ANNUAL MATURITY MATRIX AND DEVELOPMENT PLAN 2019/20
INTRODUCTION AND BACKGROUND
This document provides a framework for Primary Care Networks to use to help them identify their level of maturity and their priorities in terms of plans for 2019/20.
Primary Care Networks have been established since July 2019 and nationally it is recognised that some networks will be more mature than others. In many parts of SYB, PCNs build on already established ways of integrated working across general practice, other primary care providers and wider organisations such as community services and social care. Our approach in SYB is to keep building on this momentum, refreshed in the light of our developing 5 year ICS strategy and also reflecting the strategic direction set by the Long Term Plan and GP contract framework. In order to understand where networks are currently at, NHSE have set out the expectation of where networks will be by March 2020:
The National expectation is that by March 2020 Primary Care Networks will;
Have an idea of where they are aiming to get to, know where they are on that journey and the actions they need to take, demonstrating progress toward the next level of maturity.
Be functioning effectively as teams and have made use of additional roles
Have worked on or are working on a service improvement project of some kind – as a way of bringing organisations within networks together on practical changes in the delivery of care that will make a difference for patients, populations and staff, rather than focussing only on the organisational design of networks.
Have formed links with other local partners, such as community service providers and other NHS organisations, local authorities and the voluntary sector, and have started to play a role at both place and system level
Have used a diagnostic process to establish development needs, using a maturity matrix or similar tool
Have an iterative development plan in place, and have started implementing that plan – there isn’t a national format that plans need to take and we will agree an approach that works for networks across SYB
Have worked with community trusts and other relevant partners to ensure staff working in the community are aligned within an MDT for that footprint
Are ready to deliver the new Network Service Specifications

PCN MATURITY MATRIX – HOW TO USE THE MATRIX
Components of the matrix
The matrix should be used pragmatically and flexibly, with networks and their partners viewing PCN development as a multi-year journey, and one that can build on progress that has already been made in improving and transforming care and services for patients and populations. The matrix is set out as a table of components for the development of PCNs and is organised as follows:
•There are four columns showing a development journey over time –organised into Foundation, Step 1, Step 2 and Step 3
•The columns are subdivided into components that PCNs may find it helpful to consider as part of their development journey and components that ICSs and STPs will also want to consider as part of the wider supporting infrastructure that enables network development.
•There are five rows which organise the components into the following:
1. Leadership, planning and partnerships
2. Use of data and population health management
3. Integrating care
4. Managing resources
5. Working in partnership with people and communities

A basis for development discussions
In completing the assessment it is strongly recommended that the Networks involve their partners such as community services, local authority, and other primary care providers, CCG and third sector to get a consensus as to the current maturity of the network and also a wider understanding of what the Network priorities are for the coming year.
Initial discussions may want to reference the maturity matrix and focus on the following questions:
1. Where are you now?2. Where do you want to be in a year?3. How will you get there and what do you need?
Within this discussion, Networks will need to think about the time, capacity and support needed to build sustainable skills and confidence to deliver, which should then help Networks to identify and prioritise the specific support they require from the PCN Prospectus.
To complement the maturity matrix, there is a simple diagnostic spreadsheet tool to help systems understand local PCN maturity, target support and inform any local development plans. The tool enables PCNs to put the matrix ‘into action’.
PROCESS TO BE FOLLOWED
1. Using the self-assessment matrix, discuss with your core Network practices and wider system partners where you think the Network is for each of the maturity components, and select the step that you think your Network is currently working at. It is normal that you may conclude you have components in place at (for example) a blend of the steps. Please note that if recording a component as ‘in place’ this means that it has been implemented and is operational, rather than that it is in plan or in the early stages of progress.
2. Provide evidence to support the categorisation.3. State the level of ambition of the Network4. Identify and prioritise what your development needs are5. Identify what support you would want to support your development needs this year.6. Complete your Network Development Plan based on those domains you wish to focus on – be specific in terms of the types or support and resources that you
would like in order to help you mature as a Network.

APPENDIX B
NETWORK DEVELOPMENT PRIORITIES
This section should be used following the completion of your maturity discussions and the maturity matrix. It is designed to be used flexibly to capture your network development priorities for the coming year along with the resources and support that you would like to access.
The template is not designed to be populated in full – it is designed for you to ‘pick and mix’ what the specific priorities are.
Each of the PCN development support domains has a specific page for you to capture the specific action, timescale, resources and outcomes that you are seeking to achieve.
The plan can then be used as a useful tool to track and report progress within the Network during the coming year.
