It was developed to promote dissemination of the program to other communities and local chapters of the American Lung Association.

RESEARCH

As a full-time clinician I was first drawn into research in the early 1970's while still a medical officer in the Indian Health Service. Before residency, and in spite of the intense clinical demands of reservation medicine, I was intrigued by the case of a tiny infant with gonococcal arthritis, leading to my report in Pediatrics (1974) of both an extensive literature review and a discussion of several cases which I treated.

Since 1978 my research, like my clinical work, has focused in behavioral pediatrics, and particularly in the area of self-regulation abilities of children. By self-regulation I mean the ability of children to utilize alternative states of consciousness (through the development of imagination and relaxation skills) to both feel better and to affect perceptions and outcomes of their problems and illnesses. As I developed and refined clinical expertise in utilizing and teaching children these self-regulation skills (relaxation/mental imagery, self-hypnosis) I became increasingly aware of the need to document, measure, and further study the clinical successes children and families were experiencing in applying these strategies to their various health problems. The first result of such inquiry was the publication in 1984 (Journal of Developmental and Behavioral Pediatrics) of our experience with 505 children who had been taught Relaxation/Mental Imagery (Self-Hypnosis) skills. To date this remains the single largest such report of children learning and utilizing these techniques, and was translated into German and published in 1990.

I have published 46 articles, multiple book chapters, and co-authored the major text in the field related to children's uses of relaxation/mental imagery (hypnotherapy) for a wide range of medical conditions. For the past 20 years I have served as the coordinator and teacher of Advanced courses in Pediatric Hypnosis and hypnotherapy to over 4500 health care professionals at National and International meetings of the American Society of Clinical Hypnosis, the Society for Clinical and Experimental Hypnosis, the International Society of Hypnosis, as well as for the Society for Developmental and Behavioral Pediatrics, and for our local and regional Minnesota Society of Clinical Hypnosis and Minnsota Society for Developmental and Behavioral Pediatrics. I was recently (October, 1997) the only American keynote speaker at a Child Hypnosis Congress in Heidelberg, Germany. The largest such conference ever presented, the meetings and workshops attracted over 1000 participants. I have also been an invited guest speaker and teacher at other meetings within Germany, and in Australia, Israel, The Netherlands, Canada, and throughout the United States.

As I have continued to observe children's clinical abilities to utilize self-regulation strategies (for pain control, wheezing, habit reversal, tic

41modulation, anxiety amelioration, etc.) I have further focussed my research inquiry upon physiologic as well as behavioral correlates of the hypnotic experience, and the arena of psychoneuroimmunology. Since 1980 I have received research grant awards for these efforts from the Robert Wood Johnson Foundation, the F.A. Bean Foundation of Minneapolis Children's Medical Center, Hennepin Faculty Associates, and the Fetzer Foundation.

Most recently my research has begun to focus on children at different ends of the Behavioral Pediatrics spectrum, i.e. healthy children and the ways in which self-regulation techniques may be useful to them for the prevention of illness, and chronically ill children and how self-management strategies are useful to them and their families in both the relief of suffering and in the promotion of as high a quality of life as possible in the face of their conditions. Regarding the former, I have just completed the first phase of a controlled study examining the ability of normal, healthy children to self-regulate cardiovascular function (heart rate, blood pressure) with relaxation/mental imagery (self-hypnosis). Even as we are evaluating these data, I am also submitting for funding a prospective controlled examination of the ability of youngsters with Tourette Syndrome to apply self-regulation strategies to more effectively modulate their tic behaviors.

For the future I anticipate a continued focus upon further exploration of the physiologic, psychologic, and psychoneuroimmunologic correlates of the hypnotic experiences in children, with a particular focus on children with chronic and/or recurrent conditions.

I. RESIDENT EDUCATION

A. Behavioral Pediatrics Rotation

Beginning in July,1988 all pediatric residents have taken a required rotation in Behavioral Pediatrics during their PL-l year. After its first year trial as a 2-week rotation, this became a 3-week experience in 1989, and in July, 1991 has evolved into a full 4-week rotation. I have had the responsibility for development of this educational experience, and continue to serve as course coordinator as well as one of several key clinical mentors, personally teaching at least 2 Q l-hour tutorials/conferences with residents per week. (July, 1988 – March, 2013)

(*The content and philosophy of both the Behavioral Pediatrics Fellowship and the Residents' Behavioral Pediatrics Rotation have evolved consistent with the educational curriculum guidelines of and published by the Society for Behavioral Pediatrics in December,1988. B. Pediatric Elective in Behavioral Pediatrics

Since 1980 a 4-week elective in Behavioral Pediatrics has been available to Pediatric Residents. I have been coordinator of the elective program since 1987. Each such elective is individually developed in collaboration with the resident in order to both identify learning goals and as part of the learning experience to teach the skill of thoughtful and careful development of learning objectives. This elective has attracted one or two PL-2 or PL-3 residents from our residency program each year, as well as residents from out of state programs.

