Meet and perform with The ROAR of the Jaguars 2012, Jacksonville’s professional cheerleading squad. Be part of the Jaguars season tradition and star in the Junior ROAR Halftime Show November 25th! Voluntary Medical Consent In consideration of allowing my child to participate in the athletic and related activities of the Junior ROAR I, ___________ (parent/guardian of the child named below, the “MINOR” ), having actual knowledge, and conscious appreciation of the inherent and particular dangers involved in the participation in athletic and related activities, do voluntarily consent to my child’s participation in the aforementioned activities and assume the risk arising therefrom. I certify that my child has been examined by a physician and found to be in good health and able to participate in all activities without restriction. Should routine first aid or emergency medical needs arise, I authorize representatives or agents of the Junior ROAR to provide or consent to necessary treatment on my behalf. In the event that a major medical emergency or trauma requires treatment, I authorize representatives of the Junior ROAR to consent to such treatment of my child on my behalf and to obtain any emergency medical care that may become necessary in my absence. I understand that I will be responsible for the cost of any such treatment. I hereby release and hold harmless any and all representatives, agencies and employees of the Junior ROAR, the Jacksonville Jaguars, LLC., and all other promoters, sponsors, employees, volunteers and others involved with the Junior ROAR from and against any and all claims or liability arising from the provision of authorization of medical treatment to my child.Release and Assumption of Risk In consideration of ____________ (name of minor) being permitted to attend the Junior ROAR and being permitted to participate in all activities of the training clinic, I ___________, as the parent/legal guardian of the above named MINOR, release, waive discharge and covenant not to sue Jacksonville Jaguars, LLC., its officers, employees, partners and members and all other promoters, sponsors, employees, advertisers, volunteers and any others involved with the Junior ROAR (all referred to as RELEASEES), from any claim

for damage or otherwise, on account of injury to the person or property or resulting in death of the MINOR, whether caused by the negligence of RELEASEES or otherwise while the MINOR is attending the Junior ROAR. I, on behalf of the MINOR, acknowledge and consciously appreciate that the activities of practicing, training for and performing dance routines are inherently dangerous activities and that many injuries, including serious injuries involving paralysis and death, occur to people involved in such activities. I knowingly and expressly assume any and all risks arising therefrom. Further, I assume full responsibility for any risk of bodily injury to, or death of, the MINOR, while the MINOR is a participant in the Junior ROAR, whether due to the negligence of the RELEASEES or otherwise. I agree to indemnify the RELEASEES and to hold each of them harmless from and against any and all loss, liability, damage or cost RELEASEES may incur due to the participation of the MINOR in the Junior ROAR, whether caused by the negligence of RELEASEES or otherwise. I agree that this Release and Assumption of Risk is intended to be as broad and inclusive as permitted by the laws of the state of Florida and that if any portion of this agreement is held invalid, the balance shall continue in full force and effect. I have carefully read the above Release and Assumption of Risk and understand its contents. This Release and Assumption of Risk contains the entire agreement between the parties and the terms of this Release and Assumption of Risk are contractual and not a mere recital.Photo/Video Release I hereby authorize the Jacksonville Jaguars to make, reproduce, broadcast, sell, license and otherwise use photographs, films, videotapes, recordings or other depictions or images in whatever form or media of me and/or my child in connection with my child’s participation in the cheerleader training clinic without compensation and for an indefinite period of time. The rights herein granted to the Jacksonville Jaguars shall also accrue to persons, firms and corporations who may hereafter acquire ownership for the right to use subdepiction or images from the Jacksonville Jaguars.
