Choose “Download PCMH enrollment form” under “Enroll”

First choose “Download PCMHenrollment form”▪ If you plan to pool

for shared savings, download the pooling form next

Have your AHIN user ID and password ready

Choose “APII Portal”

Office lead for practice transformation & for care coordination can be the same or different people

If pooling, complete PCMH pooling form6

Both practices choosing to pool must submit a completed PCMHpooling form listing their pooling partner practice
