Welcome remarksOn behalf of IChemE and the Hazards Australasia 2015 technical committee, I would like to warmly welcome all delegates and invited guests to Brisbane for the second Hazards Australasia, process safety conference.

For IChemE, process safety is at the heart of what we do and you are taking part in this conference because you have recognised that process safety matters.

The impact of process safety incidents cannot be dismissed. The loss of life, economic impact and environmental harm all result in our communities, politicians and regulators demanding the highest standards from our industries. This year’s Hazards Australasia theme is, Share the journey to enhanced safety and organisational reliability. In an effort to prevent process safety incidents from occurring, we must share our lessons learned, no matter what stage of the journey we are on.

I expect that your experience at this conference will leave you with a clear vision of how process safety can be improved in your job and in your region.

Over the next two days, you will have the opportunity to improve your networks and hear from others on the same process safety journey. There are also opportunities to learn from those outside the industry, who will bring to the table a different perspective on safety processes and risk management. Allan Briggs, the crisis guy, has weathered some of Australia’s biggest crisis events. As former sergeant with Victoria Police Media Unit and head of communications at Victoria State Emergency Service he has fronted the media at major incidents from gangland shootings, rail crashes, major floods to Black Saturday. Allan will share his experience and insights on how organisations communicate during a crisis and how you can apply these strategies to prepare and respond in a crisis situation.

Kathy Hirschfeld, non-executive director & former BP refinery manager

"Process Safety: Oil Industry to the Boardroom"

Kathy will talk about some significant events in her career including being incident commander for a full-surface tank fire in Turkey, and reviewing the "Baker Panel Report" as a BP refinery manager. She will discuss how these developed her understanding and knowledge of process safety and how that has translated to the boardroom.

Deepwater safety workshop

Bringing the reality of safety and leadership to life, Susan will lead a theatre-based play of Deepwater Horizon incident to the stage. On 20 April 2010, the Deepwater Horizon drilling rig, operated by Transocean, exploded and eleven men lost their lives. But four months earlier in the North Sea on another Transocean rig, there has been a near miss with eerie similarities. Tragically the lessons from the near miss were not learned. In this hard hitting AKT safety programme we see the dramatic events onboard the Deepwater Horizon on the day of the disaster, as we follow the investigations as they uncover negligence, poor regulation, inadequate maintenance, catastrophic decision making and what the US authorities described as ‘a reckless disregard for safety’.

Late on a Sunday night in April 2006, explosives expert Darren Flanagan received a call that would not only change his life forever but the lives of two other men he had never even met. For five days, the Beaconsfield Miners Todd Russell and Brant Webb had been trapped underground after a minor earthquake had triggered a mine collapse in the small town of Beaconsfield in Tasmania.

Darren's story is one of courage, relentless pressure, self-belief and sheer determination.

Nicknamed "the gun", Darren fired explosives that eventually released Todd and Brant from their tomb, 925m underground.
