LEARNING OBJECTIVE AND OUTCOME

OBJECTIVE:1. GAIN KNOWLEDGE ON VARIOUS TYPES OF

HELICOPTER INCIDENTS AND ACTIONS TO BE TAKEN AFTER SUCH INCIDENTS OCCURS.

OUTCOME:1. EXPLAIN THE MAINTENANCE ACTIONS TO BE

TAKEN AFTER A HELICOPTER IS INVOLVED IN AN INCIDENT.

INTRODUCTION

IN THIS LESSON WE WILL DISCUSS THE INSPECTIONS TO BE CARRIED OUT AFTER A HELICOPTER IS INVOLVED IN ANY OF THE FOLLOWING INCIDENTS;

1. Rotor over-speeds2. Gearboxes and engine metal contamination3. Fuel contamination

ROTOR OVERSPEEDINGThe extent of the inspection will normally depend on the degree of over speeding.Over speeding below a specified limit will usually entail checking the rotor blades for distortion and damage and the rotor head for cracks and smooth operation, but, if this limit is exceeded it is usually specified that both the main rotor head and tail rotor head should be removed for overhaul.

ROTOR OVERSPEEDINGIf damage has occurred to the main rotor blades, the rotor head, shaft, pitch control rods, tail rotor and transmission should also be removed for overhaul and the gearbox attachments should be inspected for damage.THE FOLLOWING FEW SLIDES ARE THE INSPECTION REQUIREMENT ON AS355 FOLLOWING THE MAIN ROTOR OVER SPEED.

MAIN GEARBOX & ENGINE OIL CONTAMINATION

MAIN GEARBOX & ENGINE OIL CONTAMINATION IS DEFINED AS THE PRESENCE OF METAL PARTICLES ON THE MGB/ENGINE CHIP DETECTORS.

ENGINE OIL CONTAMINATIONReference must be made to engine maintenance manual to determine whether the amount of metal particles found is within the limits or not.Outside the limit means the engine has to be returned to manufacturer for repair.If the metal found is within the limit, then the filters, strainers has to be removed & inspected. Ensure no excessive metals are found.Engine oil need to be replaced & fresh oil put in.Do the ground run & monitor the light.Do the monitoring within the specified period.

FUEL CONTAMINATIONThe fuel tank must be completely drained & the internal of the tank be thoroughly inspected for corrosion caused by the presence of water in the tank.All the components inside the tank must be inspected for corrosion.All traces of corrosion must be removed.Both airframe & engine fuel filters must be removed & inspected.

FUEL CONTAMINATIONAll associated pipelines inspected & flushed.All affected engine fuel system components especially the fuel control unit be inspected for corrosion.If in doubt the affected components must be replaced.The source of water entry into the fuel system must be investigated & rectified.
