How to Answer the Questionnaire

The questionnaire contains questions about you, youth panel member. There are questions that you

must answer with accurate information while other questions need you to express your opinions.

Please read all the questions carefully and answer them accurately and honestly. If there are

questions that you cannot understand or you want to clarify, please ask the survey interviewer.

♣ How to Mark

After reading the question, you will be asked to choose a number from 1 to 5. After reading a

statement on the left, please circle (○) the appropriate number as in ‘1’ if you ‘strongly disagree’,

‘2’ if you ‘disagree’, ‘3’ if ‘neutral’, ‘4’ if you ‘agree’, and ‘5’ if you ’strongly agree’.
