Ongoing commitment to continuing professional development. Key Tasks and Responsibilities:

To provide assessment and appropriate speech and language therapy intervention for students with a range of speech, language and communication disorders (some of which are complex in nature), with support and advice from supervising clinician as required.

To develop and provide communication plans for specified caseload, to include communication strengths and weaknesses and strategies for use by staff in order to develop students’ communication skills and enhance communicative effectiveness between staff and students

To set individual communication targets and objectives linking to EHCP outcomes for inclusion in students’ Individual Learning Plans, and to monitor and evaluate progress towards achievement

To contribute to the planning and delivery of weekly group therapy sessions

To provide written language targets for inclusion in students Individual Learning Plans.

To provide appropriate information, advice and support to students, parents/carer and other relevant services within and out of Homefield, e.g. NHS, Local Authority, private care providers.

To prioritise caseload based on students’ needs, Homefield standards, and RCSLT best practice guidelines.

To liaise with all relevant professionals, including other therapists, regarding overall student care and service provision, working jointly with them where this is appropriate to meet their communication needs.

To work with all appropriate agencies and colleagues when necessary as part of the multi-disciplinary team, establishing collaborative working relationships, sharing skills and information to facilitate holistic student care.

To make referrals to relevant specialist services for individuals requiring assessment for a communication aid or dysphagia

To share transition documents and resources with future providers for students finishing their college placement

To facilitate compliance with the Accessible Information Standard and RCSLT 5 Good Communication Standards

To participate in clinical audit procedures and contribute to service development projects

To attend specialist short courses/conferences/clinical excellence networks appropriate to adult learning disability

To write annual progress reports for individual students in a timely manner

Attend and contribute to multi-agency meetings including annual reviews

Work collaboratively with other independent specialist colleges

Demonstrate a working knowledge of the Special Educational Needs and Disability (SEND) Code of Practice.

To recognise the need for and accept clinical support and advice from supervising clinician.

To develop and update clinical knowledge and skills under own initiative and the direction of supervising clinician.

To evaluate and monitor clinical input using evidence-based reflective practice and outcome measures.

To participate in planning and delivery of student-specific intervention and training in order to meet communication needs and promote independence and well-being

To take part in/implement all relevant activities, standards, procedures and policies related to Clinical Governance as required by the RCSLT and HCPC.

To undertake administrative duties to ensure records are maintained in accordance with agreed standards.

To act at all times in such a way as to promote a positive image of the SALT service to all.
