Managing the Challenges
The challenges facing today’s clinical operationsdirector are many, including accurately measur-ing site performance, managing partner relation-ships,and overcoming the communications barri-ers between clinical and data operations. Ourexperts discuss these and other hurdles that areall part of the evolving clinical process.
GOTTING-SMITH. ASTRAZENECA. We haveentered an era of radical change for clinicalprogram delivery. The pharmaceutical indus-try is experiencing pressures on price, earlierentry of generics, and lower output of newapproved medicines. R&D spending hasincreased 70% during the last decade. Deliv-ery of the clinical programs has a criticalimpact on both the time to delivery and thecost required to develop new medicines. Thefocus on improving clinical operations andfinding new models to deliver has intensifiedover the last five to seven years. Many optionsfor different elements of operational deliveryhave emerged, ranging from centralized datamanagement in low-cost countries to offshoreoptions for medical writing and safety surveil-lance. The large size of clinical programs, newtechnological solutions, new requirements fortranslational science, and the conducting of
studies across 20 or more countries has result-ed in a large, global, and complex infrastruc-ture that is expensive to manage. The mainrequirements for new operational models are,therefore, centered on consolidation, drivingadherence to simple standards, accessing thebest and most experienced talent, and findingcost-effective options while maintaining qual-ity.
GORDON. KING PHARMACEUTICALS. Withinthe clinical operations segment of the pharma-ceutical industry, improvements in availabili-ty of trained human resources are needed. Theidentification and recruitment of personnelwith the appropriate skills and experience areenormous challenges. Currently, training inthe clinical operations function occurs primar-ily in-house, because there are few academicprograms preparing people to work specifical-ly in this field. Most of the candidates for clin-ical operations who we see have a sound scien-tific knowledge base and come from apharmacy, nursing, or scientific background.Yet, when a person assumes a clinical opera-tions position, there is often a learning curvein developing the necessary skills for the posi-tion. overall success of a pharmaceutical organization, and like the
rest of the industry, the individuals who are on the clinical
front line are facing pressure to do more, faster, better, and of
course, more cost-effectively. PharmaVOICE asked industry
experts to find out how the changing environment is
affecting clinical operations.

States, but it is increasingly difficult tofind individuals with experience inglobal clinical studies, where a numberof studies are now conducted.
DOYLE. KFORCE. There are several chal-lenging areas facing today’s clinicaloperations managers. First, there is aneed to focus on cycle times and how toimprove efficiencies within site man-agement, which will positively impactsite and study collaboration. If process-es can be improved and errors can bereduced, then collectively we canimprove the industry as a whole. Toaccomplish these goals, we can look to
other industries, such as the IT and automo-tive industries and leverage the processimprovements they have gained by fully out-sourcing specific functions to strategic part-ners. Patient recruitment and improvedinvestigator performance are two other criti-cal success factors to any trial. There is arenewed focus on measuring site performancein terms of recruiting patients and developingbetter relationships with those who are par-ticipating. By applying lean operational pro-cesses to measure and improve efficiencies, wecan improve the quality of life for monitorsand directly impact cycle times as institution-al process redundancies are eliminated. Man-agers should be analyzing current processes tounderstand if assigning monitors to sitesinstead of trials might be a more efficient wayof resourcing. For example, they should eval-uate whether it makes more sense to sendmultiple monitors to the same site to monitormultiple studies, or does it make better senseto send one monitor to the same site and alignthem across therapeutic areas for greater effi-ciencies. Improving the myriad technologysystems used for communications and reduc-ing the instance of duplicate data entry willreduce the overall time consumed in redun-dant processes and also provide greater visi-bility to lead indicators, rather than reactingto lag indicators. Retention of staff is anothercritical element in the overall cycle time of atrial, and empowering front-line teams toachieve operational excellence and executeaccordingly will be a powerful tool in reduc-ing turnover.
EVANS. ESSENTIAL GROUP. I believe there aretwo major areas for improvement in the clini-cal operations space. One is to improveturnaround times of approved monitoringreports via electronic reports with e-signa-tures. CRAs must be able to access a Web-
based CTMS (clinical trial management sys-tem) database to import and export data whileat the site. It is also important that they havethe ability to generate the monitoring reportin real time during the visit. By doing this,monitors could decrease turnaround times bydays or even weeks compared with traditionalmethods. This not only improves quality butin many cases could favorably impact site pay-ments. Sites are motivated to enroll morepatients when payments are timely. The otheraspect involves improved CRF or EDC mod-ule designs that minimize redundancies indata collection, minimize collection of unnec-essary data points, and are intuitive to thestudy coordinators while still meeting theneeds of data management.
