We have had 76 accidents this year to date, which is the same as for last year. Although I am naturally disappointed that 76 people have been hurt whilst doing their job, I am pleased that we have only had 21 accidents in Q3 - which is our best ever performance since we started Safety With Attitude in 2010. This included our best ever accident free period of 13 days which shows me what we can achieve and is now the benchmark for Q4.

Our absenteeism continues to fall, with our current average for the year at 2.7% as we continue to work with our partner Bupa. This month we are introducing 2 new procedures to the business;

• Control of Substances Hazardous to Health – aimed at preventing illnesses such as asthma, and dermatitis

• Event Investigation – aimed at ensuring that accidents and incidents are properly investigated so that they are never repeated.

You should be briefed on these Procedures in the next few weeks, so I would really appreciate any feedback.

Four sites (East Ham, Chart Leacon, Crofton and Etches Park) completed their three yearly BT Group sponsored Legal Compliance Audits with 50% less findings and very positive comments from the auditors, particularly around

our HSE maturity and significant improvement in accountability and competency of line managers. My thanks to all those involved at the sites – it shows the progress we have made as a business around Health and Safety. Finally, as always there is a lot going on in our business as we approach the end of the year. It is vitally important that we do not allow these things to cause us to lose focus on HSE. We are continuing to drive for a safer and healthier workplace and as part of our safety journey we will be moving towards SWA Zero - a zero tolerance of accidents. In 2014 we will be rolling out a series of workshops across all staff in the UK to explore and understand how we can all contribute to the goal of eliminating people being hurt in our workplace.

Neasden Depot

Neasden depot was built in the 1800’s but the existing sheds were built in 1937 and have historically been used for maintenance and repair of 1963 C stock. LU Neasden Depot is situated in North West London just off the M1 motorway and A406.

The depot is currently utilised for the repair and maintenance of 58 sub surface S8 trains, provided by BT. The depot also contains a wheel lathe for re-profiling wheel sets. Most of the trains are planned to return to Neasden at the end of diagrams for planned maintenance and can also be out stabled at Watford, Uxbridge, Rickmansworth and Wembley Park sidings.

Bombardier currently support the customer on site with teams of Engineers which consists of reliability and train specialists, Warranty/TST technicians, Mods/Doors technicians and a Commissioning team that takes care of workbank trains being delivered to and from Derby Works, both at Neasden and Ruislip depots, and also the delivery of new S7 trains for the Hammersmith and City and District lines. This is on a 24hr

basis and is supplemented by a materials team and admin team.

Bombardier has a good relationship with the customer, particularly with the London Underground HSE team. There are regular HSE meetings to discuss safety, conduct joint walk rounds and share good practice.

Regular in-process checks, safety tours, discussions and toolbox talks are conducted on site. Recently BERT (Bombardier Error Reduction Tool) has been rolled out and this has had a positive effect on BT staff. The roll out of SWA will enhance this.

Bombardier went over 7 months (roughly 215 days) without any accidents on site from October 2012 to May 2013. This year we have had two minor first aid incidents, both relating to manual handling, and no lost time accidents.

All staff are involved in conducting regular safety tours, discussions and in-process checks. They are also party to monthly safety toolbox talks, with the last one being on hand arm vibration.

Introducing the Behavioural Safety Induction Programme

We are pleased to announce the launch of our new online Behavioural Safety Induction Programme. This programme has been built to support and reinforce all of our behavioural safety programmes we have been delivering over the past 5 years.

This new behavioural safety induction is accessed via the SWA website (which has also been revamped) and has been built to both take a new joiner through our approach to behavioural safety and act as a reminder to existing staff of the importance of the behavioural aspects of safety.

The Behavioural Safety Induction Programme has been built with the help and hard work of a large number of our colleagues within the UK business and I would like to take this opportunity to thank everyone involved in its creation. The Behavioural Safety Induction Programme consists of 7 modules.

Northumberland Park Depot Northumberland Park Depot is the home of the 09 Tube Stock operated on the Victoria Line. It is the only depot on the Victoria Line operating over 16 stations between Brixton and Walthamstow Central. The depot was built in 1967 when the Victoria Line was completed and is located adjacent to Northumberland Park Station (Network Rail).

The Victoria Line consists of 47 8 car trains made up of 2009 tube stock and each train consists of two 4 car units. The stock is not interchangeable and can only be used on the Victoria Line and is a derivation of Bombardier Transport’s MOVIA train.

Following the evening peak the depot is used for all types of maintenance activities from cleaning to heavy component changes. With each train over 130 metres in length and weighing 250 tonnes this has to be carefully managed to ensure reliable and efficient operations while maintaining safety at all times. While the servicing maintenance is performed by London Underground, there are various groups of staff at work on depot.

Northumberland Park is a TSSSA (Technical, Spares and Support Services Agreement), which covers material consumption for maintaining trains, including unplanned material, the provision of technical support staff and has a performance regime for reliability and material availability.

The LU TSSSA covers the provision of technical support to assist the customer in achieving and maintaining train reliability benchmarks and the supply of Train Parts to support the fleet of new Trains.

There is also a large modification project underway by a Bombardier team of around 30 individuals over various shifts.

Because of the close proximity of working between different teams working closely on different tasks and frequent train movements there is a constant and strong focus on communication the need to work to recognised depot principles. There have been recent improvements to

enhance physical defences such as isolation padlocks which are now utilised under locked swipe card control. There is also support for softer processes such as supporting the Train Person In Charge TPIC process with visual unit work advice boards for all roads.

Bombardier have a very good relationship with the London Underground and their HSE team, who meet on a regular basis to discuss safety, conduct joint walk rounds and share good practice.

In conjunction with Neasden, we are currently undergoing the roll-out of Human Error prevention training called BERT, the Bombardier Error Reduction Tool, to try and minimise human error and reduce our own goals.

The purpose of event investigation is;Core to the HSE culture that we desire is the belief that all events are preventable. Therefore when events happen we need:

• To understand why the current control systems did not prevent the event• To ensure proper controls are put in place to prevent similar events• To provide legal defence to satisfy legal requirements• To communicate lessons learnt across our business and to the wider BT community
