Prescribers: How to Order Core/Standard Equipment

Has prescriber registered with MSoft?

Prescriber registers to prescribe equipment from us on website: www.kent.gov.uk

Once on the website prescriber searches for “Msoft Registration” And then clicks on the Msoft registration link

Prescriber complete the online form after clicking the link and fills in their details

Submit your request for registration

Once registered: Prescriber assess their patient for equipment needs

Is the equipment from the “core sections” of the catalogue?

If a need is identified, please check the catalogue for stocked/ available equipment. Catalogue can be found on www.kent.gov.uk

Complete an ICES Request form either electronically via the catalogue and send to [email protected] or on paper and fax to 01622 713011

Is the order for a bed?

Order will be placed on MSoft and an email confirmation sent to the email address given at registration. Confirmation will also be emailed when items delivered, if items not delivered for any reason other than the client was not home, the ICES Office staff will also call to give you the failed delivery reason.

Ordering Special Non Catalogued Items from ICES

A client is assessed and after checking in the catalogue it is discovered that the item required is not provided by ICES

Prescriber to decide what is required for the patient

Prescriber completes the special order form giving ICES details of supplier and a quote where possible.

Order is sent to ICES via email or fax: [email protected]

Order received by ICES and is passed on manager on duty

If clinical input is required, or no manager is on duty, the order will be forward to clinical lead for authorisation

Ordering Beds from ICES

No

Prescriber needs to have registered with Msoft to order a Bed. Prescriber identifies a patient that may require a profiling bed

Prescriber checks the client meets the criteria for provision of a profiling bed by ICES. Prescriber locates the Bed form in the catalogue

Prescriber completes bed assessment form, including the clients name, address, clinical reasoning and identifies level of urgencey (EOL, hospital discharge, risk matrix score should be completed for long term requests

Prescriber sends the completed form to ICES via fax or email. [email protected]. Prioritised accordingly by ICES

ICES receive request, prioritise according to category of request.

ICES can deliver in 48 Hours

ICES Deliver Bed within 48 Hours Once booked in by the office, prescriber will receive an email confirmation from Msoft.

ICES confirms likely length of loan with prescriber. Will require information in writing if this has not been stated on the assessment form. ICES will rent a bed for same/next day delivery (dependant on time of order)

Is the loan for Hospital Discharge?

Is the loan for a terminally ill client with an expected length of loan being between 2 weeks and 6 months?

Order will be prioritised according to on discharge date. Where at all possible we will deliver before/ on the discharge date. It is the prescriber’s responsibility to inform ICES if any thing changes with the client. i.e likely length of loan; discharge date; order no longer required

ICES aim to Deliver Bed within 7 working days or by date planned for discharge Once booked in by the office, prescriber will receive an email confirmation from Msoft.

Is the length of loan expected to be more than 6 months?

Order is a will be prioritised dependant in date received order. We will deliver as soon as we have a spare delivery slot. It is the prescriber’s responsibility to inform ICES of any relevant changes. i.e likely length of loan; change in client condition; order to be cancelled

ICES aim to Deliver Bed between 7-14 working days Once booked in by the office, prescriber will receive an email confirmation from Msoft.

Order will be prioritised dependant on the risk scoring and then by date ICES received the order. We will deliver as soon as we have a spare delivery slot. It is the prescriber’s responsibility to inform ICES if any thing changes with the client. i.e length of loan/discharge date/order to be cancelled

There is usually a waiting list for long term requests as other categories have priority. Once booked in by the office, prescriber will receive an email confirmation from Msoft.

Using Hospital Beds at Home in a Room Where There are Gas Fires In order to ensure that the patient in safe from the risk of carbon monoxide poisoning, if there is a gas fire or other gas appliance in the room, the fire must not be used when the patient is in the room until the gas appliance has been tested for safety. If heating is necessary an alternative must be found e.g an electric fire. (N.B. this includes central heating boilers and cookers). A registered Gas Safe Register engineer must carry out the check. If you live in rented accommodation your landlord must carry out this check once a year, so all you have to do to check with your landlord to ensure that the checks have been done. If you own your home you will need to get the fire checked yourself. A list of Gas Safe Register Engineers is given in the “yellow pages” (under Gas) or on the internet. Carbon Monoxide is an invisible, tasteless gas, which kills, even in low concentrations. We therefore recommend that a carbon monoxide detector be fitted in any room where gas appliances are used. These are available from DIY shops. I have read the warning about using gas appliances in the room where patients are sleeping and I have been given a copy of this form and I understand it

1. I do not have a gas fire in the room in which the bed is to be used. 2. I will not use the gas appliance until a Gas Safe gas engineer has it tested

it. 3. I will arrange for the gas appliance to be tested by a registered Gas Safe

Criteria for Profiling Beds The provision of profiling beds in Kent Community Health is dependant on clinical need and unacceptable manual handling risk to staff, patient or carer. In order for a profiling bed to be issued the patient must fulfil the following criteria.

1. Clinical Need & 2. (a) Manual Handling or (b) Independence & 3. Demonstration of ongoing need & 4. Conditions for the loan of Equipment

Section 1: Clinical Need Criteria 1. Patient fulfils criteria for continuing care from the NHS 2. Patient has a clinical condition that comprises their airway or affects

breathing or swallowing where it is necessary to maintain an upright-seated position in bed and where other forms of management have been considered and are inadequate or inappropriate.

3. Patients clinical condition makes it necessary for them to change position

in bed or to maintain a position in bed (i.e pressure ulcers, acute/chronic pain, muscle spasm), where other forms of management have been considered and are inadequate or inappropriate.

