providing cardiac services in Ontario. In addition, CCN serves as a support system to

the Ontario Ministry of Health and Long-Term Care. In addition to helping plan,

coordinate, implement and evaluate cardiovascular care in Ontario, CCN is responsible

for developing, maintaining and reporting on the provincial cardiac wait list registry for

all patients waiting for selected adult advanced cardiac procedures in Ontario. In the

role of monitoring and enhancing quality of cardiac services in Ontario, CCN develops

strategies, based on best practices, to better manage cardiovascular disease across

the continuum of care, including strategies to prevent acute hospital readmissions,

decrease demand on emergency departments and decrease the need for initial and

repeat procedures. The Cardiac Care Network of Ontario is funded by the Ontario

STEMI, an acronym for “ST segment elevation myocardial infarction,” is the most

dangerous type of heart attack. In a STEMI, the coronary artery is completely blocked

off by a blood clot, and as a result virtually all the heart muscle being supplied by the

affected artery starts to die

In this new system, when a patient calls 9-1-1, an ambulance will pick them up, and the

crew will be able to tell whether they are having a STEMI heart attack and reroute

the ambulance to a hospital with a Code STEMI Program. In the meantime, a team of

physicians, nurses, and other personnel will be paged immediately to prepare for the

incoming patient. The patient will be brought to the Cath lab, and from there will

undergo a balloon angioplasty to restore blood supply to the heart. From the moment

that the STEMI heart attack is diagnosed, this specialized team hopes to have the

patient in recovery in approximately 90 minutes

Angioplasty (Coronary)

Coronary angioplasty is performed on patients living with blocked coronary arteries. A

catheter with a special balloon is inserted into the artery at the point of blockage. The

balloon is then blown up, compressing the fat or plaque blocking the artery against the

arterial walls, widening the passage. Allows more blood to flow through the arteries

and more oxygen and nutrients to the heart. Also reduces angina and the odds of

having a heart attack

Patients undergo Angioplasties in Trillium's Cardiac Catheterization laboratories and

Measures the function of the chambers of the heart valves, and coronary artery blood

flow. It identifies blockages or narrowing of the arteries caused by a buildup of plaque

or fat. During the procedure, a catheter is inserted into an artery or vein and guided to

the heart. If a blockage is identified, treatment includes medication, angioplasty,

coronary artery bypass surgery, or other interventional techniques. Following a short

recovery period, patients are discharged the same day

Cardiac Diagnostics

Non-invasive cardiac diagnostic services at both the Mississauga Hospital and

Queensway Health Centre

Echocardiography - An echocardiography or 'echo' is an ultrasound of the heart and

shows how strongly a person's heart muscle is moving as well as how the blood flows

through the heart and valves. Echo tests are on an outpatient basis and take

approximately one hour

Nuclear Cardiology Testing - This special type of exercise test looks at the flow of

blood to the heart muscle and shows if the heart is damaged. During the test, a tiny

amount of radioactive material is inserted into a vein and photos are taken of the heart

immediately following the exercise as well as a few hours later. If an individual is not

able to exercise, a drug called Persantine is used to open up the veins and arteries

allowing the physician to examine the flow of blood to the heart as if exercise is

occurring

Holter Monitoring - If a person is suspected of experiencing irregular heart rhythms,

a holter monitor can help determine if the person does indeed have an irregular heart

rhythm. The person wears the monitor for 24 hours while carrying on with routine

activities. When the monitor is returned to the Health Centre, the tape inside the

monitor will contain the information the physician needs to make a treatment decision

Sometimes medication or a pacemaker is prescribed. Other times the decision is made

not to do anything

Cardiac Stress Tests - This test determines if a person's heart is receiving enough

blood and oxygen during exercise. Patients exercise by walking on a treadmill while the

heartbeat and blood pressure are measured

Cardiac Surgery

For people with coronary artery disease, heart valve disease or other cardiac problems,

often the best remedy is open-heart surgery. Open-heart surgery involves the use of a

heart-lung machine that maintains blood flow for the heart and adds oxygen to the

lungs allowing the heart to rest during the operation

New Heart-Lung Bypass Technology – uses the latest heart-lung machine that

contains a new mini-pump – reduces the need for transfusions during and after surgery

– cuts risk of infection and other post-operative complications

Trillium's Cardiac Services perform the following major surgeries:

Coronary Bypass Surgery

Heart Valve Surgery

Beating Heart Surgery

Other types of surgeries performed at Trillium for adults include:

Repair of the atrial septal defect, a hole some people are born with between the

upper chambers of the heart

Aneurysm of the heart muscle

Provides assessment, education, support and personalized exercise plans to patients

recovering from heart attacks, angioplasty, and bypass surgery, and cares for those

recently diagnosed with heart disease

Patients and their families are provided with information and education on heart

health, smoking cessation, stress management, good nutrition, managing medications,

understanding diabetes and vocational counselling. An exercise routine is also developed

for each patient keeping their personal health needs in mind

The goal of this four-month program is to reduce the incidence of repeat cardiac

problems and to improve patients' quality of life. During the first year after a person

has suffered a cardiac arrest they are more prone to suffering a second one than at

any other time

Family members are encouraged to participate in the educational components of the

program. A spousal support program facilitated by a psychologist is also provided.

Patients are accepted into the program six to eight weeks after bypass surgery, and

two to four weeks following a heart attack or angioplasty. All patients must have a

referral from their cardiologist, cardiovascular surgeon or family physician.

The CWRC recently developed an exercise video for patients to use at home

A pacemaker is needed when a person's heartbeat is irregular and the heart's natural

"battery" is not working properly. It helps a person's heart to continue beating. A

person with this condition may experience irregular heartbeats, chest pains, shortness

of breath, dizziness, fatigue, lightheadedness

During this outpatient procedure, a pacemaker is surgically implanted into a person's

chest. The procedure takes approximately one hour to three hours to complete,

depending on the type of implant the person requires