C. Medical Student and Housestaff Conferences (1981-present)

II. FELLOWSHIP

A. Fellowship in Behavioral and Developmental Pediatrics

This Fellowship began in 1978. One of our earliest Fellows, Dr. Judson Reaney, was a key member of the full-time teaching Faculty in the Behavioral Pediatrics Education Program. One of three major faculty teachers in the Behavioral Pediatrics Fellowship, I have been the Director of the Fellowship since 1987. Initially the Fellowship was offered as a one or two year program, but is now a three year academic Fellowship. Our first 3-year academic Fellow began in July, 1990. There have been 12 physicians who have completed Behavioral Pediatrics Fellowships. Their current activities are noted on the following pages. (1987-2013)

When Developmental Behavioral Pediatrics was first approved as a sub-specialty in Pediatrics by the American Board of Medical Specialties in 2002, and Fellowships were first able to be accredited, our Fellowship was amongst the first to be fully accredited by the Accreditation Council on Graduate Medical Education (ACGME), in 2003; and was accredited without stipulation or deficiency. I was responsible for direct preparation (writing) and coordination of the ACGME accreditation application material.

B. “Mini-Fellowship” in Behavioral Pediatrics

Since 1987 a four to six week mini-fellowship in Behavioral Pediatrics has been offered to physicians in practice who have desired a specific and concentrated continuing education program in Behavioral and Developmental Pediatrics. To date several pediatricians have taken this opportunity and with our assistance developed programs individually tailored to meet their continuing education needs in behavioral pediatrics. I developed and coordinate this mini-fellowship in cooperation with my associates Drs. Michael Reiff and Jud Reaney. Participants have included a Group Health pediatrician (Dr. Mark Nupen) who, moving here from out state, wished to refine and define specific behavioral pediatrics principles and concepts for utilization in general pediatric practice, and an Albert Lea pediatrician (Dr. Lowell

47Barr) who worked closely with Dr. Reaney in a similarly individually tailored continuing education program.

C. “Mini-Fellowship”/Tutorial in Pediatric Hypnotherapy

Since 1980 many different child health clinicians have sought supervision, tutorial, and continuing education experiences in pediatric hypnosis through our Behavioral Pediatrics program. Like the mini-Fellowship in general Behavioral Pediatrics, this experience is individualized to meet the specific level of training and experience and educational and practical needs of the clinician. While some have been organized as a four-week experience, more often these have been arranged as a one to two days/week experience over the course of 4-6 months.

Such arrangements have averaged one or two per year over the last five years. They have included clinicians practicing in Behavioral Pediatrics, a Family physician from St. Paul (T. Quinn, MD..), a master's level clinical psychologist (R. Gephart, M.A.) from Grand Rapids, and several local licensed consulting psychologists.

A special one week tutorial in pediatric hypnotherapy was provided for Dr. Pedro Gelber, a visiting pediatrician for Uruguay. I developed and coordinated this learning experience for Dr. Gelber.

A special two and a half week (daily) tutorial in pediatric hypnotherapy was developed at the request of German psychologist Dr. Sieqfried Mrochen who designed his sabbatical to include study with us in October. 1988. I developed, coordinated, and taught most of that special tutorial.

A comparable, but longer 4 week daily tutorial was prepared for and implemented with Dr. Sepp Holtz, developmental pediatrician from Zurich, Switzerland. I met Dr. Holtz when he attended a 2-day Pediatric Hypnosis Workshop which I taught in Rottweil, Germany, near his home in neighboring Zurich, in May, 2000 and at the Zurich Children’s Hospital several days later. Shortly thereafter he spent a month of his Sabbatical learning with me (and also teaching colleagues) in Minneapolis at the University of Minnesota.

III. CONTINUING EDUCATION IN BEHAVIORAL PEDIATRICS

A. National Behavioral Pediatrics Workshops

In 1983, 1985, and 1987 Behavioral Pediatrics sponsored National Seminars distinguished by their unique participatory learning methodology, a national and local faculty, and the use of videotapes

48of actual clinical encounters and of expert panel discussions as integral to the continuing education process. For the 1983 and 1985 meetings I prepared and edited several videotapes and participated as a faculty member for several panel discussions. For the 1987 meeting I served as course coordinator in addition to developing several of the videotapes and participating on the faculty as a panelist.

B. Behavioral Pediatrics Workshops

1. Department of Pediatrics - Topics & Advances in PediatricsMany years since 1986 Behavioral Pediatrics has presented a Workshop as part of the Department of Pediatrics’ Annual CME course. I am coordinator for and co-teach these Workshops.

2. Department of Family Practice & Community Health Many years since 1986 we have presented a Behavioral Pediatrics Workshop as part of the Family Practice Department's Annual CME Review Course. I have been the coordinator for and co-teach these Workshops.

C. Clinical Hypnosis Workshops

For the past 32 years I have been a member of the teaching faculty of the University of Minnesota's Annual Workshops in Clinical Hypnosis, a now 34 year old annual course.

For over 24 years I have taught the 1.5 hour Pediatric Clinical Hypnosis section of the Introductory Workshop as well as supervising small group practice sessions as a mentor. and teaching the Introductory Course section on "Language of Hypnosis".

Since 1986 I have been the Course Director for this annual Introductory and Advanced Workshop program, also now co-sponsored by the Department of Pediatrics since 1986. During this time the attendance for both the Introductory and Advanced sections have increased annually, in 1991 attracting well over 300 participants for the first time in its then 20 year history.

Annually since 1987 I have taught from one to three workshops in general and/or pediatric clinical hypnosis (half-day to two days in length) in and around the Twin Cities.

Beginning in the mid-1980's I have been increasingly invited to teach portions of as well as entire Workshops in Pediatric Clinical Hypnosis throughout the United States and around the world. Details regarding these training workshops can be found under Presentations in the pages that follow.