4. Patients clinical condition or special needs result in them spending a major

part or all of their time in bed and where other forms of management have been considered and are not appropriate, acceptable or adequate for patient to adequately maintain their activities of daily living.

Evidence 1. Assessor to complete relevant sections from the assessment for profiling

bed and use multidisciplinary approach if appropriate. 2. Assessor to document other forms of ‘management’ considered/tried e.g

medication, pressure relieving equipment. 3. If the request for a variable posture bed is part of a rehabilitation

programme – assessment from a physiotherapist/occupational therapist will be required.

4. Indication of whether bed is for short term or long term use if possible,

(short term is less than 12 weeks)

1. The patient’s inability to change or maintain position in the bed, or get into or out of bed independently poses a manual handling risk to their care giver(s). Other handling methods or equipment fail to adequately reduce the risk of injury to the patient of those who assist him/her.

Section 2 (b) Independence and Maintenance Criteria 1. Use of bed to change or maintain position and or get in to or out of bed will

facilitate the independence of the client and form part of a management programme, where other forms of management have been considered but are not acceptable or appropriate.

Evidence 1. Where necessary the assessor should use a multidisciplinary approach. 2. The assessor should undertake a risk assessment of all the tasks involving

the patient in bed including:

Moving up and down the bed Sitting up/Lying down in bed Maintaining optimum position within the bed Getting in and out of bed

3. Risk assessment should be consider the needs and abilities of the patient, all

individuals that will be using the bed, the environment and any existing equipment.

4. Assessor should complete relevant sections of equipment checklist form 5. Indication of long or short term need for bed (short term less than 12 weeks) 6. Therapist report may be required (2b)

1. The assessor must work with the patient and their carers to establish and demonstrate how the profiling bed requested will be used to resolve the difficulties identified within the assessment. This will be achieved by the completion of a goal sheet and will record baseline (initial stating point prior to bed provision and objective (the desired outcome)

2. Goals should be agreed with the patient and their carer and should be

specific, measurable, achievable, relevant and timed. Goals should be reviewed on a prearranged date (6 weeks after issue, 12 weeks after issue and then annually, unless situation changes) to ensure that the profiling bed meets/continues to meet the needs of the patient, their carers and staff.

3. It should be made clear to patients and the carers that the bed may be

refused or withdrawn if it does not meet the identified needs. 4. Where patients or their carers are to operate the bed independently they

must be given adequate training which should include:

The function of the bed and the safety details How and in what circumstances the bed is to be used (along side other equipment and techniques) to achieve the goals set. Patients and their carers should be given contact telephone numbers to be used in events of any problems and should be informed of maintenance arrangements Written Information about bed functions, features and safety issues as well as a contact number in case of any problems should be kept at patients house.

5. At the initial review the assessor should confirm the likely duration of loan Evidence 1. Goal Sheet completed 2. Patient and or their carer to sign agreement form for goal set. 3. Evidence of review on date set or sooner if circumstances change.

Section 4 Agreement of Conditions of Loan Criteria 1. The patient and their carer must accept condition of loan, which should be

discussed with them by the assessor. 2. Equipment is for named person only. Further authorisation is to be

obtained if it is to be used far any other patient. 3. In the event of the equipment no longer being required the supplier should

be notified no later than within two working days. 4. If the equipment has been provided on a hire basis for a limited period,

authorisation must be obtained from the supplier if this period id to be extended (by telephone if necessary). Kent Community Health reserves the right to request subsequent confirmation in writing that there is a continuing need for the equipment.

5. The cost of damage or repairs to the equipment to be made by

client/relative unless otherwise agreed by Kent Community Health. 6. Kent Community Health reserves the right to undertake spot checks to

ensure equipment is in use and still required for the purpose originally intended.

INTERGRATED COMMUNITY EQUIPMENT SERVICE

CRITERIA FOR PROVISION OF EQUIPMENT TO NURSING/RESIDENTIAL HOMES Care Homes have a responsibility to provide equipment necessary to meet the homes stated purpose and the individually assessed needs of the service user (DH National Minimum Care Standards, 2002. Standard 29.1). The following criteria aim to clearly identify the circumstances in which Kent Community Health NHS Trust (KCHT) will provide equipment to Care Homes. 1. The Community Loan Store will loan a piece of equipment to care homes

(residential) for short term use for the following reasons:

A resident has been assessed as having entered the last few weeks of their life. Treatment of pressure ulcers (it is the responsibility of the care home to provide

equipment for prevention of damage to skin integrity) An acute exacerbation of a chronic illness A short period of reduced mobility attributed to an acute health issue (i.e chest

infection, urinary tract infection etc)

2. The Community Loan Store would not expect to loan equipment to nursing homes. In exceptional cases where the request is to support hospital discharge a short term loan (max 6 weeks) may be appropriate. Each case would be considered on its individual merit.

NB Walking Aids may be loaned on a long term basis

2. Any agreed loan of equipment will be for an initial period of 6 weeks. Equipment

will only be provided following assessment and request from a KCHT Nurse/Therapist.

3. The KCHT prescriber will complete a review at the end of this period and re apply

the above criteria to see if they still apply. 4. If further loan period is required the Community Loan Store will be informed and

provided with rationale. However if the equipment is to meet a long term need the care home would be expected to provide as part of their fit for purpose requirement.

5. Should the equipment no longer be required due to the resolution of the initial

acute episode, the KCHT prescriber will provide the home with advice about equipment which may be required for longer term use.

6. If the equipment is no longer the responsibility of KCHT to provide, the

Community Loan Store will write to the care home arranging to collect the equipment within 2 weeks, allowing the Care Home sufficient time to arrange alternative provision.
