MEDICAL INFORMATION:

Please note you will be asked to provide the following information prior to the first day of camp:• Doctor's name and contact number• Manitoba Personal Health ID number (9 digit)• list of allergies or medical considerations to be made• name and instructions of any medications that would be taken or administered during camp

MEDICAL AGREEMENTS AND CONSENT:

Permissions / Privacy:

1. In case of accident or illness, if a parent or guardian cannot be reached, Kids Culinary Camp will take your child to theemergency ward of the nearest hospital. Signing the consent form gives your authorization for emergency health services.

2. The Medical and Allergy Information Sheet and Consent Form must be provided by the first morning of camp.3. Information collected on this application will be used by the College for registration and communication purposes only and is

collected under the authority of The Freedom of Information and Protection of Privacy Act.

1. Discipline is handled using a 3-strike system. If a camper misbehaves or acts in an unsafe or inappropriate manner (first strike),he or she will receive a warning. If the behaviour continues (second strike), a phone call is made to the parent(s)/guardian(s).If the behaviour continues after the call (third strike), the camper is sent home with no money refunded. Kids Culinary Camphas a violence-free policy to ensure the safety of all our participants. Any physical misconduct will result in the immediateremoval from the program with no refund. Kids Culinary Camp reserves the right to refuse access to camps or programs to anyparticipant for inappropriate behaviour.

2. By signing this consent, I agree to allow Kids Culinary Camp or Red River College to reproduce the likeness of my child (photo)for use in promotional materials or publications.

3. Refunds must be requested by June 1. Requests after June 1 will be processed as a withdrawal and no refund will be granted.

Consent: I, the undersigned, the Parent/Guardian of the aforementioned camper, consent to his/her participation in the Kids Culinary Camp summer program and related activities that involve demonstrations, cooking, baking , and recreational activities.

REGISTRATION & PAYMENT: Registration forms are accepted by mail P104-160 Princess Street, Winnipeg MB R3B 1K9 Credit Cards and Cheques are accepted, with cheques made out to Red River College. Completed Forms can be emailed directly to Aaron Stringer if you omit the credit card number. You will be contacted by phone for your credit card information.
