Immunization Technical Support Unit (ITSU) is a strategic arm of Ministry of Health and Family Welfare (MoHFW) that works towards strengthening of Universal Immunization Program (UIP). ITSU reviews the reported data on Child Immunization (service delivery) that is captured in Health Management Information System (HMIS) and Mother and Child Tracking System (MCTS). The same is analysed, developed into immunization dashboards on monthly basis, program gaps are identified and state specific feedback is provided. Immunization dashboards aim to improve quality and use of reported data on immunization to promote concrete programmatic actions for addressing the challenges in strengthening UIP. Dashboards provide state wise performance on various immunization component (timeliness, coverage, VPD cases, etc.) and concurrent feedback regarding reported immunization data to MoHFW, partner organizations and immunization program managers at state level. Additionally, these dashboards collate data provided by World Health Organization – National Public Health Surveillance Project (WHO-NPSP) related to concurrent monitoring and manual reporting for newly introduced vaccines, ‘Performance of AEFI surveillance and reporting in the states’ and ‘RI Communication Indicators’. As per the HMIS guidelines, it is mandatory for all the states to upload monthly data of the preceding month on or before the 20th of each month. After this deadline, ITSU downloads the HMIS and MCTS data, analyses the data including data from other sources and prepares the dashboards. For example, data for the month of April 2018 is downloaded after 20th of May 2018, collated, analysed and published as ‘Immunization Dashboard June 2018’. On a monthly basis, a group of states is provided detailed feedback on immunization coverage and data quality issues. The following table explains the framework for categorization of states and union territories for providing the feedback.

0 – 11 Months Vaccination

• During April-May 2018, Nation's Full Immunization Coverage was 70%. Mizoram (8%),

Haryana (22%), Rajasthan (24%), Lakshadweep (26%), Manipur (30%), Punjab (35%), Telangana

(36%), A & N Islands (46%), Arunachal Pradesh (53%), Assam (55%), Daman & Diu (57%),

Puducherry (62%), Uttar Pradesh (63%), Madhya Pradesh (66%), Himachal Pradesh (67%),

Chhattisgarh (67%) and Nagaland (67%) were the states/UTs below the national coverage.

• Hep B birth dose is to be given within 24 hours of birth and is mandatory for all institutional

deliveries; however, it was given in 80% of institutional deliveries in the country. Mizoram

(45%), A & N Islands (52%), Jharkhand (54%), Assam (57%), Daman & Diu (59%), Chhattisgarh

(62%), Uttar Pradesh (63%), Kerala (65%), Goa (66%), Manipur (71%), Haryana (72%), Tripura

(76%), Maharashtra (76%), Punjab (78%) and Lakshadweep (79%) were states/UTs below the

national coverage.

• National coverage for BCG is 70%. Rajasthan (25%), Lakshadweep (31%), Telangana (38%), A &

N Islands (46%), Daman & Diu (48%), Sikkim (48%), Arunachal Pradesh (51%), Madhya Pradesh

(54%), Himachal Pradesh (58%), Mizoram (59%), Uttar Pradesh (60%), Delhi (63%), Punjab

(68%), Haryana (68%) and Odisha (68%) were the states/ UTs below the national coverage.

• National coverage for Penta-3 is 76%. Rajasthan (24%), Lakshadweep (33%), Telangana (34%),

Daman & Diu (49%), Puducherry (56%), Arunachal Pradesh (57%), A & N Islands (60%), Uttar

Pradesh (63%), Madhya Pradesh (65%), Himachal Pradesh (70%), Haryana (71%), Sikkim (71%)

and Tamil Nadu (75%) were the states/ UTs below the national coverage.

• National coverage for MCV-1 is 72%. Mizoram (9%), Rajasthan (25%), Lakshadweep (26%),

Manipur (33%), Punjab (36%), Telangana (39%), Assam (56%), Arunachal Pradesh (56%), Daman

& Diu (57%), Uttar Pradesh (63%), Madhya Pradesh (66%), Himachal Pradesh (67%),

Chhattisgarh (68%), Puducherry (69%) and Sikkim (70%) were the states/UTs below the national

coverage.

• National coverage for JE-1 dose (States with 231 JE-endemic districts) is 65%. Chhattisgarh

(0%), Kerala** (1%), Haryana (16%), Manipur (17%), Odisha (17%), Telangana (49%), Arunachal

Pradesh (49%), Uttar Pradesh (57%), Assam (57%) and Nagaland (64%), were the states/ UTs

below the national coverage.

