|||| INDUSTRIAL HYGIENE LABORATORY SERVICES Bureau Veritas’ network for industrial hygiene (IH) laboratory services has has an excellent long-standing reputation within the field of occupational health. For over 55 years, we have provided analytical support with two of our laboratories being among the first facilities accredited by the American Industrial Hygiene Association (AIHA) when the program began in 1974.

Bureau Veritas uses state-of-the art instrumentation and technologies to support all types of industries throughout the world. In addition to the mining and pharmaceutical sectors, our clients include construction materials, petroleum, chemical, automotive, manufacturing, energy, as well as contracts with independent consultants and the public sector. Bureau Veritas has been the contract laboratory for the National Institute for Occupational Safety and Health (NIOSH) since 2006, keeping us at the forefront of sampling and analytical issues. This wealth of experience means that our staff members are familiar with any industrial hygiene scenario that you may encounter.

ACCREDITATION Our laboratories maintain many national, state and provincial accreditations and testing categories are accredited through the AIHA’s Laboratory Accreditation Program (AIHA-LAP, LLC). Visit our accreditations on page 5 for more information. Our extensive experience includes developing industrial hygiene sampling and analytical methods at a clients’ request. Many of these methods are proprietary, while others have been shared in periodicals such as the Journal of Occupational and Environmental Hygiene (JOEH).

TECHNICAL EXPERIENCE Our greatest strength is our people. We offer unsurpassed technical expertise through our dedicated team of industrial hygiene experts. Our team is comprised of:

- Three Certified Industrial Hygienists (CIH) and chemists with advanced degrees at our IH laboratories, who all specialize in sampling and analytical chemistry.

- A professional geologist (P.G.) who manages our asbestos laboratory - A Ph.D. mycologist who manages our microbiology laboratory - A dedicated project manager, assigned to each client

The dedicated project manager approach is beneficial because it provides you with a point of contact that understands the intricacies of your industrial hygiene program and is responsible for all aspects of your account. This is even more valuable when you have multiple sites that require a single point of contact to maintain consistency across multiple projects according to the structure of the overall program.

These factors, together with our expertise and service, have solidified strong long-term partnerships with clients that extend over many years.

|||| SAMPLING MEDIA All media supplies are provided free of charge, except where noted by , providing that either the samples or unused supplies are returned within 60 days for US locations and 90 days for international locations. Unused media can be returned for credit on a case-by-case basis. Media that is not returned, damaged, or expired will be invoiced at the replacement cost after that time. Media can be ordered via toll free numbers on page 5 or on our website at orders.bvlabs.com/order-media/.

Sampling media is shipped at no extra charge within the contiguous United States. Overnight and international deliveries are invoiced at cost to the client. Any additional fees (hazardous materials), customs, and/or import fees are the responsibility of the client.

Media and shipping charges are invoiced when the order is placed, separate from analyses.

|||| ANALYTICAL REPORTS Our reports are transmitted to clients, via email, as a PDF image file (using Adobe Acrobat). We will e-mail reports to a regional or corporate contact, as directed. Please note that we do not mail hardcopies of reports unless specifically arranged in advance.

|||| ELECTRONIC DATA DELIVERABLES (EDD) Bureau Veritas has expertise in providing electronic data deliverables in both commercially available and customized formats. Formats commonly used for upload into a customer database include Excel, ASCII (both fixed-length and delimited) and XML files. A map of the standard output file can be supplied for formatting purposes. Consultation with our LIMS Administrator is available to discuss your requirements, document file specifications, and development of a customized file. Please contact the laboratory for further details.

|||| TURNAROUND TIME Standard turnaround time is five working days. Turnaround for asbestos and microbiological samples is tailored to meet project demands. Specialized analyses such as pharmaceuticals, mercaptans, and amines may require a longer turnaround time.

Expedited services can be arranged with prior notification. A surcharge is normally added for expedited service. Please contact the laboratory to discuss your project needs in advance.

|||| BLANKS Analysis of each client field blank is charged at the same rate as sample analysis. Blanks with multiple compounds are invoiced for the first analyte only.

|||| QUANTITY DISCOUNT SCHEDULE The following discount structure applies to published prices and is applicable to submissions of 15 or more identical analysis billed as a single batch.

If special pricing has already been negotiated, additional batch discounts may not be applicable. Contact your laboratory representative to discuss.

|||| PAYMENT TERMS Payment terms are net 30 days. Bureau Veritas accepts Visa, MasterCard, and American Express or will invoice against a purchase order. Prepayment may be required based upon credit or payment history. Prices are subject to change without notice.

|||| SAMPLING EQUIPMENT Bureau Veritas provides industrial hygiene sampling pumps and calibrators, IAQ equipment, aerosol and particle counters, stack emission equipment and other sampling instruments for a nominal fee. Pumps are available as low as $6/day. There are no hidden fees with our equipment rental program. A complete list of our equipment inventory is available upon request.

|||| USE OF THIS SCHEDULE All prices are in U.S. Dollars (USD). The "Fee" column is the price for the first analyte (chemical) on a sample and "Each Addl." column is the price for each additional analyte (chemical) on the same sample. The matrix is air, unless otherwise noted.

To obtain a quotation for a rush request, please contact the laboratory in advance to schedule the project. The common industrial hygiene scan packages are defined on pages 29 to 34.

Prices are provided by turnaround time for microscopy (asbestos and fibers). Please contact the appropriate laboratory in advance to schedule rush requests.

Prices for active pharmaceutical ingredients (APIs) can be found in the Bureau Veritas Pharmaceutical Services Fee Schedule.

|||| INDUSTRIAL HYGIENE SPECIALTY SERVICES METHOD DEVELOPMENT AND VALIDATION Bureau Veritas provides method development/validation services for compounds for which sampling and analysis protocols are not available in the public domain. This service is available for active pharmaceutical ingredients (API), proprietary chemicals, and intermediates, as well as final products in the client pipeline. All projects follow industry-accepted method development and validation protocols. Our protocol can be customized to meet your requirements. The available elements of our method development/validation projects are listed below. As a starting point, we require an Occupational Exposure Limit (OEL), or other defined control limit/range, Safety Data Sheet (SDS), as well as reference material.

- Sampling media selection - Selection of appropriate analytical technique - Demonstration of selectivity, sensitivity, precision and resolution - Determination of method linearity, Limit of Quantitation (LOQ), accuracy, and precision - Demonstration of analytical method ruggedness and robustness - Extraction efficiency from the media - Retention during sampling (sampling efficiency) - Storage stability of collected samples relative to time and storage conditions - Stability of extracted samples

At the completion of each project, we provide a comprehensive validation report and analytical method in NIOSH format. These projects are quoted on a case by case basis.

METHOD TRANSFER Bureau Veritas has the capability to verify the method performance of existing sampling and analytical methods, according to client requirements, prior to transferring a method.

SURFACE CONTAMINATION Bureau Veritas has successfully adapted industrial hygiene analytical methods to address surface sampling for housekeeping purposes. Analysis can be performed on wipes, swabs, or rinses. These methods are intended to evaluate only the surface sampling and other procedures that differ from air sampling methods. TOXICOLOGY SERVICES Toxicology services are offered independently or in support of Bureau Veritas’ method development/ validation services. Consultation is provided by a board-certified toxicologist. Curriculum vitae are available upon request. Our toxicology services include the following elements:
