Welcome to our Safety First Brief for January.

Welcome and Happy New Year to all. I hope you had a great Christmas, and my thanks go to 

everyone who gave up time at home with family and friends to work on our projects throughout 

the country. We delivered on a huge scale and with no major disruption to the network.

We’ve received some feedback from the last survey on the content of your monthly Safety First 

brief, and have started to incorporate some of the feedback and suggestions into this month’s brief. 

The idea is to continually evolve this to give you as much meaningful information as possible, and 

incorporate your thoughts and ideas – so keep in touch.

We had only 1 minor accident and 1 incident in December, so thanks for your vigilance and 

attention. We anticipate 2019 will be a very busy year for us so let’s keep the focus on working and 

acting safely, and take care of yourselves and each other.

Accidents and Incidents

1. First Accident Details

Acting as MC, IP was walking in front of machine. As he approached a viaduct hewalked towards the authorised walking route rather than walk on the ballast andsleepers. While walking on the steel walkway he did not see two steps and fellforward, onto steel walkway, injuring knee and arm.

Investigation

Low light conditions, plus there was no demarcation of the stairs on the walkway(i.e. edge strips or paint). The IP simply did not see the change in walkway fromlevel to steps. Pictures of walkway taken and sent onwards.

Recommendations

Client to mark or paint the steps to prevent future falls. Lighting could be added at this location to prevent further issues.

Accident Trends for 2018Following the overview last month of the injuries we’ve suffered over 

the year,  the final figures for 2018 are as follows:

As per last month, the common theme is an injury occurring when

someone has been used equipment incorrectly, or using equipment

without task specific PPE.

Remember to raise any concerns about unsafe working to your

supervisor onsite or to ISS oncall.

1. Incident Details

ISS Nominated Person was contacted by Authorised Person from Amey to advise a train was inthe DEP location and preventing the AP from applying earths at the Isolation limits. To avoidfurther delay, the NP advised the AP to come inside the planned Isolation limits (shorten) andapply the earths at a new DEP. The AP incorrectly walked out‐with the isolation limits andattempted to apply the earths at a DEP location which was in a live section. This caused thebreakers to trip and some damage to the OLE equipment, as well as being very dangerous tothe individual involved.

Investigation

The AP walked in the wrong direction, and did not take the live line indicator (tester) with him.Therefore he could not test the line was dead or live when he reached the new location. The NPcould not tell the AP which structure to apply the earths at as this was an ad‐hoc change to theplan due to the train blocking the planned DEP. If the NP could have identified an alternativestructure this may have warned the AP he was in the "wrong" location. Finally, the NP couldhave made the decision to stick to the original plan and wait until the train had moved beforecompleting the Isolation works, but this would have delayed the works.

Recommendations

The NP had minimal involvement so as per the Client investigation will have the incidentrecorded, and will undertake a rebrief/assessment with ISS training department on proceduresetc. The AP from another supplier had a final written warning, downgraded competence, re‐assessment planned and mentoring.

Thinking Points: Have you seen someone attempt to apply earths without first using their live line tester? 

Should the NP and AP have changed the plan? Why not move the train?

How could the communication been improved on this occasion?

Vehicle Accidents

Considerate Parking and use of Company Vehicles

During 2018 we have received a few complaints from the members of the public regarding parking of our vans, and driver behaviour while on the road. We’ve had dashcam footage sent through regarding littering, as well as pictures of poor or inconsiderate parking.

Please try to be considerate when parking your vans especially when they will be parked for longer than an overnight stay.

Driver Tracing 

We are currently starting to receive correspondence from South Yorkshire Police to say that some of the drivers of the speeding fines we receive cannot be traced at the address we have provided

Vehicle Accidents

You’ll be aware we have a telematics system in our Company and hired vehicles, and we are monitoring closely – not only speeding but also driving behaviour. 

We have taken steps recently to remove the authority to drive from some staff because of their speeding, and also have terminated the employment of someone on probation, due to their unauthorised use of a company vehicle, and due to excessive speeding.

Please treat the Company vehicles with respect.

Environmental

Prevention and Management of liquid spills

Whilst out on sites there are a number of different water sources we may come across:

• Streams

• Rivers

• Lakes

• Coastal Waters

The storage, handling and use of materials and equipment on site pose a potential pollution threat to these water bodies. 

Polluting water is an offence and large fines can be imposed

Its not just oils and fuels that pollute, paint flakes, wood waste and even silty water can cause pollution.

The Risks

• Substances may fall into watercourses

• Substances may be washed into the watercourse (silt)

• Stored oils, fuels and chemicals may spill/leak

Things to consider

• Concrete mixing away from site

• Storage arrangements

• Refuelling 

What can we do?

In the event of an incident you need to:

• Be familiar with the spill response plan/emergency spill arrangements

• Follow all instructions and task briefings

• Know where the pollution control equipment is so as to deal with the spill and be aware of how to use them i.e. Spills kits – absorbent materials/drain blockers ‐ If you don’t know, speak to the supervisor

• If there is an oil spill, try to stop the pollution entering the water and use a spill kit to clean it up. Inform the supervisor immediately.

Quick action can limit the spread of pollution, help restrict environmental damage and reduce clean up
