Optimal application of new devices typically evolves naturallywith time and experience. Yet despite the advantages of rapid de-ployment (or sutureless) valves, including ease of implantation, fastlearning curves and the associated reduction of procedural times, clear-cut indications remain broadly undefined.

As Minimally-Invasive Surgical Aortic Valve Replacement(MISAVR) requires easily implantable devices, rapid deploymentdevices are especially suitable for UHS and RAT videothoracoscopy;in our view, potentially one of the more interesting applications is forredo-MISAVR patients.

Indeed patients undergoing redo-MISAVR due to a failed con-ventional stented, stentless or full root prosthesis, often require lengthyprocedures, with a high risk of injury during debridment especiallyin small calcified annuli or during the removal of degenerated aorticroot stentless bioprostheses. In these challenging cases RDAVR couldprove the difference.

We report herein our first two implants of an EDWARDSINTUITY Elite valve in complex redo-MISAVR.

The first patient was a 55-year-old male with systemic and pul-monary hypertension, HCV chronic infection and persistent AFib.The patient underwent AVR in 2006 for severe AR and a 29 mmToronto SPV (St. Jude Medical, Minneapolis, MN) bioprosthesis wasimplanted. In 2015, the patient underwent a minimally invasive up-per J-resternotomy due to the recurrence of severe AR due to post-endocarditis with perforation of the non-coronary leaflet and a peakpressure gradient (PPG) of 37 mmHg.

Externally the aortic root was heavily calcified with massivemediastinal adhesions; the valve hada fully calcified root, and severe

fibrosis with a perforation of the non-coronary cusp.After leaflet excision, a 23 mm EDWARDSINTUITY Elite valve was easily implanted. Aorticcross clamp and CPB time were 57 and 117 minutesrespectively. The patient was weaned from CPB witha minimal inotropic support. Postoperative TEEshowed no PVLs and low gradients. The patient wasextubated after 5 hours of AMV. Inotropic agents were suspended after6 hours. ICU stay and hospital stay were 2 and 9 days respectively. Notransfusions were needed.

The second patient was a 57-year-old male affected by systemichypertension. In 2009 the patient underwent AVR for severe AR due toaortic root enlargement; at that time a 25 mm homograft was implanted.In 2015, the patient presented with a severe calcific degeneration of thehomograft and underwent a minimally invasive upper J-resternotomywith a 21 mm EDWARDS INTUITY Elite. Aortic cross clamp and CPBtime were 62 and 113 minutes respectively. No inortopic agents wereneeded. AMV time, ICU stay and hospital stay were 3 hours, 1 and 6days respectively.

These two cases demonstrate how the prosthetic choice at the timeof the first and second operation is crucial. The classical surgical ap-proach would have required a redo-Bentall-de Bono procedure with ahigher grade of surgical complexity and risk.

Rapid deployment bioprostheses demonstrated to be safe and suit-able for redo-MISAVR patients, allowing performing fast and efficientreoperation in a safe minimally invasive fashion. In our experience redo-MISAVR has finally become “minimal” for patients and “simpler” forsurgeons.

I am delighted to welcomeour attendees and industrypartners to the vibrant city ofBerlin for our 2015 ISMICSAnnual Meeting. This year’smeeting has a special significancefor its location, with our President,Professor Volkmar Falk, now aBerlin resident in his capacity ashead of the German Heart Centre.ISMICS last met in Berlin in 2010,and over the past 5 years the

society has continued to expand, grow its membership, and co-sponsor an increasing number of courses on an international basis.

That is not to say that this year’s return to Berlin has notbrought some challenges – we are sharing the city and particularlyour hotel with the UEFA Champions League Finals, which has madehousing arrangements over the weekend extremely difficult. Thatsaid, we hope that you and your family and friends enjoy the addi-tional festive atmosphere which the Champions League Finals aregenerating here in the city.

As of last week we had total pre-registration of 427, notincluding our industry partners. Europe has always had very highonsite registration for ISMICS, with the 2010 Berlin meetingsetting records, so we hope to see the high registration figures forwhich ISMICS is known continue to climb. Our membership nowstands at 874 and continues to grow thanks to the efforts of ourSecretary, Professor Grundeman, our Board of Directors, and ourMembership Chair, Dr. Nifong.

ISMICS 2015 is proud to be able to offer our attendees LiveSurgical Cases, something done only once before with the NewYork Meeting in 2005. There will be live Thoracic Presentationson Wednesday during the offsite Thoracic Program at the BerlinerMedizinhistorisches Museum de Charite, and then on Friday therewill be two live Cardiac Presentations transmitted to theInterContinental from the Berlin Heart Center. Both days offer 3Dtransmission of the surgeries.