THOMPSON. INVENTIV CLINICAL. The industryis in dire need of common tools and metricsthat can be used across studies and sites tooptimize overall project management. Specifi-cally, we need common metrics that can betracked on both a historical basis to ensuresite/study productivity as well as during theactual conduct of the clinical trial to ensurethat corrective actions can be put in placebefore a clinical trial is in jeopardy.
GORDON. KING PHARMACEUTICALS. There isalso a need to refine EDC, a tool that has dra-matically advanced in the last decade allowingsite clinicians to capture data in real time.Many challenges remain in optimizing theproduction of these tools accurately and expe-diently. Moving toward a system based onEDC allows information to be better integrat-ed and more efficiently downloaded at the endof the study, but improvements are needed instudy start-up requirements. In addition toEDC use by clinicians, recording of patientdata electronically has improved accuracy byallowing patients to efficiently report progressin real time. This concept has evolved sub-stantially over the past 10 years, but there isroom for improvement in the developmentand validation of patient-friendly data capturesystems.
STEFANZICK. CRITERIUM. There is alwaysroom for improvement in communicationsand workflow, and that is an ongoing process.A clinical operations manager needs to be con-sistently attentive to the systems, the people,and how they interact with each other in eachstudy. For instance, studies should have teamsof in-house monitors or clinical data liaisonswho communicate with monitors in the field.Breaking down the division between clinical

outsourced/offshored or remain in house.Overhauling the entire drug-developmentprocess is more difficult since ensuring wedeliver a robust data set through classic phas-es of development to clearly determine thebenefit/risk of new products is pivotal to reg-ulatory agencies being able to approve newmedicines.
WAIFE. WAIFE & ASSOCIATES. I would identifytwo widespread needs in the industry. One isto adapt clinical-trial execution properly to e-clinical requirements and possibilities, andtwo, eliminate nonvalue-added work, which iseverywhere in clinical operations. I would alsoadd that there is a need to rationalize out-sourcing. What I mean is that there is a needto examine factually whether it truly and mea-surably makes sense to offshore, or use an FSP,or offload anything to either a nonpharmabusiness, or any business that the pharmacompany does not control. Sometimes itmakes sense to outsource and a lot of times itdoes not. Just like recent approaches to clini-cal operations reorganizations, outsourcing ispursued like fashion; fashions are supposed tochange every season, which is great for theclothing industry, but not for pharmaceuticaldevelopment processes.
Outsourcing Functions
Changes in responsibilities and outsourcing havecreated a more flexible, partnership-oriented,integrated function for the clinical operationshead.
THOMPSON. INVENTIV CLINICAL. The increas-ing pressure to deliver clinical trials in a cost-effective but safe manner has forced clinical-trial sponsors and their partners to work inmore of a partnership with monitoring plansand to address quality issues up front as partof the ongoing execution of the trial. Thislink between sponsor and partner requiresflexibility from the partner to meet a varietyof sponsor needs by providing trained staffwho have the proper experience, training, andelectronic connections to ensure proper clini-cal-trial execution and delivery of high-quali-ty data. On the back end, more and moresponsors and partners are instituting alessons-learned exercise to evaluate how wellthe team performed and what improvementscan be made in the future. The industry is alsolearning the value of flexible workforces. Theability to scale-up and size down rapidly withexperienced, trained staff is key to efficientand successful product development. Because
of this trend, the roles and responsibilitiesmaintained by the core staff are focusing onsenior project management and a flexibleworkforce can be brought in to handle theresponsibilities that can be outsourced. Forexample, medical writing, monitoring, anddata analysis are tasks that can be outsourcedto flexible work staffs without impactingstudy control and quality.
DOYLE. KFORCE. Many of the major pharma-ceutical firms have moved to regionally basedmonitoring solutions that are either partiallyor fully outsourced. Monitoring needs to beapproached in a top down and bottom upworkflow analysis. As such, people responsi-ble for site management can execute studyplans and provide feedback to mutuallyevolve the workflow. Some pharma companieshave outsourced study management, and webelieve that this trend will only increase ascompanies seek to outsource more of theiroperational roles so they can focus on the sci-ence of developing drugs. Functional serviceproviders are retaining more of the intellectu-al capital regarding site and study manage-ment as pharma companies outsource more ofthese functions.
