Epilepsy: a brain disorder in which a person has repeated seizures over time.
Seizures: episodes of disturbed brain activity that causes changes in behavior Symptoms of seizures can range in individuals from
starring spells to violent convulsions to loss of alertness
Types of seizures Absence (petit mal) Generalized tonic-clonic (grand mal) Partial (focal)
Seizures typically begin between ages 5 and 20, but can occur at any age

200,000 new cases of epilepsy are diagnosed each year.
Prevalence of active epilepsy is estimated as nearly 3 million in the United States.
Incidence is highest in individuals under the age of 2 and over 65.
326,000 children through age 15 have epilepsy. More than 300,000 persons over the age of 65
have epilepsy. Males are slightly more likely to develop
epilepsy than females.

+Pathophysiology of Epilepsy
The seizures result from excessive neuronal discharges in the brain
Seizures may be due to medical conditions, injury to the brain, or idiopathic (unknown)

Anticonvulsants can be taken to prevent seizures Surgery
First line of treatment for seizures due to tumors, abnormal blood vessels, bleeding of the brain
Often used if medications don’t work Ketogenic diet
This diet therapy is not a first-line treatment for epilepsy, it is primarily used for children whose seizures are not well controlled by available medications.
In patients with seizures that result from metabolic disorders, such as glucose transporter type 1 (GLUT-1) deficiency or pyruvate dehydrogenase deficiency the ketogenic diet may be considered first line therapy
The diet is not appropriate for patients that have metabolic disorders such as: organic acidurias, lipid transport, or storage disorders and pyruvate carboxulate deficiency because a low-carb, high-fat diet would be harmful

+History of the Ketogenic Diet

The Bible
1924- diet was officially discovered by Dr. Peterman at the Mayo Clinic
Created a diet that stimulated a fasting state
1940- decrease in diet popularity due to increasing amount of medications available
1994- new increase in popularity of the diet, when Jim Abrahams, a movie producer, was searching for alternative treatment for his son’s, Charlie, uncontrollable seizures.
The family started the Charlie foundation to create awareness about the ketogenic diet

+Rationale for the Ketogenic Diet
Starvation has been shown to result in decreased seizure activity, however the mechanism is poor understood
Starvation causes the body to go into acidiosis, ketosis, dehydration, and hypoclycemia
Thus, the aim of the ketogenic diet is to induce the symptoms of starvation such as ketosis, which can done by restricting carbohydrate intake
May be due to changes in neuronal metabolism, whereby the ketone bodies behave as inhibitory neurotransmitters, thus having an anticonvulsant effect

The diet is usually initiated gradually in a hospital setting over a 3-4 day period During the initiation phase blood sugar and ketone
levels are monitored. Recent studies suggest that it is not necessary to have

+Efficacy of Treatment
Compliance to the diet is easiest for pre-school aged children and becomes more difficult as children get older
Children also have a greater capacity to extract and oxidize ketones
Many studies have been done on the efficacy of the classic Ketogenic diet, which suggest:
50% of children will experience at least 50% reduction in seizures on the KD
one third may achieve a 90% seizure reduction
10% will eventually become seizure-free and even medication free

+Efficacy of Treatment
Modified Atkins Diet Preliminary evidence from prospective studies showed that
the diet is effective in improving seizure control in 45% of the patients who have trialed it.
Further pediatric and adult studies are underway.
Low Glycemic Index Diet (after 6 months of diet) 34% had a >90% reduction

Epilepsy is a brain disorder in which a person has repeated seizures over time.
A Ketogenic diet can be used to controlled seizures in children who’s seizures are not well controlled by available medications.
The ketogenic diet is a high fat, very low carbohydrate diet that puts the body into ketosis, which has been shown to reduce the incidence of seizures
The diet is not without risk and is primarily used as a last resort, since it is difficult to follow and has many adverse side effects
