit oder ohne Allergie) begleitet wird.
ACOS
• Alter bei Erstdiagnose meist über 40 Jahre, aber Symptome seit der Kindheit oder im frühen Erwachsenenalter
• Symptome mit wechselnder Ausprägung, darunter auch belastungsabhängige Atemnot
• die Einschränkung des Atemflusses ist nicht voll reversibel, aber variabel
• in der Anamnese kommen oft eine Asthmadiagnose, Allergien oder eine familiäre Belastung vor, aber auch die Exposition von Noxen, insbesondere dem Rauchen
• Symptombesserung unter Therapie, jedoch ein Fortschreiten der Atemwegsobstruktion und ein hoher Medikamentenbedarf
• gehäuft vorkommende Exazerbationen • Eosinophile und /oder Neutrophile im Auswurf Sputum

Exazerbationen zeigen die Progression der Erkrankung
� Exazerbationen beschleunigen den Krankheitsverlauf, verschlechtern den Gesundheitsstatus und erhöhen das Mortalitätsrisiko.1
� Jede Exazerbation verursacht einen zusätzlichen FEV1-Verlust.2
� Eine Verzögerung bis zum ersten Auftreten einer Exazerabation könnte helfen die Symptome zu verbessern und die Atemnot zu reduzieren.3,4