GOTTING-SMITH. ASTRAZENECA. Historical-ly, the pharma industry has used CROs to takeon whole trials or certain activities for wholetrials when the in-house capacity is alreadyfull. In recent years, this partnership hasmoved toward business process outsourcing.In other words, companies are stopping theactivity in-house and are relying on a partner-ship with an external vendor to provide thisparticular service for all clinical studies. Exam-ples of this include clinical data management,which several pharma companies now based inIndia through external providers. Interesting-ly, more than 90% of the decisions that havebeen made to offshore data management to anexternal vendor have gone to companies thatdid not have data management capabilitiesfour years ago. This is a sign of just how rapid-ly the changes are happening. The types ofactivities that have been outsourced or offshored are data management, medical writing,statistical programming, biostats, and safetysurveillance.
GORDON. KING PHARMACEUTICALS. Over thepast few years, there have been a number ofchanges in the roles and responsibilities in theclinical operations function. EDC has been aprimary driver for shifting workflow sincemany elements of data entry are performed at

the clinical sites. The processes for validatingand querying data have shifted from manualreview to the development of programs thatidentify potential errors and generate ques-tions directed to the sites. Although EDC isneeded to improve the efficiency of the devel-opment system, this is a change in the waythat clinical-trial data have been captured andmonitored over the years. This, in turn, haschanged the investigator’s workflow andresponsibilities.
BUTLER. APTUIT. Information hand-off typi-cally slows the process, and integrated projectteams working in a collaborative environmentfrom the very beginning of a project —instead of working across traditional drugdevelopment silos — reduce the time burnedat the start of each stage. In practice, thismeans that there is a dedicated operationsproject leader, and not several project leadersacross each operational area. Also, by settingup standardized protocols for precursor mate-rials, data formats, and deliverable formats,scientists and others involved in a project canfocus on advancing a molecule into the clinicrather than designing a new process once theproject scope is defined. This does not meanthat the processes are not flexible and nimbleenough to accommodate the changingdynamics of any particular drug developmentproject, but rather that the groundwork is inplace to reduce the time required at thesedecision interfaces that typically slow the pro-cess.
WAIFE. WAIFE & ASSOCIATES. Most changesbeing pursued today are still adapting 20- or30-year-old practices to modern day realities.Too often companies experiment with dramat-ic reorganizations and role changes withoutany particular grounding in an operationalreality. We encourage the industry to re-imag-ine clinical development, which does not startfrom automating old processes but by design-ing fresh ones, based on fresh roles, whichrequires more courage from an HR perspec-tive, among others.
Technology Influencers
According to our industry experts, technologysolutions, particularly electronic data capture(EDC), have been the single biggest driver influ-encing the changes taking place in the clinicalspace.
GOTTING-SMITH. ASTRAZENECA. The adventof EDC for clinical studies five to 10 years ago
was really the first major step towardsignificant changes for clinical delivery.The options for electronic data captureare now primarily Web-based, whichhas reduced the overall volume of datachecking requirements for in-housestaff to review and check the quality ofdata coming in-house on paper. Morerecently, the development of datareview tools, new programming stan-dards, publishing tools, patient report-ed outcome tools — the list continues— have all revolutionized the way wedo our work. As a result, there are jobdescriptions that now exist in theindustry that did not exist five or 10years ago and I am sure this will con-tinue to evolve. Also, new providersemerge each year to provide improvedservices to our industry — whether tostreamline our payments to investiga-tors, to ensure we comply with fairmarket value, or to match patients to the tri-als we have open. To date, the greatest driversfor change have been the efficiency and costreductions gained from centralizing and off-shoring data management.
GORDON. KING PHARMACEUTICALS. EDC haschanged the role of data collection, making itmore efficient for both patients and site inves-tigators. As a result, the role of the data col-lector has evolved and requires a differentknowledge base. The impact of patient andsite electronic data collection is fairly consis-tent, both contributing to better data collec-tion and process outcomes. As EDC continuesto expand, many aspects of data monitoringcan occur remotely. Of course, all of thesechanges increase the challenges for investiga-tor sites in recruiting, training, and establish-ing optimal workflow to adjust to the changesin expectations.
DOYLE. KFORCE As trials have grown andmigrated to global markets, the need for real-time project management metrics is crucial.Data that are held in multiple systems and arepulled rather than pushed to front-line teamsare by definition less valuable than if the datawere available in real time to allow for proac-tive corrections. EDC has and will continue todramatically improve the process of data man-agement, but the use of CDISC and HL7 stan-dards, for example, is even more critical toimproving operational efficiencies. If thesestandards are not in place, manual processeswill simply become automated manual pro-cesses.