The expansion of programming to include live surgery is a signof ISMICS’ continuing mission to innovate across all areas ofcardiac, thoracic and cardiovascular surgery. Yet in its efforts toinnovate ISMICS does not forget popular programming aspects,such as our 2-day Poster Competition, the hands-on opportunitiespresented by “SIMCity,” and the ever-popular “Technical Chal-

lenges: Pitfalls and Disasters” session on Wednesday evening. On the“change” side, you will note that this year the INNOVATIONS abstractissue is online only. Despitean incredibly busy schedule with his move to Berlin last fall, Dr. Falkalways found time for ISMICS and overseeing all aspects of theSociety, from Board Meetings, to industry discussions, to scientificprogramming. He joined ISMICS in 1998, only one year after theSociety was established, and has always given tirelessly in so manycapacities. It has been my honor work with him over this past year.

The ISMICS Annual Meeting has always been the forum forpresenting the latest innovations in the field and this year’s meeting will present and reviewnew technologies, techniques, and challenging ideas that are changing the way we practice.There are comprehensive sessions in both cardiovascular and thoracic surgery.

The program begins on Wednesday with a full Masters Day Program. The morningGeneral Session courses will included “How I Do It: Live in a Box” presentations, as well asstate-of-the-art lectures on Hybrid Coronary Revascularization, Establishing a MinimallyInvasive Aortic Surgery Program, and New Innovations in LVAD Therapy. Focused sessionswill follow that delve into more details and provide ample opportunity for discussion anddebate among presenters and the audience.

The morning program also features a Thoracic Masters off site opportunity at theBerliner Medizinhistorisches Museum der Charite where attendees will view two parallel liveoperations on robotic thymectomy and uniportal VATS.

The afternoon sessions will continue with the “How I Do It” presentations and additionalstate-of-the-art lectures, as well as focused sessions on Minimally Invasive Robotic MitralValve Surgery, Arrhythmia, and Transcatheter Mitral.

Wednesday evening will feature the always popular “Technical Challenges: Pitfalls andDisasters” course, chaired by Valavanur A. Subramanian, MD and Ramchandra C. Reddy,MD where fifteen of the most challenging and unusual cases are presented and reviewed bythe audience.

Speaker highlights include the Presidential Address by Volkmar Falk on Thursday, theKit V. Arom Lectureship on “The Evolution of Modern Cardiac Surgery and the Necessity toChange” by Hermann Reichenspurner, MD from University Hospital Eppendorf, Hamburg,Germany, and the Keynote Address which will be presented by Prof. Wendelin Stark from theETH Zurich Institute for Chemical and Bioengineering in Zurich, Switzerland.

Once again this year “SIM City” will feature hands on simulation and training, where youcan actually test and practice various techniques, technologies, and equipment. We encour-age you to stop by and experiment. First round presentations are divided into topicgroups, with several groups presenting concurrently. Topic groupsinclude: Minimal Invasive General; Imaging, Perfusion, andClotting; Assist Devices, Heart Failure and Pediatric; ArrhythmiaSurgery; Coronary Grafting and Conduits; RevascularizationStrategy; Cancer; Thoracic General; Aortic Valve; Mitral; andValve - Transcatheter Therapies. Participants score each presenta-tion in the First Round.

The highest scoring posters in each group are the semi-finalists. In the Final Round, on Friday at 16.30, the semi-finalistsmake their presentations to the entire audience. Topic moderatorsscore the semi-finalists to select the Best Moderated Poster.

I am delighted to welcomeour attendees and industrypartners to the vibrant city ofBerlin for our 2015 ISMICSAnnual Meeting. This year’smeeting has a special significancefor its location, with our President,Professor Volkmar Falk, now aBerlin resident in his capacity ashead of the German Heart Centre.ISMICS last met in Berlin in 2010,and over the past 5 years the

society has continued to expand, grow its membership, and co-sponsor an increasing number of courses on an international basis.

That is not to say that this year’s return to Berlin has notbrought some challenges – we are sharing the city and particularlyour hotel with the UEFA Champions League Finals, which has madehousing arrangements over the weekend extremely difficult. Thatsaid, we hope that you and your family and friends enjoy the addi-tional festive atmosphere which the Champions League Finals aregenerating here in the city.

As of last week we had total pre-registration of 427, notincluding our industry partners. Europe has always had very highonsite registration for ISMICS, with the 2010 Berlin meetingsetting records, so we hope to see the high registration figures forwhich ISMICS is known continue to climb. Our membership nowstands at 874 and continues to grow thanks to the efforts of ourSecretary, Professor Grundeman, our Board of Directors, and ourMembership Chair, Dr. Nifong.

