Project AimReduce hospital admissions from nursing homes by enhancing local skills in identifying and managing risks ofmalnutrition and dysphagia. 

Objectives Implement accurate ‘MUST’ screening and appropriate

use of care plans  Improve the identification of dysphagia and use of safer

swallowing strategies