EDC, IVRS, e-diaries, study Web-portals, scanning, CTMS,and central lab Web-access all have opened thevisibility of field activity to the project team.In this global world, technology is the highestdriver impacting speed and quality, allowingfor improved program management and morerapid response times. Near real-time access todata and documents is available as neverbefore. This provides clinical operations a win-dow into quality measures and enrollmenthurdles so that rapid and proactive responsescan be employed. Technology solutions alsoenable global teams to rapidly exchange infor-mation and key learning across projects.
THOMPSON. INVENTIV CLINICAL. EDC andhealth record systems have offered our indus-try the opportunity to significantly changehow we do business, focusing more on proto-col compliance and patient safety rather thansource data verification and query reconcilia-tion. In addition, the access to electronic dataopens up potential opportunities of havinginitial data reviews done remotely. This freesup valuable on-site monitoring time for pro-tocol and regulatory compliance reviews.
STEFANZICK. CRITERIUM. EDC is becomingincreasingly popular with sponsors and gener-ally throughout the industry. What is oftenoverlooked is that EDC by itself cannot pro-vide real-time data; therefore it’s often betterfor sponsors to combine EDC with an IVRsystem. EDC combined with an IVR systemcan provide real-time information that gives asponsor the ability to make faster decisions.With trials becoming more global, it isimportant to have a good grasp of the cultureand infrastructure of the regions in which thestudies are being conducted, including mak-ing use of each region’s most current technol-ogy. Well-designed protocols will take cultur-al and technological factors into account toguarantee a successful trial.
BUTLER. APTUIT. Modern drug-developmenttechnologies have enabled scientists to devel-op many more new drug candidates than tech-nologies of the past allowed. With anincreased number of compounds, we’re alsowitnessing more poorly bioavailable drug can-didates. Advanced technology has actually cre-ated a greater need for operational team inte-gration than ever before. To effectivelydetermine which drug candidates to pursue,pharmaceutical and biotechnology innovatorsmust have a team in place, free of decision-making silos, to provide formulations that
will work in development and during scale-up; minimize the delays in information handoff; and focus on the candidates that have thebest possible chance of being commercialized.Information technology (IT) has also signifi-cantly impacted clinical operations. Evenbeyond pure data capture, IT has evolved to bean enabler of team integration by allowingreal-time access to a project from anywhere inthe world.
WAIFE.WAIFE & ASSOCIATES. All e-clinical tech-nologies — EDC, ePRO, CTMS, IVRS, AES— impact clinical operational roles. The lackof recognition of the benefits they bring seri-ously hinders the efficiency gains expected byexecutive management, and thescientific/business benefits possible. Forinstance, EDC challenges all players in clini-cal-trial execution to re-examine long-heldassumptions of responsibilities and process.And yet many, if not most, EDC adopters haveonly layered the technology like a veneer on apaper-based operational structure.
THOMPSON. INVENTIV CLINICAL. Medicalthought leaders, clinical operations, and datamanagement personnel must work hand inhand during the planning stages of a clinicaltrial to ensure that the data collected cananswer the questions needed for today’s andtomorrow’s patients. Many times there is arush to get to the first patient visit withoutadequate focus on the operational feasibilityof the clinical trial or the data necessary toanswer the critical questions. Sponsors andtheir provider partners need to work togetherto understand and agree upon the data pointsthat are needed to answer the critical trialquestions. Once this is decided, proper train-ing of the site management staff on each ther-apeutic area and protocol is key to gainingclean, meaningful data. EDC systems can alsoallow the early detection of failures in the con-duct of the study and the implementation ofa contingency plan to avoid the collection ofpoor or invalid data. Objectives must beclearly communicated to the team, includingthe sponsor, clinical operations, and the sites.And common technologies, such as Web sem-inars, videos, or teleconferences, can be valu-able tools to improve the teams’ consistencyand to communicate issues or changes inobjectives.
BUTLER. APTUIT. EDC is the most significantadvancement in managing clinical-trial dataand results. In working with our pharmaceu-tical and biotechnology clients, advocating

Using electronic data collection meth-ods is a way of getting more accurate,real-time data from the patient and thesite. This also allows for the sponsoringorganization to be aware of the data andthe study as it is conducted.
Key Performance IndicatorsThe one thing that hasn’t changed and
has become more important than ever is the needfor accurate metrics and favorable ROI.
