You are already your own doctor!Tom Ferguson MD 1985

• Our interactions with the healthcare system are just the tip of the iceberg

• Self Care is and has always been our predominant form of health care

Would you like to see your medical records?

• Not really, why would I want to do that?

• Sell the sizzle not the sausage!

Would you like to be able to………?

• Read what the doctor has written about you

• Order repeat prescriptions

• Have a list of what happened & when

• Get more information about your condition

• Read consultants’ letters

• See your test results

• Check referrals

• etc.

Click here to see more information

about the problems you

suffer with

Test results

• All test results available

• Blood tests

• If regularly tested, can monitor trends

• Understandable, value, graphic, Normal / Abnormal

• Labtests Online

• Comment by doctor

• Can avoid repeating tests

• No need to contact practice

Summary of significant events

• Form filling, insurance & benefits etc.

• Useful when you are asked the same thing over and over again!

• Good to be a reminded of progress

• Links to trusted sites where information is available

Letters, referrals etc

• All communications that go in & out of the practice

• Gives the patient confidence, knows what has been done

• No need to contact practice

• Letters from consultants……supposed to send but often don’t

• Can check understanding and roles in what happens next

• Can use the letters to inform others who don’t communicate

• Examples…..France, Grandson, Yvonne, Margaret

Consultations etc.

• Patient retention of information can be as low as 10%

• Can check understanding & what happens next

• Can get family or friends / carers help

• Language difficulties …… Australia………. Nursing homes

• Elderly man in A&E

• Safe medicine

• No need to contact practice

Mobile Phone app

• Have information available anywhere home and abroad with just 2 passwords! How cool is that?

• A large percentage of the population have a suitable mobile phone

• Information about passwords can be stored under ICE or using an ICE app on the lockscreen

• ICE app is next to the Patient Access records access app

• If someone steals the phone, passwords can be changed

Having access to my medical information has given me………

• More confidence

• Better understanding of my conditions

• More productive conversations leading to shared decision making

• More control

• More independence

• More convenience

• I’m very lucky!

Challenges for Patient Engagement

• Digital Divide

• No interest / lack of interest in their health

• Think it’s too complicated can’t be bothered

• Don’t know about it

• Needs support from practice / staff / clinicians

• Want to be cared for and told

• Worries about external pressures

• Don’t want it…it’s scary, can get hacked

• Lack of understanding of what is possible

• Nothing to see…….Wonderful!

Challenges for Practice Engagement• Who has the time ?

• Where will the funding come from ?

• Will this just build higher expectations that we cannot deliver on?

• What about those who can't do it ?

• If staff themselves don't have it, why should patients ?

• What if patients ask questions about errors, info that is missing etc ?

• Why should practices prioritise this over others ?

• Our notes are not fit to be shared because…..

• I don't mind as long as I do not have to do it….

• This does not fit in with the wider engagement of others….

• It's just too difficult so why bother ?

• I know other practices don't do questionnaires, why should we ?

My motivation• My patients and their carers

• My staff

• My family, my values, my work ethic

• Emotional intelligence

• Puzzles – how can we make things better?

• Always wanted to do the best

• It’s getting tough but it’s about to get a whole lot tougher

• Listening

• Locus of control – internal vs external

• Shared decision making

• Servant leadership

• Success breeds success – 7 habits of highly effective people

• Passion

What can we do to bring more people in?• Get some outcomes from today’s meeting

• Inspire, Believe, feel, Do

• Work with the willing and create the opportunities

• We need to develop leaders and followers from amongst you

• Develop a united patient voice for the “top table”

• Start with one patient, one clinician and one manager in one practice

• Fund them and get out of the way – let them innovate

• Who are the representatives from the system supplier?

• Who are the payer?

• Manage expectations

• Build on what you have, small steps – not giant leaps of faith

• Prepare to fail many times to get to success

• Awareness + Commitment + Passion + Delivery + Place of Work

• Our commitment is to support you

The (7) Ps for Success

•Patients and the Public

•Partnership of Trust

•Paradigm Shift in Healthcare

•Practice makes Perfect
