Background
• GP initiated at Kerikeri Medical Centre in May 2014• Stakeholders: KKMC, NDHB Kerikeri District Nursing
and TTT PHO.• Systems: funding, technology(Medtech)
confidentiality.• Relationship: building relationships, increased
understanding of roles, better information sharing time management.
• Patients: patient centred, responsive to patients, increased efficiency – reduce duplication
Pre HRCOP
• KKMC often unaware KKDN were seeing patient • Poor understanding of DN role/work in community
> ? timely DN referral• Limited access to patient information/Medtech• Liaison with KKMC staff - time consuming, an
interruption• Difficult to access GP via phone • Joint consultations with KKMC were “ad hoc”• Not patient “friendly”
Post HRCOP
• Improved relationships between stakeholders• KKMC notified DN is home visiting patient • Earlier referrals > quicker resolution for
patients• Documentation - Medtech / DN notes• Easier GP access• Improved patient centred care within team
Future
• Case management reviews of complex patients
• DN to access patients that GP unable to contact
• Extend closer relationships to other health providers
• Patient involvement/reflection
