We will coordinate your care with your Catholic Health physician and other medical providers. This will include securing key medical records to ensure that we provide appropriate medications.

We will also inform your doctors of the results of your assessment and treatment at Horizon and ensure effective two-way collaboration.

We encourage the involvement of your family or significant others in your care. Their involvement, input, support, and active participation in your care are often critical to successful recovery. This participation is also often helpful to family members or significant others who want to help. Of course, their involvement is based on your choice.

You will work with a counselor to establish a treatment plan that addresses your needs and goals for treatment.

Generally, these goals include reducing emotional symptoms (anxiety, depression, etc.) and/or addressing health or other problems associated with the use of alcohol or other drugs.

The treatment plan is designed to help you be more effective and content in your family, work, school, leisure time and general community functioning.

Treatment is provided by a team of behavioral health professionals under the direction of a psychiatrist or psychiatric nurse practitioner.

Each person receives a primary counselor who is responsible to meet with you for counseling and to develop your treatment. The team meets weekly to review client’s progress and to ensure that treatment is effectively coordinated.

Your Catholic Health physician has referred you to Horizon to address the emotional challenges often associated with your medical condition. Both your physician and Horizon recognize that your emotional and physical health is directly related and that your progress in each area supports your progress in both.
