Instructions to complete a Work Request (AF IMT 332)

What is a 332?

332’s are typically submitted for repairs or corrections requiring more than 50 man hours or of high cost

(i.e. sprinkler/suppression systems, fire alarm systems, or the changing of walkways and doorways).

They are also utilized for facility renovations and upgrades desired by the occupant. Most fire safety

write-ups not meeting the above criteria only require a service order over the phone, and not 332; but

Customer Service may still ask for one. Our office is happy to assist you with fire safety related 332’s,

and the below instructions are provided as a tool for those forging ahead on your own; as we want to

ensure that all required information is included to prevent technicalities that may slow the process.