DOYLE. KFORCE. Metrics are always criticaland will become more so as we focus on risingR&D costs, negative press regarding recentsafety issues, the increase in prescription drugcosts, and mounting FDA regulatory over-sight. If we focus on the process of runningclinical trials and apply lean operational met-rics to improving efficiencies, we can greatlyimprove safety, which is always the primaryfocus, and simultaneously better manageR&D costs and improve cycle times. Anytimewe improve efficiencies we improve profitabil-ity. Metrics, especially lead indicators, and theability to analyze and respond to those willgreatly improve the reactions of front-lineteams that are currently juggling gut instinctsthat are predominantly based on lagging indi-cators. If someone told me that analyzing abetter way to work would help save lives bymaking a drug available more quickly andsafely — I don’t think I’d care if they called it“improving cycle time” or “operational excel-lence” — I’d find all the ways to improve pro-cesses.
EVANS. ESSENTIAL GROUP. Metrics are vital toprogram success. Project managers, with pro-ject teams, have to identify leading and lag-ging indicators and then project or forecastprogram outcomes. It is no longer enough toreport what happened last week, and thenthink there is visibility into the future. Pro-jections, including rolling averages, means,trends, and gap analyses are necessary to pre-dict outcomes and make adjustments asquickly as possible.
THOMPSON. INVENTIV CLINICAL. Chasing the
wrong metrics can rob companies of efficien-cies and can ultimately mislead managementand distract them from key project objec-tives. Metrics such as data quality, enroll-ment rates, and regulatory status are keymeasures in the management of clinical oper-ations. For example, the ability to measuresite performance at the click of a button iskey to ensuring ROI is maximized for eachsite. Too often, efforts are spent on sites thathave historically poor quality or enrollment,which leads to very low ROI at the site level.The ability to identify and select the bestsites, set realistic expectations, and track per-formance against expectation allows an ongo-ing assessment of ROI. Additionally, havingrisk mitigation plans in place allows pharma-ceutical sponsors and their partners to derivethe highest ROI.
BUTLER. APTUIT. In drug development andclinical operations, metrics are vital not onlyto determine the success of an individual pro-ject but also to improve overall processes.Metrics are essential in defining the scope of aproject, ensuring that the project is progress-ing within the proper timeframe, and captur-ing the type of data sponsors want to collect.Solutions and standards in measuring metricsand ROI are not found in a new technology orcomputer program, but rather can be seen inthe evolving relationship between an innova-tor company and contract organizations. Theindustry has always recognized the necessaryrelationship between a pharmaceutical com-pany and contract organizations, but nowthere is also the realization that those rela-tionships are not merely a product of capacityissues but rather are driven by collaborativemotives. Contract research organizations areplaying a much larger role in the develop-ment of drug candidates and advancing thosecandidates into the clinic, and similarly, phar-maceutical and biotechnology companies arelooking to contract organizations to step out-side the role of simply managing manufactur-ing and clinical trial-operations to leveragescientific expertise as a way to positivelyimpact the companies’ overall portfolios andpipelines.
WAIFE. WAIFE & ASSOCIATES. Of course metricsand ROI are very different from each other.Focusing on only a few metrics can be usefulto operational guidance, motivation, and eval-uation. The trick is choosing the right onesand getting everyone to know them, ownthem, and learn from them — which is veryrarely done. ROI is a completely different

matter and usually misunderstood. The pri-mary barrier to useful ROI discussions isthat virtually no biopharma company accu-rately understands its current developmentoperational costs, so that a “return” on a“change” cannot be compared with anythingmeaningfully, because the denominator isunknown. With some research and thought-ful analysis, however, this understanding canbe achieved.
GORDON. KING PHARMACEUTICALS. From thepharmaceutical perspective, we cannotoveremphasize the importance of metrics inthe clinical operations realm. Clinical trialsare extremely expensive to initiate, fromprotocol development through patientenrollment and trial completion. In addi-tion, pharmaceutical companies are facingcontinued challenges from regulatory agen-cies to meet ever-changing guidelines andprotocols. All available data to measuretime and costs to complete each step of thedrug-development process are vital to iden-tifying process improvements that can ulti-
mately reduce costs for sponsors. In thefuture, I expect that companies will examineeach step in the clinical operations process todemonstrate more efficient ways of conduct-ing a trial — for example, determining bet-ter methods of patient enrollment or moreefficient means for closing the database.Ultimately, time and cost-savings in theclinical-trial process will have a positiveimpact on ROI. ✦
