AbstractType 1 diabetes is an autoimmune disease caused by the immune-mediated destruction of insulin-producing
pancreatic b cells. In recentyears, the incidence of type 1 diabetes continues to increase. It is supposed that genetic, environmental
and immune factors participate inthe damage of pancreatic b cells. Both the immune regulation and the immune response are
involved in the pathogenesis of type 1 diabetes,in which cellular immunity plays a significant role. For the infiltration of CD4+and CD8+
T lymphocyte, B lymphocytes, natural killer cells,dendritic cells and other immune cells take part in the damage of pancreatic b
cells, which ultimately lead to type 1 diabetes. This reviewoutlines the cellular immunological mechanism of type 1 diabetes, with a particular
emphasis to T lymphocyte and natural killer cells, andprovides the effective immune therapy in T1D, which is approached at three stages.
However, future studies will be directed at searching foran effective, safe and long-lasting strategy to enhance the regulation of a diabetogenic

Diabetes tipe 1 adalah penyakit autoimun yang terjadi akibat destruksi sel beta pankreas yang memproduksi insulin. Belakangan ini insiden penyakit diabetes tipe 1 semakin meningkat. Faktor genetik, lingkungan dan imun diduga berperan dalam proses kerusakan sel beta pankreas. Baik respon imun maupun regulasi imun berperan dalam patogenesis penyakit diabetes tipe 1, dimana imunitas seluler memainkan peranan yang sangat penting. Infiltrasi dari sel CD4+ dan CD8+ limfosit T, limfosit B, sel Natural Killer, sel dendritik dan sel imun lainnya berperan juga dalam proses kerusakan sel beta pankreas, dimana pada akhirnya menyebabkan penyakit diabetes tipe 1. Dalam jurnal ini dibahas tentang mekanisme imunologikal seluler dari penyakit diabetes tipe 1, ditekankan pada peran limfosit T dan sel Natural Killer, dan menyajikan terapi imunitas yang efektif pada penyakit diabetes tipe 1 yang dapat diraih melalui 3 tahapan. Namun penelitian lebih lanjut akan diarahkan dalam mencari strategi terapi yang efektif, aman dan jangka panjang dengan efek samping yang lebih sedikit, dengan atau tanpa terganggunya fungsi imun secara keseluruhan.
Sel Th1
Sel Th1 bertanggung jawab dalam respon imunitas seluler dan respon protektif fagosit, yang dapat juga menghancurkan islet sel beta dan mempercepat timbulnya penyakit diabetes tipe 1 lewat produksi dari interferon gama dan IL-2. Namun, peran sitokin ini dalam patogenesis T1D masih kompleks. Sebagai contoh, interferon gama memiliki 2 peranan dalam penghancuran sel beta, yaitu lewat transduksi sinyal dan aktivator dari transcription-1 (STAT-1) pathway, dan berperan juga dalam melindungi lewat faktor interferon regulatory factor-1 (IRF-1) pathway.
Sel Th2
Sel Th2 bertugas memproduksi IL-4 dan IL-10, yang bertanggung jawab dalam memproduksi antibodi kuat, aktivasi eosinofil dan penghambat dari beberapa fungsi makrofag. Pendekatan imunoterapi seperti contoh stimulasi anti-CD28, yang dapat mempromosikan dan mendukung fungsi dari sel Th2 dan sekresi IL-4, dapat mencegah onset penyakit diabetes tipe 1 secara efektif. Tikus NOD transgenic yang mengekspresikan IL-4 di sel islet pankreas terbukti terlindung dari penyakit diabetes. Onset hiperglikemi pada tikus NOD berkurang setelah
diberikan IL-4 ke dalam sel beta pankreas secara in vivo. IL-10 adalah sitokin imunoregulator yang memiliki banyak efek. Beberapa bukti menunjukkan bahwa IL-10 penting dalam membangun toleransi imun pada tikus NOD, namun beberapa melaporkan justru IL-10 memiliki efek sebaliknya. Telah banyak diketahui bahwa sel Th1 meregulasi imunitas seluler, dimana sel Th2 memediasi respon imun humoral dan respon alergi. Sel Th1 dan Th2 telah ditelti lebih lanjut, sitokin Th1 dipercaya memperparah penyakit diabetes tipe 1, sedangkan sitokin Th2 memiliki efek sebaliknya, yaitu memproteksi dari penyakit diabetes tipe 1. Namun, ada penelitian lain yang mengindikasikan bahwa kedua sitokin baik Th1 dan Th2 bekerja sama dalam menghancurkan sel beta islet, yang akhirnya berujung pada hiperglikemia.
