L'ami du Dr House est le Docteur Wilson alors que celui de Sherlock Holmes est le Docteur Watson. De plus, House occupe un appartement n° 221 B qui n'est pas sans rappeler le 221 B à Baker Street où réside Holmes. Mieux encore, les héros sont tous deux dotés d'une intelligence hors du commun, font preuve d'arrogance et ne respectent aucune règle. Grands et minces, ils sont tous deux dépendants de drogues, tous deux musiciens à leurs heures perdues, etc. La liste des points communs est longue et n'est pas le fruit du hasard. David Shore, le créateur de la série, revendique lui-même cette filiation spirituelle. Certes, House est médecin et n'est pas détective.