• National coverage for RVV 1st & 3rd dose (in RVV introduced States) is 57% & 58% respectively.

Of the ten states where it was introduced, two states for RVV-1 and three states for RVV-3 have

reported below national coverage.

o For RVV 1st Dose – Rajasthan (22%) and Haryana (50%).

o For RVV 3rd Dose – Rajasthan (22%), Haryana (53%) and Madhya Pradesh (55%).

• National coverage for fIPV-1 is 60%. Rajasthan has reported lowest coverage of 21%.

• National coverage for fIPV-2 is 61%. Rajasthan has reported lowest coverage of 21%.

12-23 Month Vaccination:

• National coverage for MCV-2 is 66%. Lowest coverage reported by Mizoram is 10%.

• National Coverage for JE 2nd dose (States with JE-endemic districts) is 65%. Chhattisgarh (0%),

Odisha (8%), Haryana (20%), Manipur (20%), Telangana (38%), Arunachal Pradesh (47%),

Jharkhand (48%), Maharashtra (57%), Uttar Pradesh (60%) and Assam (64%) were the states/

UTs below the national coverage.

• National coverage for DPT Booster-1 is 72%. Lowest coverage reported by Lakshadweep is 21%

and Rajasthan is (22%).

5-6 years Vaccination:

• National coverage for DPT Booster-2 is 44%. Lowest coverage reported by Puducherry (1%),

Rajasthan (14%), Mizoram (19%) and Nagaland (19%). Kerala showing highest coverage of 132%

reported.

Drop-out Rate (Penta1–Penta3)

• National level drop-out for Penta1-Penta3 is (-)3.2%. Meghalaya reported highest drop-out

of 17%.

• Most of the states still reporting coverage data on Hep-B 1, 2 and 3 that is mentioned in

‘Administrative Data’.

• Timeliness of HMIS Data – Non-reporting districts (42) for the month of May 2018 only:

• Rota Virus Vaccine (RVV) – RVV was introduced in phased manner in the country. In 2016 under

phase-1, four states were included viz. Andhra Pradesh, Himachal Pradesh, Haryana and Odisha.

In 2017 under Phase-2, five more states were included viz. Assam, Madhya Pradesh, Rajasthan,

Tamil Nadu & Tripura making a total of nine states. In 2018 under Phase-3, it has been

introduced in Jharkhand. Status of RVV coverage as per HMIS portal reported by states:

Pneumococcal Conjugate Vaccine (PCV) – PCV was introduced in 2017 in three States viz. Bihar

(July’17 in 17 districts), Himachal Pradesh (June’17 in 12 districts) & Uttar Pradesh (June’17 in 6

districts). Vaccine is newly introduced in 2017, reporting fields are not introduced in HMIS portal,

hence manual reporting is done from states and are same being reviewed and analyzed. In 2018,

PCV was introduced in two more states viz. Madhya Pradesh (April’18 in 51 districts) and Rajasthan

(April’18 in 9 districts) with expansion in Uttar Pradesh (May’18 in 6 districts).

• Measles & Rubella (MR) – MR campaign is being conducted in the country in phased manner.

During phase-1, five states (Goa, Karnataka, Lakshadweep, Tamil Nadu, Puducherry) and in

phase-2, eight states (Andhra Pradesh, Dadra and Nagar Haveli, Daman and Diu, Chandigarh,

Telangana, Himachal Pradesh, Kerala and Uttarakhand) were covered. In phase-3, six states

(Odisha, Arunachal Pradesh, Manipur, Mizoram, A&N Islands, Haryana) have started activity

out of which Odisha & Arunachal have completed the activity.

• Japanese Encephalitis (JE) – JE campaign is being conducted in the country in phased manner

(231 JE endemic districts). During phase-1, 201 JE endemic districts (in 20 states) were covered.

In phase-2, 16 JE endemic districts (in 04 states – Arunachal Pradesh, Jharkhand, Telangana and

Tripura) were covered. In phase-3, selected 14 districts of two states (Odisha & Chhattisgarh)

were covered and total 74% of children were vaccinated against the target.

