If you don’t make a selection, you’ll automatically be invested in the default investment option, which is our Balanced (MySuper) option.

Step 3 – Sign the form

by signing this Investment Choice form, i understand that:

> the investment choice I have requested will usually be processed on the Wednesday following the week it is received, as described in the Member Guide Product Disclosure Statement and other associated reference material;

> the investment choice requested will apply until further notice from me;

> the Trustee’s administrator will not action my request if the information I’ve provided is incomplete or unclear;

> a switching fee will be deducted from my account if I have chosen to vary the investment options for my current balance. The switching fee is waived for new members (within their first 30 days) and for changes which only affect future contributions and transfers;

> and accept that the illustrations of risk and return shown in the PDS are based on assumptions which may or may not be borne out in practice;

> investment returns are not guaranteed, and can be positive or negative;

> and acknowledge that, if I need assistance in relation to my investment choice, I should seek advice from a licensed financial adviser;

> and acknowledge that I have read and understood the Member Guide Product Disclosure Statement and other associated reference materials.
