CVCA HOMECOMING BANQUET WAIVER Student and Guest Guidelines and Appearance Standards

All sophomores, juniors, and seniors are cordially invited to attend the 2018 Homecoming Banquet!

• Quaker Station – 135 South Broadway, Akron 44308 • Saturday, September 29 • 6:00 p.m. – Doors Open, Photo Booths, Music, Appetizers • Tickets – $35 per person • 7:00–10:00 p.m. – Dinner and Entertainment • Age Limit – 21 years old and under

Modesty for girls is a very significant consideration in selecting a stylish formal or dressy dress. Parental cooperation is needed. We understand that modesty is somewhat subjective in nature; however, we have established a description of modesty for girls’ dresses that we believe is in keeping with the spirit of our appearance guidelines. This is our standard and we expect students and guests to know and to adhere to our standard. Ladies: Dresses with the following characteristics do not meet the modesty standard:

• Excessively tight/clingy material • Backless dresses • Low cut dresses, cleavage showing • Hemline or slit more than 3 inches higher than the top of knee • Midriff showing (2 piece dresses, mesh, cut-outs)

Gentlemen: Attire for this event is suit and tie OR dress pants, dress shirt, and tie. Male students and guests are not permitted to wear earrings to this CVCA sponsored event. • Cash or check for the tickets desired • This CVCA Banquet waiver signed by the student and guest and parent/guardian (regardless of age).

Ticket sales will take place Monday through Friday, September 17-21 during lunch periods next to the Welcome Center in the Front Lobby.

Except for gross negligence on the part of Cuyahoga Valley Christian Academy (CVCA) and/or the sponsoring organization, the participant and parent/guardian accept personal financial responsibility for any bodily or personal injury sustained during the activity. Further, the participant and parent/guardian promise to hold harmless CVCA and the sponsoring organization and their respective representatives from any claim and/or injury related to the activity. If a dispute over this agreement or any claim for damages arises, the participant and parent/guardian agree to resolve the matter through a mutually acceptable arbitration process. If the parties fail to agree on a process, the parties agree to use the Institute for Christian Conciliation of Billings, Montana (406-256-1583) to establish a mediation/arbitration procedure.
