tAI AVAILABILTY oF APPI-ICATION FORM :-

Application forms for admission to recruitment examination of Junior Specialistcan be obtained from the Commission's office ltanagar, on all working days from 10 AMto 4 PM upto 30-04-2014 onipayment of Rs. 12Ot: only forAPST c-andidates and Rs.1801 only for General Candidates in cash, No separate examination fee in the form ofDD/Treasury challan will be ltequired along with the application. The examination feehas been included with the cost of application form.

tBI ELIGIBILITY:- ;

i

(i) Candidates must hold a recognized Medical Degree.(ii) Post graduate Degree/Diploma in the concerned specialty.(iii) Working experieqllce of 3 (three) years for post graduate degree, 5 (five) years

for post graduate' Diploma from a Govt. Health institution or Govt.sponsored/Recogh:ized health institution is compulsory.

The selection will be made through Viva-voce/interview only,However, ifthe numbers of applications rreceived in respo-nse to the ,Jr.rtir.r"ni'ir huge, thecandidates will be short listdd: on the basis of marks obtained in screening test ingeneral knowledge paper ca,r'/ying 100 marks as may be decided by the Commiision.

lEl HOW To APPLy :-

Along with the duly filled up application, candidates must enclose thefollowing documents other wise the application will be rejected.

i. 2 (two) copies ,of recent color pass-port size photographs with selfattestation in the'front size which should be pasted on the space providedin the application form and attendance sheet.

ii. Self attested copies-of certificates and marksheets issued by the AssistantRegistrar/Registrpri Dy.Controller/Controller of ExaminationA/ice-Chancellor of the concerned University to ascertain educationalqualification.

iii' Self attested copy of Matriculation Certificate for proof of age issued bythe Board only will be accepted to ascertain the age of candidate.

iv. Self attested copy of ApST certificate.

v. self attested copy of pRC in case of General candidates.

vi. self attested copy of Arunachal pradesh Medical councilCertificate ;

a) The decision of the Commission as to the eligibility or otherwise of acandidate for admission to the interview or selection will be final and nocorrespondence or personal enquiry will be entertained.

b) Candidates already in service may apply directly to this Commission withNOC from his/her Head of Deptt.

c) Application-in the prescribed form along with necessary documents shouldreach this Commission in a cover/envelope superscribed " Application forthe post of Junior: Speciatist(Eye)-2A14.

d) The Commissionrwill not be responsible for delay in receipt of applicationdue to any reason whatsoever,

E) APPLICATION FORM OTHER THAN THE ONE PROCURED FROM THE APPUBLIC SERVICE COMMISSION, ITANAGAR WILL BE SUMMARILYREJECTED. NO DUPLICATE OR PHOTOCOPY FORM WILL BEENTERTAINED.
