When completing the health history on your child, please describe the condition, its signs & symptoms, and any re-

strictions or assistance needed. Submit physician documentation to the School Nurse if applicable.

All nonprescription medication must be in the original bottle/package, with the child’s name clearly marked on the con-

ATTENDANCE PROCEDURES AND PRACTICES

Attendance monumentally impacts student academic success! Wisconsin law requires students to attend school regularly during all scheduled school hours, with the exceptions of certain religious holidays. When a student is sick, parents or guardians are to call the Attendance Office at 262-359-8500 the same day. If after school hours, please leave a message at the same number.

In conjunction with the citywide truancy ordinance, it is our commitment at Indian Trail High School and Acade-my to work diligently with parents and guardians to promote consistent and committed school attendance. Please remember that a student cannot be excused more than TEN TIMES per school year without written documentation from a physician.

DEFINITION OF ABSENCE – KUSD BOARD OF EDUCA-TION POLICY

EXCUSED ABSENCES – A student's absence may be ex-cused by the principal/designee for acceptable reasons such as personal illness, illness in the immediate family, or death of a family member or relative or other rea-sons of legitimate health or educational benefit to the student.

TRUANCY – “Truancy” means any absence of part or all of one or more days from school during which a school designee has not been notified of the legal cause of such absence by the parent/guardian of the absent stu-dent.

EARLY DISMISSAL – Students who, for good reason, wish to be excused from school for part of the morning or afternoon session are required to bring a request, written and signed by the parent/guardian, to the AT-TENDANCE OFFICE.

PRE-ARRANGED ABSENCE – If a student is going to be absent all day or more than one day for reasons other than illness (vacation, college trip, etc.), a request should be made one week in advance. A parent written note specifying dates and reason for the absence must be submitted and approved by individual teachers and entered through the Attendance Office.

FREQUENTLY ASKED ATTENDANCE QUESTIONS

When my child is absent, what should I do? Call 262-359-8500 any time within 24 hours of the ab-sence. If you must leave a message, please identi-fy yourself by name and then be sure to include the student's name, grade, ID number, and rea-son for their missing school. Absences not cleared by a parent/guardian within five school days will not be excused. If at anytime a student comes late to school, he/she MUST sign in at the Attend-ance Office whether or not a call has been previ-ously placed.

What should I do if I receive a call regarding my child

being marked truant or tardy for a period or pe-riods during the school day? Call 359-8612 with-in 24 hours of receiving that call so that the truan-cy can be addressed or if it is an attendance error it can be corrected. Parents, guardians and stu-dents have a five day window to clear truancies and tardies or they will not be excused.

What should my child do if he/she becomes ill at school? Students must go to the nurse either dur-ing passing time or with a pass from their class-room teacher. The nurse will contact a guardian, verify permission to go home, and send the stu-dent to the attendance office for proper sign-out. Students who are on release or student partner-ship are expected to follow the same procedure or the absence will not be excused.

What does my child do if he/she has an appointment to leave during the day? The student must bring a note from his/her parent/guardian to the Attendance Office before school begins on the day of the appointment to receive a pass to sign out. The pass will be dated and used as verifica-tion to the teacher who’s classroom your child is leaving at the appointed time. Your child should then go to the Attendance Office for proper sign-out before leaving the building. A timely return is expected with the student signing back in at the Attendance Office unless otherwise specified.

ATTENDANCE PROCEDURES AND QUESTIONS CALL 262-359-8500 OR 262-359-8612

INDIAN TRAIL NEWSLETTER-REGISTRATION 2014 PAGE 9

NOTE: State law does not allow the school to ex-cuse students for job interviews or any appoint-ments that are not specific to that student. With-out a note the guardian must come into the build-ing to sign out their student, they must be pre-pared to present ID. In order to pick up a stu-dent, the person must be listed on the student Emergency Contact List.

What happens if my child is tardy to school? Students

who do not make it school on time need to report to the tardy sign in area. If for some reason the student is being excused for their tardiness the parent or guardian should call 359-8500 to report the reason. Repeat occurrences will lead to addi-tional follow-up, including potential counseling, parent meetings, interventions, or disciplinary ac-tion through the County Truancy Ordinance.

Is the attendance expectation the same for athletic

participation? Athletes must be in school all day beginning at 7:30am for all blocks in order to prac-tice or compete. The only exception is a verified medical/dental/court/etc. appointment. Unless the appointment is related to an injury from the previous evening's practice or event, the student must get the prearranged absence form and follow school policy for signing in and out of school.

I received a letter in the mail and it said my child is

excessively excused, what does this mean and who do I contact regarding this letter? Per state law, students can have only 10 excused absences (either partial or all day) per school year. Students are marked “excused” for any medical/dental ap-pointments as long as the parent calls in or writes

a note. A student’s absence can be marked as “Medical” if the student brings in a note from the doctor/dentist. “Medical” absences DO NOT count towards the maximum ten excused absences per year. It is in the student’s best interest to obtain a medical note each time they have a medical ap-pointment so they are not placed on the excessive excused list. Contact the attendance dean at 359-8612 regarding any questions.

I want to take my son or daughter on a college visit,

what are the procedures? Have your child pick up a prearranged absence form from the attendance office and fill it out with the date and times of the college visit. Have them take it and get it signed by their teachers and turn it into the attendance office. Upon their return from the college visit please bring some material indicating the student had been to the college so the college visit can be verified and then the absence can be coded appropriately. A prearranged absence form can be found at http://www.kusd.edu/sites/default/files/document-library/english/pre-arranged-absence_1.pdf

We have a family vacation planned during the school

year what are the procedures? Please if at all pos-sible, do not plan vacations during the scheduled school year. If a vacation is necessary during the school year a prearranged absence form needs to be filled out and signed by all teachers and the prin-cipal one week prior to the absence. A prearranged absence form can be obtained from the attendance office or the KUSD website at http://www.kusd.edu/sites/default/files/document-library/english/pre-arranged-absence_1.pdf

INDIAN TRAIL IS A PBIS SCHOOL OF MERIT Through the Positive Behavior Intervention System, Hawk Habits have been established to create a positive and safe learning environment. All members of our learning community (students, staff, parents, and visitors) are asked to be RESPECTFUL, RESPONSIBLE, and SAFE during the school day and at school-related events (sporting games, concerts, plays, etc.). These expectations are in the student handbook and post-ed throughout the building as visual reminders. At Indian Trail we are focused on creating a safe environment free from distractions and disruptive behaviors that detract from learning and cast a negative image of our school. We see education as a part-nership between all members of the learning community. We ask for your support in maintaining that positive culture at Indian Trail. Welcome to a new school year! IT Staff

Student handbooks will be distributed

the first week of school. Important school infor-mation is easily retrieved in the handbook.

Student leadership and active involve-

ment with clubs and activities not only creates lasting memories, but students also learn life skills through meaningful experiences that cannot be taught in a classroom or learned in a book. Further-more, we take great pride in the rich diversity of our student body and of the those that work here. Our diversity programs bring people together for culture understanding, clarity of a variety of view-points, and increased advantage for leveraging different skills.

Students should get involved in

an extracurricular activity. Activities help make a big school feel smaller. Indian Trail offers a wide range of clubs, activities and sports team. Being present on time daily is a critical behavior that deeply im-pacts academic success.

SCHOOL SUPPLY LIST A copy of the suggested school supply list is availa-ble at indiantrail.kusd.edu.

You know your Senior is awesome, and you are so proud of their accomplishments. Tell them how great

they are by purchasing a senior ad in the Blaze yearbook.

Use an ad to congratulate your graduating senior or even acknowledge a special academic or athletic

achievement during the school year. An ad in the yearbook allows you to recognize your Hawk with

photos and words. While the yearbook lets your student remember all the special people and events from

the school year, the personal touch of an ad will make the book that much more special for them.

Senior ads are available in a variety of sizes and prices. Ads are easy to order. Buy online at

yearbookforever.com.

If you have any questions or would like more information, please feel free to contact the yearbook staff at

(262) 359-8700 weekdays between 8 a.m. and 4 p.m. or [email protected] at any time.

All seniors are responsible for submitting their portraits to the school main office by Friday,

January 16, 2015. NO EXCEPTIONS. It is recommended that you contact your photographer

several times if they are going to submit your portrait to make sure it has been received. Many

emails are blocked by the school’s server and are never received, so it is important that you

obtain verification. If you are having Lifetouch/Prestige take your portraits, they will submit

your photos for you.

Portraits will be formatted vertically, so we recommend that you choose one that will best fit that

frame. At Indian Trail you are free to submit the portrait of your choice, but it must be one that

shows your face the best. No hats (except with a military uniform) or other people are allowed.

Senior picture formatting requirements:

1. Photo must be vertical and at least 3MB!

2. Must be a head and shoulder shot only! No hands or arms!

3. No other people, pets, or props 4. Must be a standard blue or gray background

5. No hats other than with a military uniform (KMA students)

6. Photos can be submitted on a flash drive labeled with the students first

and last name or emailed to [email protected] (ID photos

will be used when no other photo is submitted.)

All photos are subject to editing based on content and appropriateness

The deadline for submitting photos is January 16, 2015.

Photos will NOT be accepted after this deadline for any reason due to

strict printing schedules!
