The student will be able to:-

• Prepare nursing care plan of a patient

Activities of Daily Living (ADLs)
• Activities of daily living in health care are routinely performed by any individual to maintain themselves.
• They are: feeding ourselves, bathing, dressing, grooming, homemaking, and leisure activities.

General Duty Assistance in the Hospital
The GDA’s assistance may be of help in doing the following self care activities.
• Bathing and showering (washing the body).
• Bowel and bladder management (related to incontinence).
• Eating (difficulty in chewing and swallowing)
• Dressing

• Feeding (setting up food and bringing it to the mouth)
• Functional mobility (moving from one place to another while performing activities)
• Personal device care
• Personal hygiene and grooming (including washing hair)
• Toilet hygiene (completing the act of relieving oneself)

Daily Care Plan of Patients by General Duty Assistant (GDA)
• Way to reduce stress of a patient is to establisha regular routine.
• Fine-tuned to ensure patient seemscomfortable.
• Environment needs to be relaxed and friendly.
• Pictures of happy days, or incense, or music , ifthese are helpful to the patient feel comfort.

Helping Tips
These are some specific activities in health care:-
• Bathing
• Grooming
• Dressing
• Toileting
• Dental care
• Eating
• Drinking water
• Taking medicine10
Bathing
• Temperature of the water is normal.
• A bathroom can be very unsafe for a patient ifleft alone, so stay with the patient.
• A thin towel can be used.
• A bath stool may be needed so that the patientsits down comfortably for the bath.
• Use the bath time to check the patient forinjuries and sores.

Grooming
• Patients may cut themselves while shavingwith an ordinary razor, and therefore switchto twin blade or electric razors.
• Combing of hair is another activity that needsassistance.
• Fine coordination activities like nail cuttingand filing also needs help to perform.
• If patients are unable to groom forthemselves, the caregiver needs to ensure thepatient is presentable.

Dressing
• Clothing needs to be comfortable to wear aswhen coordination reduces it becomes difficult.
• Make sure clothes are not long to avoid fall.
• Switch to clothes without much zippers orbuttons.
• Instead of choosing dress with strings, use withelastic or Velcro straps.
• Use shoes with Velcro straps instead of shoeswith laces.

Toileting
• Incontinence occurs for many reasons likeinability of patient to reach the bathroom intime, or forgetting where the bathroom is.
• Be aware of signs of constipationand dehydration in patient and change diet andwater intake accordingly.
• If the patient shows signs of pain while passingurine or during bowel movements, consult withthe doctor.
• Accidental fall may occur in toilets, hence itshould be kept dry always.

Toileting
• Grab rails or toilet seats with rails may makethe experience of sitting on the toilet seat lessfrightening for the patient.
• Watch the patient to ensure proper wiping, andproper washing of hands.
• For visits outside, diapers may be a good option.Patients will need assistance in wearing andremoving diapers.

• Ensure patient is reminded of taking food by a caregiver.
• Food may need to be cut down in smaller sizes to cheweasily.
• Patients may not mix food while eating hence caregiversmay need to mix food or make combined dishes likepulao, khichdi, bisi bele rice and curd rice.
• Chewing becomes a problem over time, and food mayneed to be made softer, and finally, liquidized in a Mixer.
• Consult doctors about diet supplements

General Instructions for GDA in Feeding Patients
• Diet of patient should be planned according tohis need, metabolic changes, food habit andsocioeconomic status.
• Provision should be made to wash hands andthe face of the patient before and after themeals.
• Dressings and painful treatments are finishedat least 1 hour before meal is served.
• Playing a soft music adds to the pleasure of thepatient and can serve as appetizer to the meals. 18

• The patient should be placed in a comfortable position inbed or out of bed.
• Meals should be served in clean and covered containers.
• Fluid requirement should be met to prevent dehydration.Fluids are given at the end of a meal or in between themeals.
• The patients on tube feeding may be given a chance to tastethe food so as to arouse the appetite and for theirsatisfaction.
• The GDA should record and report the quantity of food thepatient has eaten.
Drinking Water
• Sometimes, patients reduce their water intake,to avoid repeated trips to the bathroom.
• They may also forget to drink water.
• Caregivers need to make sure that patients aredrinking enough water.
• Doctors may also ask patients to includeelectrolyte drinks in the daily routine duringelectrolyte imbalance.

Taking Medication
• Patients cannot keep track of their medicines.so use small labeled boxes for the medicines.
• Forgetting to take medication is a commonproblem thus, caregivers should remind this.
• Doctor should be consulted if the patient finds itdifficult to chew or swallow