ISMICS 2015 is proud to be able to offer our attendees LiveSurgical Cases, something done only once before with the NewYork Meeting in 2005. There will be live Thoracic Presentationson Wednesday during the offsite Thoracic Program at the BerlinerMedizinhistorisches Museum de Charite, and then on Friday therewill be two live Cardiac Presentations transmitted to theInterContinental from the Berlin Heart Center. Both days offer 3Dtransmission of the surgeries.

The expansion of programming to include live surgery is a signof ISMICS’ continuing mission to innovate across all areas ofcardiac, thoracic and cardiovascular surgery. Yet in its efforts toinnovate ISMICS does not forget popular programming aspects,such as our 2-day Poster Competition, the hands-on opportunitiespresented by “SIMCity,” and the ever-popular “Technical Chal-

Despitean incredibly busy schedule with his move to Berlin last fall, Dr. Falkalways found time for ISMICS and overseeing all aspects of theSociety, from Board Meetings, to industry discussions, to scientificprogramming. He joined ISMICS in 1998, only one year after theSociety was established, and has always given tirelessly in so manycapacities. It has been my honor work with him over this past year.

EDWARDS EDITORIAL: Rapid Deployment with the EDWARDS INTUITY Elite Bioprosthesis: MIS-AVR Reoperations Made Simpler?Tommaso Danesi Hinna, Loris Salvador, San Bortolo Hospital, Vincenza, Italy


Optimal application of new devices typically evolves naturallywith time and experience. Yet despite the advantages of rapid de-ployment (or sutureless) valves, including ease of implantation, fastlearning curves and the associated reduction of procedural times, clear-cut indications remain broadly undefined.

As Minimally-Invasive Surgical Aortic Valve Replacement(MISAVR) requires easily implantable devices, rapid deploymentdevices are especially suitable for UHS and RAT videothoracoscopy;in our view, potentially one of the more interesting applications is forredo-MISAVR patients.

Indeed patients undergoing redo-MISAVR due to a failed con-ventional stented, stentless or full root prosthesis, often require lengthyprocedures, with a high risk of injury during debridment especiallyin small calcified annuli or during the removal of degenerated aorticroot stentless bioprostheses. In these challenging cases RDAVR couldprove the difference.

We report herein our first two implants of an EDWARDSINTUITY Elite valve in complex redo-MISAVR.

The first patient was a 55-year-old male with systemic and pul-monary hypertension, HCV chronic infection and persistent AFib.The patient underwent AVR in 2006 for severe AR and a 29 mmToronto SPV (St. Jude Medical, Minneapolis, MN) bioprosthesis wasimplanted. In 2015, the patient underwent a minimally invasive up-per J-resternotomy due to the recurrence of severe AR due to post-endocarditis with perforation of the non-coronary leaflet and a peakpressure gradient (PPG) of 37 mmHg.

Externally the aortic root was heavily calcified with massivemediastinal adhesions; the valve hada fully calcified root, and severe

fibrosis with a perforation of the non-coronary cusp.After leaflet excision, a 23 mm EDWARDSINTUITY Elite valve was easily implanted. Aorticcross clamp and CPB time were 57 and 117 minutesrespectively. The patient was weaned from CPB witha minimal inotropic support. Postoperative TEEshowed no PVLs and low gradients. The patient wasextubated after 5 hours of AMV. Inotropic agents were suspended after6 hours. ICU stay and hospital stay were 2 and 9 days respectively. Notransfusions were needed.

The second patient was a 57-year-old male affected by systemichypertension. In 2009 the patient underwent AVR for severe AR due toaortic root enlargement; at that time a 25 mm homograft was implanted.In 2015, the patient presented with a severe calcific degeneration of thehomograft and underwent a minimally invasive upper J-resternotomywith a 21 mm EDWARDS INTUITY Elite. Aortic cross clamp and CPBtime were 62 and 113 minutes respectively. No inortopic agents wereneeded. AMV time, ICU stay and hospital stay were 3 hours, 1 and 6days respectively.

These two cases demonstrate how the prosthetic choice at the timeof the first and second operation is crucial. The classical surgical ap-proach would have required a redo-Bentall-de Bono procedure with ahigher grade of surgical complexity and risk.

Rapid deployment bioprostheses demonstrated to be safe and suit-able for redo-MISAVR patients, allowing performing fast and efficientreoperation in a safe minimally invasive fashion. In our experience redo-MISAVR has finally become “minimal” for patients and “simpler” forsurgeons.