• Mission Indradhanush (MI) Campaign under Gram Swaraj Abhiyan

Gram Swaraj Abhiyan (GSA) was launched as a campaign under the Ministry of Panchayati Raj

and was initiated under the name of “Sabka Sath, Sabka Gaon, Sabka Vikas” with the aim of

integrated development at the village level, promotion of social harmony and spreading

awareness about pro-poor schemes of government. MI was one of the 7 programmes being

undertaken with GSA, to increase the immunization coverage in selected villages with

underprivileged population. A total of 16,850 villages have been identified in 533 districts across

27 states. All union territories (UTs) are also covered under this activity. Mission Indradhanush

(MI) under Gram Swaraj Abhiyan is planned to reach unvaccinated and partially vaccinated

children and pregnant women in these villages. The 1st and 2nd round of MI under GSA was

conducted from 23rd to 27th April 2018 and 21st to 27th May 2018 respectively. Coverage

related indicators of MI GSA are given below.

The concurrent monitoring indicators mentioned below are from the available data of the

monitored states.

• Percent monitored children found ‘fully immunized’ is 83%

o Lowest percentage of children found ‘fully immunized’ was in Manipur (64%)

• Percent monitored immunization sessions not held was 7%

o States with high percentage of monitored immunization sessions not held:

Uttarakhand (10%) and Jharkhand (9%), Maharashtra (8%), Uttar Pradesh (8%).

• Percent of monitored sessions with due list not available was 26%

o States with % of monitored sessions with due list not available: Jharkhand (46%),

Bihar (31%) and Karnataka (30%).

• Percent of monitored sessions where all vaccines were not available was 15%

Uttarakhand (100%), Karnataka (98%), Jharkhand (76%), Delhi (46%), West Bengal

A total of 26 States/ UTs, including Andhra Pradesh, Arunachal Pradesh, Bihar, Chandigarh,

Dadra & Nagar Haveli, Daman & Diu, Delhi, Goa, Haryana, Himachal Pradesh, Jammu & Kashmir,

Jharkhand, Lakshadweep, Madhya Pradesh, Maharashtra, Manipur, Meghalaya, Mizoram,

Nagaland, Odisha, Punjab, Sikkim, Telangana, Tripura, Uttarakhand and West Bengal have

migrated to RCH Portal, so data of these states is not available in MCTS portal. Three districts

of Uttar Pradesh have started migrating data in RCH portal as reported by MoHFW.

• Only 17.9% of data entry for infants was done in April-May 2018 on pro-rata basis at MCTS

portal, with lowest entry made by Kerala (2.61%) and Chhattisgarh (5.69%).

• Percentage of villages not mapped with any facility in MCTS portal: Total of 3.63% villages

were not mapped in India. A & N Islands (39.04%), Assam (8.8%) and Rajasthan (7.71%)

were the three states with highest percentage of villages not mapped in MCTS portal.

• National Full Immunization Coverage (FIC) was 0.02%.

• Antigen-wise all-India coverage against registered children in April-May’18:

- Hep B birth dose: 40.25% - A&N Islands, Assam, Puducherry and Rajasthan and Uttar

Pradesh have reported below national coverage whereas.

- BCG: 47.69% - A&N Islands, Assam, Puducherry, Rajasthan and Uttar Pradesh

reported below national coverage.

- Penta 3rd dose: 1.74% - A&N Islands, Karnataka, Manipur, Puducherry, Rajasthan

and Tamil Nadu did not report any coverage for Penta 3rd dose.

- Measles 1st dose coverage was 0.33%.

- Measles 2nd dose coverage was 0.03%.

- DPT Booster coverage for the country was 12.06%.

The communication indicators mentioned below are from the available data of the

monitored states.

• Percent monitored sessions with one/more mobilizers found working was 98% and states

below the average were Chhattisgarh (88%), West Bengal (94%), Madhya Pradesh (97%)

and Uttar Pradesh (97%).

• Percent monitored sessions where ASHA was found working was 74% and states below

the average were West Bengal (54%), Uttar Pradesh (65%) and Chhattisgarh (72%).

• Percent monitored sessions where AWW was found working was 59% and states below

the average were West Bengal (26%), Chhattisgarh (29%), Maharashtra (35%), Madhya

Pradesh (42%) and Uttar Pradesh (54%).

• Percent monitored sessions where Link Workers found working was 4% whereas state

wise: West Bengal (25%), Gujarat (17%), Odisha (8%), Uttar Pradesh (6%), Bihar (3%),

Madhya Pradesh (3%), Chhattisgarh (0%) and Maharashtra (0%).

• Percent monitored session sites with IEC visibility was 66% and states below the average:
