Diabetes is increasing in epidemic proportions worldwide. In India alone, more than 77 million people have Diabetes. The need of the hour is to empower Health Care providers with Basic and Advanced Diabetes Care expertise for better management of Diabetes and its associated complications.

The Chellaram Diabetes Institute (CDI), well known for its commitment towards the fight against Diabetes, has been organising International Diabetes Summits every year since 2017 which were a huge success. Case study and management optionsA 58 year old post-menopausal lady comes with diabetes for 12 years. She is on lifestyle modi�cations, metformin1000 mg twice a day and glimepiride 3 mg per day. She has had one episode of urinary infection 10 years ago anda history of an mild episode of genital mycotic infection 3 years ago. There is a family history of coronary arterydisease and CKD in father and CAD in brother. On examination the BMI is 30 kg/m2 and rest of examination isnormal. The FBS is 153 mg/dl, the Post-breakfast value is 212 mg/dl, Hba1c is 8% and UACR is positive formicroalbumin. The rest of lab investigations, including CBC, liver function, renal function, UACR and podiatryassessment are normal. The stress test is normal and retinal examination shows background retinopathy. Whatwould be the choice of medicines?

Case study and management options

A 65 year old gentleman is admitted to hospital with breathlessness and fever. The CXR shows features ofconsolidation in the left lower lobe. COVID-19 RT PCR test is negative. In view of leucocytosis he has been startedon parenteral antibiotics. Urine ketones are 2+ The HbA1c is 9%. He has not been on any anti-diabetes therapies.He is able to tolerate soft feeds, but has required supplemental oxygen.
